Opis:
Purpose: To establish the relation between the reasons one registers to a primary care physician and a patient's expectations of and satisfaction from the medical services provided. Material and methods: A total of 422 patients from public and non-public health care centres participated in the research. The research was carried out in 20 institutions of public and non-public centres for primary care in the Swiętokrzyskie Voivodeship. A Patient Request Form was used as the research tool. The Polish version of the Patient Request Form (PRF) is composed of 18 statements concerning different reasons for the present contacting of a general practitioner. Comparative analysis of interval or ratio scale type variables was performed using variance analysis, which in the case of significance of the main effects or interactions was supplemented by post-hoc analysisResults: After a repeated measures analysis of variance was done, a significant variation was found in terms of the type of institution: a public and non-public health care centre (p=0.01955). The effect of the type of expectation also proved significant (p=0.00000). A significant interaction was also found between the type of institution and the kind of PRF (Patient Request Form) (p=0.00985). A significant effect (p=0.00805) was obtained using single-classification analysis of variance upon analysing the assessment of the treatment conditions at a primary care clinic (JUM2) in terms of the type of institution.Conclusions. Patients participating in the study indicated expectations related with an explanation of the illness and obtaining information about test results and further treatment as the reasons for the present registration to a primary care physician. As regards assessment of the treatment conditions (JUM2) at primary care clinics in view of the type of institution and area of services, the patients participating in the study assessed non-public health care centres more highly.

A number of regenerative materials are currently used to regenerate or preserve the alveolar process. One of these is autogenous dentin matrix. With many valuable properties such as easy availability, simple preparation, low cost, low risk of disease transmission and no risk of triggering an immune response against the graft, autogenous dentin matrix appears to be a very good material of choice. The following article is intended to provide an overview of the use of autogenous dentin matrix.

Does the application of autologous injectable Platelet-Rich Fibrin (i-PRF) affect the patient’s daily performance during the retraction of upper canines? A single-centre randomized split-mouth controlled trial

Background Previous studies have assessed different aspects concerning the applications of i-PRF in the oral cavity. However, nothing is known regarding patients’ perceptions of the injection of autologous platelet-rich fibrin (i-PRF). Objectives To investigate patients’ perceptions after injecting platelet-rich fibrin (i-PRF) in the course of retracting upper canines. Methods Twenty-one patients, whose treatments required extractions of both upper first premolars, were recruited. Extraction side was randomly allocated to the intervention or control sides. After the alignment phase, i-PRF was injected twice with a one-month interval on the buccal and palatal aspects of the extraction sites (intervention side). Patients’ perceptions were evaluated with two questionnaires: the first was used to assess the level of pain, discomfort, swelling, eating and swallowing difficulties as well as jaw movement restriction after 1 h (T1), 2 h (T2), 6 h (T3), 24 h (T4) and 48 h (T5) of the second injection; the second questionnaire was used to assess the acceptance of the i-PRF injection and overall satisfaction with this technique at the end of canine retraction phase. Visual Analogue Scale (VAS) was adopted for this purpose. Wilcoxon Signed Rank Test was used to compare between both sides at all time points while Friedman’s Test was the selected test for detecting variables’ changes over time. Post-hoc Wilcoxon Matched-Pairs Signed-Rank Tests were applied when any of the results were significant. As to the multiplicity of tests, Bonferroni Correction was implemented. Results Pain and swelling levels were significantly higher on the experimental compared to the control sides at T1, T2, and T3 (P < 0.05), whereas they declined sharply and went back to almost normal values at T4 (after 24 h). At T5 they were 0. Discomfort and difficulty in mastication on intervention sides were significant only at T1 and T2. Pain, swelling, and chewing difficulties were significant (P < 0.001) during the 4 assessed time points. The increase was insignificant regarding swallowing difficulties and jaw movement limitations at all time intervals. Conclusions Injecting autologous (i-PRF) during orthodontic canine retraction is a well-perceived and well-tolerated method due to the limited discomfort which significantly diminishes 24 h afterwards.

Opis:
High-repetition-frequency Q-switched laser is realized through adopting a Nd:LaMgAl11O19 (Nd:LMA) disordered crystal as the gain material, a laser diode lasing at 796 nm as the pumped source, and a semiconductor saturable absorber mirror (SESAM) as the Q-switched device. The out-put characteristics are analyzed under using different transmittance T plane mirrors as an output coupler. Without adopting SESAM, the laser is operating at a CW state, and a relatively high transmittance is helpful for achieving high output power, slope efficiency and light-to-light efficiency. ForT = 7.5% and an absorbed power of 6.17 W, the output power arrives at its maximum of 1160 mW,and the corresponding slope efficiency and light-to-light efficiency are 20.71% and 18.78%, respectively. After introducing SESAM into the cavity, the laser operates at a passively Q-switched state, and the largest slope efficiency is 13.14% under T = 5.0%. Adopting five different output couplers, with the increase of the absorbed power, the pulse repetition frequencies, the pulse energies and the peak powers will ascend while the pulse widths will decline. The observed narrowest pulse width, the maximum pulse repetition frequency, the highest pulse energy and peak power are 1.745 μs, 175.88 kHz, 3.21 μJ and 1.84 W, respectively.
