Opis:
The present research article deals with the study of a boundary value problem of a one-dimensional semi-infinite hygro-thermoelastic rod of length l. The deformation of the rod is under consideration when the left boundary of the hygro-thermoelastic rod is subjected to a sudden heat source. The solutions of the considered variables are decomposed in terms of normal modes. Analytical expressions of displacement, moisture concentration, temperature field, and stresses are obtained and presented graphically for different periods. By studying the one-dimensional thermal shock problem for a semi-infinite hygrothermoelastic rod, the authors aim to gain insights into the fundamental behavior of materials subjected to rapid temperature changes and moisture effects.

Opis:
With continuous miniaturization of modern electronic components, the need of better cooling devices also keeps on increasing. The improper thermal management of these devices not only hampers the efficiency but can also cause permanent damage. Among various techniques, microchannel heat sink has shown most favourable performance. To further enhance the performance, two techniques i.e., active and passive are used. In passive technique, no external power source is required like heat sink design alteration and working fluid modification. External power source is necessary for heat transfer augmentation in the microchannel heat sink when using the active approach. Due to compact size of microchannel, active techniques are not used more often. However, the present work highlights the different active technique used in microchannel i.e., Electrostatic forces, flow pulsation, magnetic field, acoustic effects, and vibration active techniques. Above mentioned techniques have been analysed in detail.

Aneurysm of mid and apical interventricular cardiac septum dissecting along the basal part : an uncommon entity diagnosed with CT angiography

BACKGROUND: Aneurysm of the muscular interventricular septum is a rare entity as compared to the membranous part. Only a few cases of dissecting septal aneurysm have been reported in literature. Two-dimensional echocardiography is the initial diagnostic modality with ECG-gated CT and MRI being non-invasive imaging modalities for comprehensive evaluation. The complications can arise from chronic pressure erosion of the intervening septal myocardium, leading to left-to-right shunting in the form of ventricular septal defect and paradoxical thromboembolism. Radiologists should be aware of imaging findings of interventricular septal aneurysm, because of its rarity of occurrence and complications. CASE REPORT: A 48-year-old male patient presented to a cardiology department with complaints of intermittent chest pain, palpitations and exertional dyspnoea. CT angiography revealed a wide-mouth large aneurysm arising from the mid and apical portion of the interventricular septum dissecting into the basal part. There was associated significant bowing (>15 mm) of the septum and mild obliteration of the right ventricular cavity. Myocardium surrounding the aneurysm was identified with no associated ventricular septal defect (VSD). No evidence of intraventricular clot was found. Catheter angiography confirmed the CT angiographic findings. CONCLUSIONS: Radiologists should be aware of imaging findings of interventricular septal aneurysm, because of its rarity of occurrence, complication in the form of thromboembolism, dissection and intracardiac shunting and mass effect over adjacent cardiovascular structures. Careful scrutiny is essential to avoid labelling of these cases as cardiac masses.

Opis:
Being a biodegradable polymer, poly(lactic acid) (PLA) based composites receive greater preference over nonbiodegradable plastics. Poly(lactic acid) has to find its place in various applications such as polymer composites, agriculture, biomedical, etc. Polymer composites based on PLA possess comparable mechanical strength, endurance, flexibility and endures future opportunities. Several combinations of natural fibers and filler-based PLA composites have been fabricated and investigated for physical and mechanical changes. Moreover, several biopolymers and compatibilizers are added to PLA to provide rigidity. The paper presents a tabulated review of the various natural fiber/filter-based PLA composites and the preparation and outcomes. In addition, enhancement made by the reinforcement of nano filler in the PLA are also discussed in brief. The significance of PLA in the biomedical application has been discussed in brief. The paper also shed lights in the social and economic aspects of PLA.

Introduction: Blunt trauma chest contributes to significant number of trauma admissions globally and is a cause of major morbidity and mortality. Many scoring systems and risk factors have been defined in past for prognosticating blunt trauma chest but, none is considered to be gold standard. Aim: This study was conducted to reassess the significance of available scoring systems and others indicators of severity in prognosticating blunt trauma chest patients. Materials and Methods: In this prospective observational study from November 2016 till March 2018, 50 patients with age more than 12 years with blunt chest trauma who required hospitalization were included. Nine risk factors were assessed namely-age of the patient, duration of presentation after trauma, number of ribs fractured, bilateral thoracic injury, evidence of lung contusion, associated extra thoracic injury, need for mechanical ventilation, Revised trauma Score (RTS) and Modified Early Warning Sign Score (MEWS). Severity of blunt thoracic trauma was assessed on following outcomes-SIRS, ARDS and Death. The inferences were drawn with the use of statistical software package SPSS v22.0. Results: The age of 50 patients included in our study with a range of 15 to 76 years, the median age was 35.5 years. Statistically significant association was observed between occurrence of SIRS and multiple ribs fractured (p-value- 0.049), associated extra-thoracic injury (p-value-0.016) and higher MEWS score (p-value-0.025). ARDS occurrence was statistically significantly associated with all the risk factors except age. Death occurred more in patients with delayed duration of presentation to hospital (p-value <0.001), multiple ribs fractured (p-value-0.001), bilateral thoracic injury(p-value<0.001), associated extra-thoracic injury (p-value-0.004), patients who required ventilatory support (p-value<0.001), low RTS (p-value-0.006) and high MEWS (p-value-0.005) on admission. This association was found statistically significant. Conclusion: High MEWS, associated extra-thoracic injuries and multiple rib fractured can very well predict poor outcome in terms of SIRS, ARDS and death. Aggressive treatment protocols should be established for better outcome in these patients with blunt trauma chest.

