Objectives: To examine heart rate variability during alternate nostril breathing (ANB) in a group of individuals novice to yoga practice and to compare the effects of ANB to that of paced breathing (PB) at the same respiratory rate. Design: The study involved randomized repeated measures of two different breathing patterns. Participants: Twenty healthy individuals (22.3 ± 2.9 years) participated. Intervention: Subjects were asked to rest for 5 minutes (PRE-ANB and PRE-PB) before they participated in two different breathing exercises (ANB and PB) in a random order for 30 min. Outcome Measures: Heart rate (HR), mean arterial pressure (MAP) and heart rate variability (HRV) were measured during PRE-ANB, PRE-PB, ANB, and PB and compared using analysis of variance. HRV was reported as frequency and time domain parameters. Total power (TP; 0.0-0.4 Hz), low- (LF; 0.04-0.15 Hz), and high-frequency (HF; 0.15-0.40 Hz) ranges were selected and were natural log (ln) transformed. Time domain variables such as RMSSD, pNN50, and SDNN were also reported. Results: There was a main effect of breathing condition on HR and MAP (p < 0.05 for both) such that HR was higher during PRE-ANB and ANB compared to PRE-PB and PB and that MAP was greater during ANB compared to the other conditions. There were main effects of time on lnTP, lnLF, and lnHF (p < 0.001 for all) such that lnTP and lnLF were greater and lnHF was lower during ANB and PB compared to the PRE conditions. There was a breathing x time interaction on lnLF/lnHF (p <0.05) such that it was greater during PB compared to ANB and that it was greater during ANB and PB compared to PRE-ANB and PRE-PB. There was a main effect of breathing on pNN50 such as that it was lower during Pre ANB and ANB. There was also a main effect of time on SDNN such that it was higher during PB and ANB. No significant changes were noted in RMSSD measurements during any conditions. Conclusions: During both ANB and PB, there was a decrease in parasympathetic and/or increase sympathetic control of the heart. Additionally, ANB may result in less of a reduction in parasympathetic control than PB.

Introduction This study was undertaken to determine the mean peak filter resistance to airflow (R filter) encountered by subjects while wearing prototype filtering facepiece respirators (PRs) with low Rfilter during nasal and oral breathing at sedentary and low-moderate work rates. Material and methods In-line pressure transducer measurements of mean R filter across PRs with nominal R filter of 29.4 Pa, 58.8 Pa and 88.2 Pa (measured at 85 l/min constant airflow) were obtained during nasal and oral breathing at sedentary and low-moderate work rates for 10 subjects. Results The mean R filter for the 29.4 PR was significantly lower than the other 2 PRs (p < 0.000), but there were no significant differences in mean R filter between the PRs with 58.8 and 88.2 Pa filter resistance (p > 0.05). The mean R filter was greater for oral versus nasal breathing and for exercise compared to sedentary activity (p < 0.001). Conclusions Mean oral and nasal R filter for all 3 PRs was at, or below, the minimal threshold level for detection of inspiratory resistance (the 58.8–74.5 Pa/l×s⁻¹), which may account for the previously-reported lack of significant subjective or physiological differences when wearing PRs with these low R filter. Lowering filtering facepiece respirator R filter below 88.2 Pa (measured at 85 l/min constant airflow) may not result in additional subjective or physiological benefit to the wearer.

Rib fracture : different radiographic projections

Background: Rib fracture is the most common thoracic injury. It is thought to be present in 10% of all traumatic injuries and in almost 40% of patients who sustained severe non-penetrating trauma. There are 12 pairs of ribs. This study reviews various methods of acquisition and reconstruction of radiographic images of traumatic rib fractures in order to determine the optimal views and to simplify rib fracture diagnostics. Material/Methods: Eight different plain radiography pictures of ribs were performed with the patient in an erect position. The following projections were obtained in sequence: oblique at 45° or 30° angle on inspiration, oblique at 45° or 30° angle on expiration as well as 45° and 39° projections during slow and fast breathing. All radiographic examinations were performed using a Philips three-phase scanner installed at the Al- Razi Hospital in Jenin, Palestine. Results: The results demonstrate that the 45° antero-posterior oblique projection performed on expiration is recommended for diagnostics and interpretation of traumatic rib fractures. Conclusions: Conclusion emerging from this study are such that for a 45° oblique view on expiration is recommended for radiographic imaging of patients with clinical signs of fracture, e.g. evaluation of lower rib fractures, while 45° oblique view during fast breathing is recommended for suspected upper rib fractures.

