Opis:
A 43-year-old man was admitted to the hematology department due to second recurrence of anaplastic lymphoma T-cell ALK+. Lymphoma was diagnosed 28 years earlier. The patient received COP regimen (17 cycles) and CHOP (5 cycles), radiotherapy and underwent splenectomy. He achieved complete remission at that time. Relapse of the disease was diagnosed 8 years later, which was treated with 6 cycles of chemotherapy (cytarabine, mitoxantrone, vepeside and glucocorticoids) and high-dose chemotherapy followed by hematopoietic stem cell transplantation. Second disease recurrence was found in 2013, it was anaplastic lymphoma T-cell ALK+ stage IIA by Ann Arbor. Echocardiography and myocardial perfusion scintigraphy revealed chronic heart failure NYHA class I. Angiotensin receptor antagonist (ramipril) and β-blocker (carvedilol) were recommended. The patient underwent 6 cycles of ESHAP, complete remission was reported after the second cycle. High-dose therapy with autologous stem cell rescue was considered at that time. However, having T-cell ALK+ lymphoma with a relatively good prognosis, previous prolonged complete remissions (respectively – 12 and 8 years), an insufficient yield from the harvest (3,33 × 106/kg CD34+ cells), heart failure and chronic active viral hepatitis B, the high risk intensive chemotherapy followed by hematopoietic stem cells transplantation was discontinued. The chemotherapy was complicated by brachial vein and superficial vein thrombosis of left upper limb and hypogammaglobulinemia. Follow-up echocardiography performed after completion of chemotherapy showed improvement in EF (64%).

Introduction: Gastrointestinal bleeding is a common disease that surgeons encounter in everyday clinical practice. It is most often easy to diagnose and treat. However, rare causes of bleeding can lead to delayed diagnosis and ineffective treatment. Dysfibrinogenemia is a qualitative fibrinogen disorder in which functional fibrinogen level is reduced with normal antigenic level. Case report: Herein we present the case of a 59-year-old female with recurrent gastrointestinal bleeds, that turned out to be an unusual manifestation of congenital dysfibrinogenemia. Detailed imaging and endoscopic diagnostics revealed portal hypertension with a non-bleeding 1-cm gastrointestinal stromal tumor and multiple angiodysplastic lesions in close proximity.

Opis:
Typical radiological findings in patients with SARS-CoV-2 pneumonia consist of the presence of bilateral pulmonary opacities (both ground-glass attenuations and consolidations) with a peripheral or subpleural distribution, often involving the posterior regions of both lungs specifically. Although interstitial pneumonia represents the hallmark of COVID-19, the clinician should remember that sudden worsening gas exchange may have different aetiology. Pneumothorax and pneumomediastinum are well-known complications of invasive mechanical ventilation. However, since the beginning, the development of spontaneous pneumothorax and pneumomediastinum was reported in COVID-19 patients, without risk factors. The exact mechanism for the development of spontaneous pneumothorax and pneumomediastinum in COVID-19 patients is still unclear, and findings suggest that these clinical manifestations can be interpreted as an index of lung disease severity associated with complicated hospital courses, worst prognosis and increase in mortality rate. We report a case of a SARS-CoV-2 infected obese male patient, in quarantine and without risk factors, presenting to the emergency department for severe dyspnea due to COVID-19 pneumonia complicated by spontaneous pneumothorax and pneumomediastinum.

Opis:
While titles in scientific journals in general have attracted considerable interest, titles in medical research seem not to have been examined quite extensively. In these few studies, attention has been paid primarily to the form and length of titles as well as to their content. What is also emphasised is the need for discussion and further research on the topic in order to establish consistent guidelines regarding the formulation of clear and precise titles, which would aid journal editors, authors and readers. This paper constitutes an attempt to examine practices of formulating titles in medical case reports in Polish, following the study of F. Salager-Meyer et al. (2013a), in which title length, structure and content were considered as well.

Opis:
The present paper constitutes a qualitative discursive investigation of the nineteenth century case reports from ophthalmology derived from a specialist American medical journal, and focuses on citation as well as on authorial and patient’s presence. Regarding the first aspect, the present study generally confirms the results of the previous research, i.e. significant subjectivity, directness and informal character of the texts at hand. However, it also provides another insight into the scholarly communication in the nineteenth century, focusing on how patients are positioned therein, which has not received significant attention in the literature of the subject. Additionally, the paper offers an overview of the studies on specific aspects of scientific prose of the nineteenth century, including the medical context, as well as attempts to show the relation between the texts analysed and the context of their production and functioning, following the tradition of Genre Rhetorical Studies.