BACKGROUND: Delayed presentation of post-traumatic aortic pseudoaneurysm and its fistulous communication with the right renal vein is a very rare entity. Most of the cases described in literature are due to abdominal aortic aneurysm (AAA) rupture into the left renal vein. To the best of our knowledge, communication with the right renal vein has not been described in published literature. Our patient also had a circumaortic renal collar, which is a rare renal vein anomaly. Aortic pseudoaneurysm, its fistulous communication with the right renal vein and circumaortic renal collar in a single patient is of extremely rare occurrence. CASE REPORT: A 29-year-old male presented to the cardiology department with complaints of breathlessness, abdominal pain and hematuria for the last 6 months. On clinical examination there was evidence of audible bruit over the abdomen. He had a past history of a gunshot injury around two years back. CT angiography revealed a large partially calcified pseudoaneurysm arising from the right lateral wall of the abdominal aorta with the neck of the pseudoaneurysm at juxtarenal location with a fistula between the anterior wall of the pseudoaneurysm and the posterior wall of the right renal vein. There was an associated incidental finding of circumaortic left renal vein with gross aneurysmal dilatation of both pre- and retro-aortic part of the renal vein. CONCLUSIONS: Delayed presentation of aortic pseudoaneurysm with its fistulous communication with the right renal vein is a rare entity. CT angiography is a non-invasive modality for diagnosis of the exact site of communication, length of aneurysm, proximal and distal extent of the affected segment and its relationship with surrounding structures.

There is no doubt that the miniaturization of various electronic devices, including processors, servers, micro-electromechanical system devices, etc. has resulted in increased overall performance. However, there is a major problem with thermal management in these devices, as well as in many others. One of the most promising solutions is liquid cooled microchannel heat sink. In the current work, different cases of open micro pin-fin configurations of heat sink were considered. The configurations considered were a uniform height micro pin-fin heat sink, three-stepped unidirectional micro pin-fin heat sink and three-stepped bi-directional micro pin-fin heat sink. These configurations were also oriented in two dissimilar fashions, i.e. inline and staggered, so the total of six heat sink configurations are compared and analysed. Using single phase water as a coolant and copper as a substrate, these configurations were simulated numerically for different Reynolds numbers (10−160) under heat flux of 500 kW/m2 . It can be concluded that at low Reynolds numbers, steepness does not contribute much in both inline and staggered arrangements, while at higher Reynolds numbers, 3 stepped staggered configurations has revealed the best performance due to boosted fluid mixing and more projecting secondary flow. Furthermore, bi-directionality in steepness shows augmented performance only in inline arrangement.

Background The burden of cardiovascular diseases (CVDs) remains unclear in many regions of the world. Objectives The GBD (Global Burden of Disease) 2015 study integrated data on disease incidence, prevalence, and mortality to produce consistent, up-to-date estimates for cardiovascular burden. Methods CVD mortality was estimated from vital registration and verbal autopsy data. CVD prevalence was estimated using modeling software and data from health surveys, prospective cohorts, health system administrative data, and registries. Years lived with disability (YLD) were estimated by multiplying prevalence by disability weights. Years of life lost (YLL) were estimated by multiplying age-specific CVD deaths by a reference life expectancy. A sociodemographic index (SDI) was created for each location based on income per capita, educational attainment, and fertility. Results In 2015, there were an estimated 422.7 million cases of CVD (95% uncertainty interval: 415.53 to 427.87 million cases) and 17.92 million CVD deaths (95% uncertainty interval: 17.59 to 18.28 million CVD deaths). Declines in the age-standardized CVD death rate occurred between 1990 and 2015 in all high-income and some middle-income countries. Ischemic heart disease was the leading cause of CVD health lost globally, as well as in each world region, followed by stroke. As SDI increased beyond 0.25, the highest CVD mortality shifted from women to men. CVD mortality decreased sharply for both sexes in countries with an SDI >0.75. Conclusions CVDs remain a major cause of health loss for all regions of the world. Sociodemographic change over the past 25 years has been associated with dramatic declines in CVD in regions with very high SDI, but only a gradual decrease or no change in most regions. Future updates of the GBD study can be used to guide policymakers who are focused on reducing the overall burden of noncommunicable disease and achieving specific global health targets for CVD.

BACKGROUND: Although the rising pandemic of obesity has received major attention in many countries, the effects of this attention on trends and the disease burden of obesity remain uncertain. METHODS: We analyzed data from 68.5 million persons to assess the trends in the prevalence of overweight and obesity among children and adults between 1980 and 2015. Using the Global Burden of Disease study data and methods, we also quantified the burden of disease related to high body-mass index (BMI), according to age, sex, cause, and BMI in 195 countries between 1990 and 2015. RESULTS: In 2015, a total of 107.7 million children and 603.7 million adults were obese. Since 1980, the prevalence of obesity has doubled in more than 70 countries and has continuously increased in most other countries. Although the prevalence of obesity among children has been lower than that among adults, the rate of increase in childhood obesity in many countries has been greater than the rate of increase in adult obesity. High BMI accounted for 4.0 million deaths globally, nearly 40% of which occurred in persons who were not obese. More than two thirds of deaths related to high BMI were due to cardiovascular disease. The disease burden related to high BMI has increased since 1990; however, the rate of this increase has been attenuated owing to decreases in underlying rates of death from cardiovascular disease. CONCLUSIONS: The rapid increase in the prevalence and disease burden of elevated BMI highlights the need for continued focus on surveillance of BMI and identification, implementation, and evaluation of evidence-based interventions to address this problem.
