Autor zachęca do przyjrzenia się różnemu rozumieniu zaburzeń psychicznych oraz celów terapii w zależności od przyjmowanego przez terapeutę paradygmatu. Wskazuje, jak przerwanie terapii wpisuje się w obraz zaburzeń pacjentki oraz odzwierciedla charakter jej motywacji do leczenia.
The article is a case report of a woman with dependent personality disorder treated in individual psychodynamic psychotherapy. The essential background of the treatment is based on an earlier, 20 year long period of a previous therapy, which was conducted in the humanistic approach, in the Gestalt model. The immediate reason to begin the treatment described in the article was for the patient’s worsening of her psychic state. This was after receiving the news of death of the therapist, who led the previous Gestalt therapy for a long time. In the discussion, the author focuses on the analysis of how the differences in diagnosis and ways of treatment depend on the psychotherapeutic paradigm in which the therapist works. The analysis is done on the examples of patients with psychopathology of over-dependence and takes into consideration both the therapeutic paradigms which the reported patient met. In the context of mode of ending the treatment through breaking it off by the patient, weaknesses and shortfalls of the presented process of psychotherapy are mentioned.

Parents who come with their children to psychiatrists pretty often expect to gain not only professional psychiatric help but also educational advice. Sometimes all the complaints reported to a doctor are a picture of the parents’ difficulties in bringing up the child rather than anything else. Those difficulties often increase during the period of adolescence. The goal of the review paper is to describe possible relations between psychotherapy and upbringing as well as the ways therapists can cope with the titled dilemma. Reflections were built above all on the basis of out-patient work with adolescents. The main conclusion is that it seems impossible to separate both types of influences but the fact that they coincide is of great importance for the child and adolescent therapists as being not only a source of new challenges but also of additional responsibility.

Some problems concerning the psychotherapy of patients with the so called microorganic brain damage

The diagnosis of organic mental disorders stands out from a number of other diagnoses used by contemporary psychiatrists in this way that it assumes the knowledge of the aetiology of disorder. Even if this assumption is usually too optimistic, the tradition to establish such diagnoses is firmly consolidated and an example of its presence on the area of child psychiatry is MBD (minimal brain damage/dysfunction). Admittedly, the diagnosis of MBD is from today’s viewpoint rather of historic value but clinical features of this kind are still met in our medical and psychological practice although under other names. The subject of the paper is the influence of so-called microorganicity on the personality development and thus on mental and social functioning of the child and the adult. Besides this, the paper concerns the use of psychotherapeutic treatment in this group of patients and describes specific problems and limitations of such therapy. The issues raised acquire special importance in the light of broadening of our knowledge regarding the influence of psychotherapy on the function, not only of the mind, but also of the brain.
Autor przedstawia koncepcje wpływu tzw. mikroorganicznego uszkodzenia mózgu na przebieg rozwoju człowieka, jego relacje z ludźmi oraz występowanie zaburzeń emocjonalnych. Zwraca też uwagę na specyfikę relacji terapeutycznej tworzącej się podczas leczenia pacjentów z tego typu dysfunkcją w kontekście trudności występujących w procesie ich psychoterapii.

Today more and more often there are prognoses that in the future psychiatry will have been absorbed by neurology. It would be thanks to the stormy progress of research on the neurophysiological, genetic and molecular foundations of mental disorders. The aim of the article is to assess the possibility as well as the supposed consequences of such an evolution of psychiatry. The considerations concern the peculiarity of the object of interest and the methods used in psychiatry in relation to the neurological object and methodology. This way the appraisal of raison d’etre of one common science: neuropsychiatry becomes possible. The question of fundamental importance for the evaluation of similarities and differences between the psychiatric and neurological perspectives is the way the psychophysical issue and especially the problem of the mind - brain relation are approached. The article presents the manners of solving these problems proposed by the contemporary philosophy of the mind. Together with parting with the full of errors and simplifications heritage of Descartes it appears the necessity to regard the presence of subjective mental states both conscious and unconscious in model of mind - brain relation. The example of such a solution is the biological naturalism of John Searle. The psychical life of the man in its subjective dimension remains the peculiar area of interests for psychiatry irrespective of the progress in research on the biological base of mental disorders. The especially valuable cognitive and therapeutic tool in this aspect is psychotherapy constituting the integral part of psychiatry. The present state of knowledge does not indicate that the psychotherapeutic wing of psychiatry can lose its importance and rather somewhat the contrary. The progress of neurobiology does not have to threaten the autonomy of psychiatry by any means and the maintenance of this autonomy depends decisively on the psychiatrists themselves.

The article presents the process of individual psychotherapy of cachectic patient with chronic anorexia nervosa. The therapy was conducted in conditions of the inpatient psychiatric ward for adolescents as a part of treatment during the twenty-four-hour hospitalisation. Understanding of the patient psychopathology as well as phenomena appearing in the time of the psychotherapy process were described in the psychodynamic paradigm. Basic difficulties met by the therapist of chronic patients with eating disorders are mentioned. Particular attention is paid to the situation when the psychotherapy takes place within the inpatient unit and thereupon as integral part of hospitalization has to be only a short episode in the broader plan of long-term treatment.
W artykule prezentowany jest przebieg procesu indywidualnej psychoterapii 18-letniej pacjentki leczonej na oddziale stacjonarnym z powodu wyniszczenia w przebiegu trwającego od kilku lat jadłowstrętu psychicznego. Rozumienie psychopatologii i zjawisk zachodzących w psychoterapii ukazane jest z perspektywy psychodynamicznej. Autor poddaje analizie własne trudności w kontakcie terapeutycznym z pacjentką.
