ObjectivesThis study aimed to assess the surgical outcomes of patients with work-related upper extremity musculoskeletal disorders (UE-MSDs) who failed conservative treatment.Material and MethodsThis was a retrospective study of 17 patients who had work-related UE-MSDs and underwent the following surgeries and follow-up evaluations: decompression, external and internal neurolysis of the upper trunk of the brachial plexus and the long thoracic nerve (LTN), and a partial resection of the anterior and middle scalene muscle. A detailed history of clinical presentation including pain, physical and clinical examinations of the extent of scapular winging (ESW), and upper extremity anatomical postures, such as active forward arm flexion and shoulder abduction, were recorded before and after 3 months of the surgery. Nerve conduction velocity and electromyography examination reports were obtained to assess the sensory or motor loss of the nerve injury before their operation.ResultsAll 17 patients included in this report showed some improvement anatomically in the scapula appearance and functionally in their shoulder movements. More specifically, 9 (53%) patients got a restored to near healthy appearance of the scapula, and 11 (65%) patients recovered a full range of motion, 180° post-surgically. Overall, the mean shoulder flexion and abduction improved to 157±37.5° and 155±40.2° after the surgery from 106±30.2° and 111±34.8°, respectively (p < 0.0001). The ESW score also significantly improved from a mean of 1.5±0.51 to 3.5±0.71 (p < 0.0001). The post-surgical improvements were statistically highly significant (p < 0.0001).ConclusionsDecompression and neurolysis of the upper brachial plexus and the LTN, and the partial release of the scalene muscle contracture procedures have fostered improvements in the shoulder anatomical appearance and movements in all 17 patients.

Opis:
An enzyme-assisted modified steam distillation process was adopted to extract turmeric oil from Curcuma longa L. rhizomes. The diastase, xylose, cellulase, pectinase, and lipase enzymes were used for the pre-treatment of fresh turmeric rhizome to obtain a higher yield by rapturing biological cells. The quantitative and qualitative analysis of turmeric oil was performed by GC–MS. The various influencing parameters for the extraction of turmeric oil such as an enzyme, incubation/pre-treatment time, distillation time have been studied in the present work. The obtained turmeric oil by enzymatic pretreatment process is richer in bioactive/medicinal components than in the other traditional methods. The maximum yield was obtained with cellulase enzyme, which is 25–27% higher than the yield obtained by the traditional hydro distillation process. The detailed qualitative and quantitative analyses are also presented. The present method can be considered energy-efficient, effective, economical, and eco-friendly.

Opis:
Air pollution has a serious impact on the health of human beings and is a major cause of death worldwide every year. Out of the many sources of air pollution, the smoke generated from household combustion devices is very dangerous due to the incomplete combustion of fuel. Women from rural areas suffer a lot due to this harmful smoke. Diseases like cancer, throat, and lung infection occur in adults and children due to inhalation of this smoke. The traditional chulha used by rural women is operated by using cow dung, straw, and wood, and the air is blown manually by using small metallic pipes. This paper presents the design and development of an innovative stove to maximize flame temperature and minimize air pollution to overcome the health-related issues of rural women. A smokeless stove is presented, in which wood, straw, and cow dung are taken as primary fuel, and superheated steam as a secondary oxidizer for its operation. In this stove, a forced draft is created by the provision of a small fan, which is operated by solar power thus eliminating the need of creating a forced draft manually by the cook which makes this innovative stove superior to the traditional chulha. Owing to the provision of superheated steam, the flame temperature as well as the burning efficiency increases. The cooking time is reduced due to higher flame temperature as compared to the liquefied petroleum gas stove. The main objective of this work is to minimize air pollution and provide a smoke-free environment to the people using such devices as this innovative stove offers complete combustion of fuel. The flame temperature of the designed stove ranges from 595°C to 700°C and its thermal efficiency is 10–17% higher than that of the traditional chulha. The design of this stove is unique, and its maintenance cost is also much less.

A recent development in the material studies provides beneficial application of lightweight alloys such as aluminium, magnesium as well as composites and metal matrices. The alloys are experimentally improved by increasing hardness in the ballistics testing using projectiles, makes them viable for the areas such as aerospace, military, defence, automobiles and so on. So the study is made on different approaches. First, by comparing different types of non-ferrous alloys and projectiles regarding sizes, structures. Second, the materials with heat treatment are also studied for investigating the hardness property by overcoming successful penetration on non-ferrous alloys. Third, material to be improvised by use of numerical studies such as 3D models, empirical models and software such as ANSYS, ABAQUS and AUTODYN, etc. Finally, the aim of this paper is to review the recent progress ballistic studies of lightweight materials and to provide a best choice of material for further on-going research.

