Evaluation of neurotherapy for a patient with chronic impaired self-awareness and secondary ADHD after severe TBI and long term coma using event-related potentials

We examined the effectiveness of neurotherapy for chronic, impaired self-awareness and secondary ADHD (SADHD) diagnosed in the patient after severe TBI. We hypothesized a good response to relative beta training. Patient A.A., age 30, after severe TBI and long-term coma, suffered from chronic, impaired self-awareness and SADHD, manifesting two years post trauma. Only slight progress was made after behavioral training. The patient took part in a neurotherapy program consisting of 40 sessions of relative beta training. We used standardized neuropsychological testing, as well as ERPs before and after the completion of neurotherapy. At baseline, A.A. showed an excess of relative mu-rhythm, possibly associated with ADHD (alpha subtype); decreased beta generated centrally, indicating hyperactivation of the central medial cortical area and possibly associated with anxiety; and a decreased visual-related component, an indicator of TBI. There was no significant improvement of the P300 NOGO component after the conclusion of the neurotherapy program. However, as hypothesized, she showed improvements in cognitive parameters, especially attention, memory and executive function, including remission of the impaired self-awareness and SADHD. The patient finished her studies and has now started working. A neurofeedback program using relative beta-theta training produced slight physiological changes in our TBI patient, but major cognitive and behavioral changes, including reduced impaired self-awareness and SADHD. ERPs can be used to assess SADHD and functional brain changes induced by neurotherapy, but has limited application for interpreting the brain mechanism of chronic, impaired self-awareness after severe TBI and long term coma.

Objectives The aim of this qualitative research study was to explore the barriers encountered by Ukrainian war migrants and refugees in accessing vaccinations in Poland and the challenges related to delivering vaccinations observed by healthcare professionals (HCPs). Material and Methods The study was based on an analysis of data from 18 in-depth interviews with HCPs working with Ukrainian refugees conducted in Poland in July and August 2023. The authors analyzed the data using the UNICEF Journey to Health and Immunization (JHI) framework in order to identify bottlenecks and possible interventions that could solve existing problems in preventive healthcare for migrants. Results This qualitative study revealed that at each stage of the JHI, there were challenges related to vaccinating Ukrainian children in Poland, which were similar to those experienced in other countries – gaps in routine immunizations and the need to fill these gaps by ensuring refugee populations are fully included in routine immunization in the host country. The work environment, training, and communication with the Ukrainian mothers contribute to HCPs’ engagement in increasing vaccine uptake among their patients. The HCPs’ attitudes, skills, and experiences impacted their interactions with patients and participation in the immunization process. Healthcare professionals observed that the mother’s journey was influenced by vaccine literacy level, competing priorities, individual barriers of access (e.g., language barrier, costs), as well as feelings associated with the decision to vaccinate a child, including worries about vaccine safety. The surrounding cultural norms, social support, and past experiences with the Ukrainian health system also influenced decisions on vaccinations. Conclusions Overcoming barriers related to vaccinations requires a comprehensive approach, starting with expanding HCPs’ knowledge about migrants’ rights to health services, including vaccinations, improving communication between patients and HCPs, building vaccine literacy/trust in vaccinations, and achieving vaccination coverage through tailored and flexible systemic solutions.

In frame of the European Project CHANGE (Care of Health Advertising New Goals for Elderly people) the study of health behaviours regarding healthy nutrition and physical activity was conducted on patients in age of 60 and more covered with care of community nurses. Objectives: To assess association between physical activity and nutritional habits, stages of change of these behaviors among older patients and other psychosocial conditions regarding planning of health education intervention to change their health behaviours. Methods: 108 persons at age avg. 69.6 (SD 60–87) were interviewed by questionnaire (assessing the frequency of intake of particular food products, physical activity, motivation and stage of change these behaviors, satisfaction with physical condition and general wellbeing) by the trained community nurses. Results: Results showed sedentary style of life in most studied patients. Among physical activity the frequency of daily walking was the highest (69%), while 70% of older people did not exercise. In study group 42% were not aware of importance of physical activity for their health. But the awareness of importance of healthy nutrition was high, and about half of study group (48%) reported that they eat meals with reduced fat, and 80% eat high fiber food. The rules of healthy nutrition were not fully respected. Positive correlations were shown between intention to change, stage of change of health behavior and performance of physical activity and healthy eating. Those behaviors were positively related to well-being and satisfaction with physical condition. Conclusions: Health promotion programs for older patients should be planned in different way depending on their stage of change of health behaviors. There is a need to raise awareness of importance of physical activity, and to educate older patients about recommended types of exercises and recreation. In case of recommendations of healthy nutrition, the awareness is high and performance definitely more frequent, therefore health promotion programs should stimulate development of permanent social support to maintain change of these behaviors.

