Active ageing index in Russia : identifying determinants for inequality

This paper is aimed at the development of a tool analysing the AAI results for the Russian older citizens from different population groups, as well as at identifying factors underlying the inequalities in active ageing outcomes by calculation the AAI on the national and individual levels. The adaptation of the methodology of the AAI to the individual-level data and the limitations of the approach are explicitly explained. The older generations of Russia show relatively high levels of education, financial security and engagement in family care, especially in the care to children. The most significant potential for development have employment, volunteering, political engagement, physical activity, lifelong learning and use of the Internet. The calculation of the AAI at the individual level has revealed significant inequalities in the degree of realisation of potential in different areas of active ageing. The results of the project provide scientific evidence for the implementation of policy measures in the target groups. The high correlation of the index values with human capital indicators (health and education) underlines the importance of the early interventions aimed at promoting and supporting human capital at the earlier stages of the life course till the old age. The substantial positive connection of employment with other forms of activity stresses the necessity of developing a package of activation policy measures aimed at the retention of older adults in the labour market. At the same time, the statistical analysis showed the absence of a "dilemma of choice" between certain types of activity of the older generation, for example, between caring for grandchildren and employment, or employment and volunteering - the potential in different areas may be increased simultaneously.

The second transition manifesting in the preference of informal relationships, decreased death rate, longer life, increased migration, strive for maximum use of education, income, and increased life standard and quality contribute to the decreased birth rate (below the level ensuring linear replacement of generations) lids to ageing of population. It is a process in which changes occur in population age structure and an increase in the number and proportion of older people (especially people aged over 65) in the general population, as well as a decrease in the number of young people This process leads to outcomes which, apart from demographic implications, are of socioeconomic significance and influence changes in the structure of consumption, income and savings distribution, decrease in labour productivity, changes in household structure, healthcare and social care for the elderly. This is why questions resulting from ageing of the population must be an element of adequate social policy. The aim of the paper is an evaluation of Poland’s population ageing by regions, in 1995-2035 years.

Opis:
The progressive ageing process concerns both Poland and other Member States of the European Union. In recent years, the share of workers of non-mobile and post-productive age in the total population has been rising, and according to forecasts, in 2035 people aged 45 years and over will represent two-thirds of our society. Since the year 2012, announced the Year of Active Ageing and Solidarity between Generations by the European Commission, more and more attention has been paid in the Polish public space to the issues of an ageing society, including its impact on the health of the economy. The article notes that the ageing process does not occur uniformly across all Polish regions, i.e. at the same pace and with the same intensity. The results of studies indicating the regions being most vulnerable to the ageing of the regional community and the negative consequences of this process in the next two decades are presented.

The main aim of the article was to review measures of the population ageing process. In the first part, traditional (conventional) measures of population ageing are presented (i.e. old-age rate, median age, ageing index, dependency ratios, potential support ratio, parent support ratio). These measures are based on chronological age and give a coherent picture of population ageing. However, they do not take into account changes in life expectancy. In other words, they are based on the assumption that a 65-year-old person in 2000 is considered to be just as old as a 65-year-old person in 1900. But the probability of surviving in these two years is different: the remaining life expectancy at age 65 was much higher in 2000 than in 1900. Therefore, the second part of the article presents the new (alternative) measures of population ageing, which are adjusted to life expectancy. In particular, it describes the following measures: proportion of the population in age groups that have a remaining life expectancy of 15 years or less (Prop. RLE15-), prospective median age, population average remaining years of life, and prospective old-age dependency ratio. In the last section, the results of a comparison between conventional and alternative measures are presented. The new measures indicate a slower pace of population ageing in the future than the traditional ones. Thus, the measures recommended for designing governmental programmes aimed at the elderly are the new measures, which take into account life expectancy.

There are two ageing processes, one physiological, also known as normal ageing, and that of pathological ageing, which depends on the occurrence of chronic diseases. The essence of the former is a gradual, progressive, and a very indivi­dual restriction of the organs’ functional reserve with age. The changes occur in all cells, tissues and systems and do not affect each organ at the same time, and the rate of change can vary between organs. Therefore, the chronological age alone cannot be the factor determining the terapeutic decisions, including surgical treatment. Among the elderly, there is a distinct situation where acute stress response associated with surgery is imposed on the otherwise ageing-related, reduced physiological reserves and the cumulative effect of any accompanying diseases. Standard preparation for surgery and routine perioperative management in such patients can lead to serious complications. In older persons, even mini­mal injury can exceed the body’s capacity to compensate, especially among those with frailty syndrome. It is critical to understand the physiological changes associated with ageing to better understand the differences in the management of these elderly patients.