Opis:
Introduction: To assess the diagnostic accuracy of an integrated approach using conventional ultrasonography, colour Doppler ultrasonography, and elastography strain ratios in tandem in the evaluation of superficial soft tissue lesions. Material and methods: Sixty-five subjects were included in this prospective cross-sectional study. Greyscale features and Doppler parameters were recorded. Strain elastography of the non-vascular and non-cystic lesions was performed and strain ratios were calculated. Fine-needle aspiration or biopsy of all the lesions was performed depending on their site and condition. Inter-rater k agreement was used to determine the strength of agreement between imaging-based diagnosis and histopathological diagnosis. A diagnostic test was used to calculate the sensitivity, specificity, positive predictive value, and negative predictive value. A p value of < 0.05 was considered statistically significant. Results: Multiple superficial soft tissue lesions were studied, the majority of which were lipomas, vascular anomalies, and epidermoid cysts. The diagnostic accuracy was very high and varied from 92.31% to 100% for various masses. The imaging-based diagnosis was in agreement with the histopathological diagnosis in 86.15% (n = 56) and disagreement in 13.85% (n = 9) of the cases (p < 0.007). There was very good inter-rater agreement between the imaging-based diagnosis and histopathological diagnosis (κ = 0.818). Conclusions: The combined use of conventional ultrasonography, colour Doppler, and elastography strain ratios provides a very effective non-invasive tool for the diagnosis of superficial soft tissue lesions and may negate the need for unnecessary biopsies. The advantage of this integrated approach using various ultrasound techniques needs no further emphasis.

Opis:
We present the first results of a systematic search for the rare extragalactic radio sources showing an inverted (integrated) spectrum, with spectral index α ≥ +2.0, a previously unexplored spectral domain. The search is expected to yield strong candidates for α ≥ +2.5, for which the standard synchrotron self-absorption (characterized by a single power-law energy distribution of relativistic electron population) would not be a plausible explanation, even in an ideal case of a perfectly homogeneous source of incoherent synchrotron radiation. Such sharply inverted spectra, if found, would require alternative explanations, e.g. free–free absorption, or non-standard energy distribution of relativistic electrons which differs from a power law (e.g. Maxwellian). The search was carried out by comparing two sensitive low-frequency radio surveys made with sub-arcminute resolution, namely, the Westerbork In the Southern Hemisphere (WISH) survey at 352 MHz and TGSS/DR5 at 150 MHz. The overlap region between these two surveys contains 7056 WISH sources classified as ‘single’ and brighter than 100 mJy at 352 MHz. We focus here on the seven of these sources for which we find α > +2.0. Two of these are undetected at 150 MHz and are particularly good candidates for α > +2.5. Five of the seven sources exhibit a ‘Gigahertz-Peaked-Spectrum’.

Opis:
We have studied three most recent precursor flares in the light curve of the blazar OJ 287 while invoking the presence of a precessing binary black hole in the system to explain the nature of these flares. Precursor flare timings from the historical light curves are compared with theoretical predictions from our model that incorporate effects of an accretion disk and post-Newtonian description for the binary black hole orbit. We find that the precursor flares coincide with the secondary black hole descending toward the accretion disk of the primary black hole from the observed side, with a mean z-component of approximately z_{c} = 4000 AU. We use this model of precursor flares to predict that precursor flare of similar nature should happen around 2020.96 before the next major outburst in 2022.

Opis:
The Tarantula Nebula in the Large Magellanic Cloud is known for its high star formation activity. At its center lies the young massive star cluster R136, providing a significant amount of the energy that makes the nebula shine so brightly at many wavelengths. Recently, young massive star clusters have been suggested to also efficiently produce very high-energy cosmic rays, potentially beyond PeV energies. Here, we report the detection of very-high-energy γ-ray emission from the direction of R136 with the High Energy Stereoscopic System, achieved through a multicomponent, likelihood-based modeling of the data. This supports the hypothesis that R136 is indeed a very powerful cosmic-ray accelerator. Moreover, from the same analysis, we provide an updated measurement of the γ-ray emission from 30 Dor C, the only superbubble detected at TeV energies presently. The γ-ray luminosity above 0.5 TeV of both sources is (2–3) × 10$^{35}$ erg s$^{−1}$. This exceeds by more than a factor of 2 the luminosity of HESS J1646−458, which is associated with the most massive young star cluster in the Milky Way, Westerlund 1. Furthermore, the γ-ray emission from each source is extended with a significance of >3σ and a Gaussian width of about 30 pc. For 30 Dor C, a connection between the γ-ray emission and the nonthermal X-ray emission appears likely. Different interpretations of the γ-ray signal from R136 are discussed.