Since Covid 19 – social isolation process started, 131 femicides and 95 suspicious deaths occurred from April to August 2020 in Turkey. In this context, it can be construed that digital public relations and online event managements have become increasingly crucial in terms of improved social awareness. The research objective is to implement a scale, which has been developed for digital public relations, online events and awareness concepts, on persons who participated in 7 different Webinars themed on women and violence, hence to put forward the power of creating any behavioral change in awareness extent of online events in the scope of digital public relations. Methods: For this research study, a scale has been devised based on the online event model designed as per digital public relations. In this context, the survey method, one of the quantitative research method, has been used. The Cronbach Alpha measure and the Kaiser-Meyer-Olkin statistic have been used in order to test reliability and adequacy of the survey, respectively, thus the reliability-adequacy measures have been determined. Results: Research findings reveal that the higher online event driven awareness is the more behavioral changes are in digital public relations; that women, when compared to men, are more responsive to digital public relations applications, and have a better level of awareness by comparison; that 18-25 age group is more responsive with better level of awareness to digital public relations applications when compared to 26-33 age group; and that parents’ educational background makes no difference in this. Conclusions: In light of the research findings, it can be seen that digital public relations applications help to create awareness in terms of online events, and also pave the way for behavioral changes. In this context, it needs highlighting that digital public relations applications should be further improved in particular matters such as violence against women in which creating awareness is crucial.

The primary goal of adaptive physical and special education is to increase the participation of individuals with special motor needs in educational and social activities. Current behavior modification and motivation techniques are based on descriptive explanations of physical activity behaviors and exercises provided by special educators to patients and parents; however, the efficacy of such techniques is largely unknown. The main purpose of this review was to determine how effective different special education techniques and behavior change interventions are for different domains of physical activity for patients with cerebral palsy spastic motor type. A scoping review of scientific/medical databases was conducted to identify relevant studies matched the pre-defined inclusion and exclusion criteria. Four studies were selected (n=173) implementing behavioral interventions across internet-based and in-person settings. Three of these studies were randomized controlled trials with pre-post differences in physical activity observed in youth with cerebral palsy following behavioral intervention, although improvements were not statistically different from controls. This scoping review collectively demonstrates that design and implementation of behavioral change interventions lead to improvements of specific motor skills and highlights the need for ongoing research in children and adults with special motor needs.
Podstawowym celem adaptacyjnej edukacji fizycznej i specjalnej jest zwiększenie udziału osób ze specjalnymi potrzebami ruchowymi w działaniach edukacyjnych i społecznych. Obecnie techniki modyfikujące zachowania i motywację opierają się na wyjaśnieniach, jakich pedagodzy specjalni udzielają pacjentom i rodzicom, opisując ćwiczenia i zachowania związane z aktywnością fizyczną, niemniej jednak skuteczność tych technik jest nieznana. Podstawowym celem pracy jest określenie skuteczności różnych technik edukacji specjalnej i interwencji w zakresie zmiany zachowań w różnych domenach aktywności fizycznej pacjentów z mózgowym porażeniem dziecięcym typu spastycznego. Przeprowadzono przegląd naukowych/medycznych baz danych w poszukiwaniu stosownych badań, które odpowiadały wcześniej zdefiniowanym kryteriom włączenia i wyłączenia. W czterech wybranych badaniach (n=173) zastosowano interwencje behawioralne przeprowadzone za pomocą Internetu oraz osobiście. Trzy z badań były randomizowanymi kontrolowanymi badaniami, w których zaobserwowano różnice w aktywności fizycznej młodzieży z mózgowym porażeniem dziecięcym przed i po przeprowadzeniu interwencji behawioralnej, niemniej jednak stopień poprawy nie różnił się statystycznie od grupy kontrolnej. Przegląd wykazał, że projektowanie i wdrażanie zmian behawioralnych prowadzi do poprawy określonych umiejętności motorycznych, a także, że istnieje zapotrzebowanie na nowe badania dotyczące dzieci i dorosłych ze specjalnymi potrzebami ruchowymi.