Opis:
This article aims to present and discuss new developments in the macro-genre of medical case reporting. These developments are novel forms of case reports published in international medical journals, going beyond the mere reporting of a case of a new disease or of its novel aspects. They may consist in the differences at the level of form, function and mode of content presentation. The data for the study constitute various publications derived from prestigious medical publishing outlets in English and will be analysed from the qualitative discourse perspective. The article demonstrates both generic creativity on the part of the authors as well as reflections on the changes taking place in medical practice.

Opis:
Though fibroadenoma is the most common benign tumor of the breast and is more common under the age of 30, giant fibroadenoma is rare representing less than 4% of all fibroadenomas. A 12 years old girl presented with rapidly enlarging well-circumscribed firm, non-tender mass in right breast for 2 months which was painless and with no history of trauma, nipple discharge, fever, anorexia, weight loss or axillary lymphadenopathy. There was no family history of neoplasms. Clinically, the lump was about 12 x 12 cm and not fixed to skin or underlying structures with the absence of nipple retraction or discharge. There was no axillary lymphadenopathy. Fine needle aspiration cytology showed a benign proliferative breast disease. Total excision of mass was done preserving nipple and areola having weight of 535 gm with histopathological features suggestive of giant fibroadenoma. Giant fibroadenoma is a benign breast disease that may mimic rare malignant lesion. So, breast and nipple conserving surgery should always be performed irrespective of size of tumor as in this case.

Simultaneous measurement of maximum respiratory pressures and respiratory muscle electromyography in a stroke patient changes after implemented physiotherapy preliminary case report

Introduction: In subject-based literature, there are no studies in which the results obtained simultaneously from surface electromyography of respiratory muscles and measurements of maximal respiratory pressures in stroke patients would be analysed. Research objective: The purpose of the study was to evaluate the effects of physiotherapy with breathing exercises in a subject with stroke-induced hemiplegia. Material and methods: In this pilot study, we presented a 36-year-old female with a medical history of haemorrhagic stroke in the left hemisphere, and with right-sided hemiparesis 76 months prior to research. The patient underwent 5, 60-minute individual physiotherapy sessions with general fitness training and breathing exercises for 1-week, with pre- and post-intervention outcome measures including functional assessment, respiratory pressure measurements using spirometer together with the electromyographic assessment of the respiratory muscles. Results: After implementing the physiotherapy with breathing exercises, equalisation of EMG activity and proper muscle recruitment during forced inhalation and exhalation have been demonstrated. The subject increased maximal expiratory and inspiratory pressures: (69.3% to 95.8% and 79.5% to 100.1% of predicted values, respectively). Conclusion: The results from this pilot study allow to indicate that the use of physiotherapy with breathing pattern correction may have positive impact on pulmonary function in a chronic stroke patient.

The Coronavirus disease 2019 (COVID-19), caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), has posed a public health system challenge across the globe. A retrospective analysis of COVID-19 globally surmises that the fungal co-infections associated with COVID-19 might be missed or misdiagnosed. However, data regarding all the signs and symptoms of COVID-19 are insufficient. The available few publications conclude that patients with COVID-19 have a higher susceptibility to fungal coinfections. Mucormycosis is a rare and often lifethreatening fungal disease characterized by vascular invasion by hyphae, resulting in thrombosis and necrosis. Based on the available data it seems COVID-19 patients, especially severely ill or immunocompromised, have a higher susceptibility to invasive mycoses. Therefore, it is important to assess the risk factors, the types of invasive mycosis, the strengths and limitations of diagnostic methods, clinical settings, and the need for standard or individualized treatment in COVID-19 patients. A 33 years old female operated case of laparoscopic ectopic removal with salpingectomy and tubectomy, at post-operative Day 5 had redness and pus discharge from the operative site and was diagnosed with abdominal wall cellulitis. She underwent local exploration and wound wash. At post operative day 5, the patient came to our emergency room with mild disorientation, cellulitis, and pain at the port insertion site. On examination, we highlight BP 90/50 mmg and blood test analysis with HB-8.3, leucocyte count 29.91×109/L, CRP 333mg/L. CT scan revealed necrotizing fasciitis. She then underwent wide local excision and debridement. Post debridement the next day during dressing, the wound showed a cotton fluffy appearance at the edges and part of the base with black necrotic areas. Wound swab was sent for fungal culture, KOH mount, blood culture, pus culture, and tissue for histopathology. In the meantime, she was started on empirical antifungal amphotericin B, meropenem and minocycline antibiotics. Covid antibodies test was done which were reactive: 1.96. Tissue histopathology revealed mucormycosis. A high degree of suspicion and promptness in starting antifungal prevented fatal outcome.
