Opis:
Aortic stenosis is the most common acquired valvular heart disease. Aortic stenosis has growing prevalence in people older than 75 years and natural course of disease is characterized by high mortality rate. According to epidemiological data all patients with aortic stenosis will die after 2–5 years from the first signs of disease if not undergo aortic valve replacement. However, even 40% of patients do not have surgery because of comorbidities related to advanced age. This was the main reason why in 80- and 90-ties of XX century there were intensive attempt to developed an alternative, less invasive treatment methodology for people with aortic stenosis and comorbidities and at high surgical risk. Transcatheter aortic valve implantation (TAVI) was introduced by Dr Alain Cribier in February 2nd, 2002 in Rouen, France. Since that day different aortic transcatheter bioprostheses were used in many randomized clinical trials comparing their safety and effectiveness versus surgical aortic valve replacement. Gradually, it became clear that in all older patients in all risk groups TAVI was equally or even more safe and effective than surgery. Complications after TAVI are relatively rare, but some of them are life-threatening. Heart Team plays a key role in patients selection to TAVI.

Opis:
This year the new international guidelines of WIST (The World Federation for Interventional Stroke Treatment) on the physician’s training in the interventional acute stroke treatment were published. WIST multispecialty guidelines outline competency and quality standards for physicians and centers to perform safe and effective endovascular stroke treatment with mechanical thrombectomy. The role of quality control and quality assurance is highlighted. WIST establishes an individualized approach to acquiring clinical knowledge and procedural skills to meet the competency requirements for certification of interventionalists of various disciplines and stroke centers in endovascular treatment. WIST guidelines encourage acquisition of skills using innovative training methods such as structured supervised high-fidelity simulation and procedural performance on human perfused cadaveric models. In Poland in 2018 a pilot program on mechanical thrombectomy was established, however endovascular acute stroke treatment is still underused due to limited patients access to stroke centers and too small number of operators.

Nieinwazyjne metody obrazowania serca pozwalają ocenić żywotność miokardium (ocena wskazań do zabiegu) oraz ułatwić wybór strategii zabiegu i sprzętu stosowanego podczas przezskórnej interwencji wieńcowej (ang. percutaneous coronary interventions, PCI). Przedstawiamy opis zabiegu u pacjenta poddanego PCI przewlekłej okluzji tętnicy wieńcowej po diagnostyce metodami nieinwazyjnymi, w tym po wykonaniu wielorzędowej tomografii komputerowej (multi-slice computed tomography, MSCT).
Non-invasive diagnostics of the heart allow one to assess myocardium viability (indication for procedure) and facilitate the choice of strategy and tools used during percutaneous coronary intervention (PCI). We present a case of PCI of chronic total occlusion (CTO) after non-invasive diagnostics, including multi-slice computed tomography (MSCT).

The Polish Interventional Cardiology TAVI Survey (PICTS) : adoption and practice of transcatheter aortic valve implantation in Poland

Introduction: Few studies have assessed the development of transcatheter aortic valve implantation (TAVI) in Poland since its introduction in 2008, and data on current TAVI activity or practice are missing. Aim: To assess the dynamics of TAVI adoption in Poland and to detect differences among Polish centres in TAVI practice and decision-making. Material and methods: The Polish Interventional Cardiology TAVI Survey (PICTS) was approved by the Polish Association of Cardiovascular Interventions and presented to all 21 national TAVI centres. Between 2008 and 2015 the cumulative number of TAVI performed in Poland was 2189. The annual number of TAVI rose from 8 in 2008 to 670 in 2015 (0.21 to 17.4 implants per million inhabitants, respectively). Results: The median TAVI experience per centre was 80 procedures (95% CI: 38.1–154.6). In 2015 the TAVI penetration rate reached 5.12% of the estimated eligible Polish population. Inoperable and high-risk patients are treated with TAVI in all centres, with 52% of Heart Teams also qualifying medium-risk patients. The rate of transfemoral implantations increased to 83.2% of all procedures in 2015, while transapical implantations decreased to 12%. The frequency of subclavian, direct aortic or transcarotid routes in 2015 was below 3% each. Conclusions: The PICTS survey observed a positive but slow rate of adoption of TAVI in Poland. When compared to Western European countries, our findings highlight a significant treatment gap in high or prohibitive surgical risk patients with severe aortic stenosis. Remarkable variations in TAVI practices among Polish TAVI centres warrant publication of joint national guidelines and recommendations.