Introduction: In the general population, nasal obstruction is a common complaint. However, an objective evaluation of nasal obstruction is difficult. Nose examination, computed tomography (CT), acoustic rhinometry, and anterior rhinomanometry do not accurately reflect the discomfort reported by patients with nasal obstruction. In patients with nasal obstruction, this study evaluated nasal breathing with a unique device for continuous nasal-oral spirometry – a nasal-oral flow analyzer (NOFA); moreover, quality of life was compared between patients with normal nasal breathing on NOFA and of those with impaired nasal breathing on NOFA.Methods: Of 181 adult patients admitted to an ENT department due to nasal obstruction that were enrolled in the study, 97 (53.6%) completed all per-protocol assessments, including the SF-36 questionnaire and 3-hour, continuous nasal-oral spirometry with NOFA. Based on the presence of normal nasal breathing defined as ≥95% of nasal flow, the 97 patients were divided into those with normal nasal breathing (n=31) and impaired nasal breathing (n=66).Results: Patients with normal nasal breathing differed from those with impaired nasal breathing with respect to all SF-36 subscales (physical functioning, p=0.004; role-physical, p=0.009; bodily pain, p<0.001; general health, p=0.007; vitality, p=0.002; social functioning, p=0.008; mental health, p=0.009; physical component summary, p<0.001; mental component summary, p=0.02), except for the role-emotional subscale (p=0.1).Conclusions: Among patients with symptoms of nasal obstruction, compared to patients with normal nasal breathing, those with impaired nasal breathing had significantly lower quality of life in the physical and mental domains. Further research needs to determine whether NOFA can be used to diagnose nasal obstruction.

This article is a continuation of the article published in issue No. 3 (64) 2018 of Polish Hyperbaric Research, which demonstrated the scope of changes which a typical car lift should be subjected to in order to extend its functionality to that of a hoist, that lifts the lid of the decompression chamber, in a breathing simulator used at the Department of Underwater Works Technology of the Naval Academy in Gdynia.

Purpose: The aim of this paper was to evaluate the effects of breathing retraining with chest wall mobilization on the onset of accessory breathing muscle recruitment and respiratory reserve in individuals with chronic neck pain. Methods: Thirty-two participants with non-specific chronic neck pain were randomly assigned into intervention and control groups. The intervention group received 30 minutes of breathing retraining with chest wall mobilization and the control group was assigned to rest for 30 minutes. Electromyography (EMG) of upper trapezius (UT), scalene (SC), and sternocleidomastoid (SCM) muscles were recorded during respiratory excursions by cycling for 12 minutes. Measurement of maximum voluntary ventilation (MVV), chest expansion, and pain intensity were taken during normal breathing. The immediate effects within each group and between two groups were analyzed. Results: Significant improvement in respiratory reserve was observed in the intervention group compared to control group through prolonged EMG onset of accessory breathing muscles. Moreover, increase of MVV, chest expansion and decrease in pain intensity were observed. Conclusions: This research suggests that breathing patterns and chest expansion should be considered within the physical assessment of breathing retraining, and that chest wall mobilization offers clinically important improvements in patients with chronic neck pain.

The aim of this study was to examine the effect of resonant breathing biofeedback training for reducing stress among manufacturing operators. Resonant breathing biofeedback works by teaching people to recognize their involuntary heart rate variability and to control patterns of this physiological response. Thirty-six female operators from an electronic manufacturing factory were randomly assigned as the experimental group (n = 19) and the control group (n = 17). The participants of the intervention received 5 weekly sessions of biofeedback training. Physiological stress profiles and self-perceived depression, anxiety, and stress scale (DASS) were assessed at pre- and post-intervention. Results indicated that depression, anxiety, and stress significantly decreased after the training in the experimental group; they were supported by a significant increase in physiological measures. Overall, these results support the potential application of resonant biofeedback training to reduce negative emotional symptoms among industrial workers.

The topic of this review is the clinical usefulness of techniques involving controlled breathing, based on Eastern traditions, aimed at restoring autonomic balance in psychiatry. Although these techniques have a long tradition as "relaxation methods", they gain additional meaning in the context of prolonged stress resulting from the ongoing COVID-19 pandemic. This review, however, is not limited to the pandemic; it also focuses on recent findings regarding clinical populations and provides basic information on the biological mechanisms of respiration and related markers (respiratory rate - RR and heart rate variability - HRV) and their use as effect indicators in research on the population of patients with mental disorders. On the basis of the available data from empirical studies and previous systematic and narrative reviews, it is possible to indicate the possible benefits of using techniques based on controlled breathing as a complementary method in the treatment of a number of mental health disorders. Possible side effects associated with the proposed techniques are also discussed.
