Opis:
This paper explores the implications of social and economic changes in the ‘Global South’ of the World. In particular, we examine case studies of Japan and China and the impact of populational aging on their economic policies and social practices. Key examples of uneven distributions of, or access to, opportunities have the potential to give rise to further social or economic tensions. Whilst the scholarly base is expanding, more is to be done to ascertain the characterization of inequalities. Indeed, if these substantive issues are to be addressed comprehensively, the key then is to move beyond a Western academic paradigm, and to purposefully involve critical scholarship from intellectuals from the Global South. Doing so will add a vitality of experience in discussing how economic growth is, or may not be coupled with, inequality.

Opis:
In contemporary history, the use of gendered treatments for women with mental health issues in the psychiatric secure estate is an issue of major concern in Great Britain. This paper examines women and gender in the psychiatric secure estate from a structural analysis drawing influence from Connell’s (1987) theoretical and conceptual work on hegemonic masculinity. Bio-psychological approaches have almost dominated academic discussion in relation to women’s incarceration and there is an reflexive need to develop other sociological frameworks on hegemonic masculinity because dominant bio-psychological models have failed to identify underlying configurations which combine to oppress women whilst simultaneously reproducing consequences of masculinity and power within institutional structures.

Opis:
The current investigation aimed to examine the effects of different orthotic conditions on the biomechanical mechanisms linked to the aetiology of chronic pathologies using musculoskeletal simulation. Methods: 16 male and 20 females ran over an embedded force plate at 4.0 m/s, in five different conditions (medial, lateral, no-orthoses, semi-custom and off the shelf). Kinematics of the lower extremities were collected using an eight-camera motion capture system and lower extremity joint loading also explored using a musculoskeletal simulation approach. Differences between orthoses conditions were examined using 2 2 mixed ANOVA. Results: External instantaneous load rate was significantly reduced in the off the shelf orthoses (male = 1290.60 and female = 1567.10 N/kg/s), compared to the medial (male = 1480.45 and female = 1767.05 N/kg/s) and semi-custom (male = 1552.99 and female = 1704.37 N/kg/s) conditions. In addition, peak patellofemoral stress was significantly lower in the off the shelf orthoses (male = 68.55 and female = 94.91 KPa/kg) compared to the lateral condition (male = 70.49 and female = 103.22 KPa/kg). Finally, peak eversion angles were significantly attenuated in the medial orthoses (male = –6.61 and female = –7.72 deg) compared to the lateral (male = –9.61 and female = –10.32 deg), no-orthoses (male = –8.22 and female = –10.10 deg), semi-custom (male = –8.25 and female = –9.49 deg) and off the shelf (male = –7.54 and female = –8.85 deg) conditions. Conclusions: The current investigation shows that different orthotic devices/ configurations may provide distinct benefits in terms of their effectiveness in attenuating the biomechanical parameters linked to the aetiology of chronic running injuries.

Background and Objectives: The efficacy of deep brain stimulation of the anterior nucleus of the thalamus (ANT DBS) in patients with drug-resistant epilepsy (DRE) was demonstrated in the double-blind Stimulation of the Anterior Nucleus of the Thalamus for Epilepsy randomized controlled trial. The Medtronic Registry for Epilepsy (MORE) aims to understand the safety and longer-term effectiveness of ANT DBS therapy in routine clinical practice. Methods: MORE is an observational registry collecting prospective and retrospective clinical data. Participants were at least 18 years old, with focal DRE recruited across 25 centers from 13 countries. They were followed for at least 2 years in terms of seizure frequency (SF), responder rate (RR), health-related quality of life (Quality of Life in Epilepsy Inventory 31), depression, and safety outcomes. Results: Of the 191 patients recruited, 170 (mean [SD] age of 35.6 [10.7] years, 43% female) were implanted with DBS therapy and met all eligibility criteria. At baseline, 38% of patients reported cognitive impairment. The median monthly SF decreased by 33.1% from 15.8 at baseline to 8.8 at 2 years (p < 0.0001) with 32.3% RR. In the subgroup of 47 patients who completed 5 years of follow-up, the median monthly SF decreased by 55.1% from 16 at baseline to 7.9 at 5 years (p < 0.0001) with 53.2% RR. High-volume centers (>10 implantations) had 42.8% reduction in median monthly SF by 2 years in comparison with 25.8% in low-volume center. In patients with cognitive impairment, the reduction in median monthly SF was 26.0% by 2 years compared with 36.1% in patients without cognitive impairment. The most frequently reported adverse events were changes (e.g., increased frequency/severity) in seizure (16%), memory impairment (patient-reported complaint, 15%), depressive mood (patient-reported complaint, 13%), and epilepsy (12%). One definite sudden unexpected death in epilepsy case was reported.
