Tytuł:: Relationship between oral hygiene and fungal growth in patients : users of an acrylic denture without signs of inflammatory process

Opis:: Objectives: To answer to the following questions: is there any relationship between oral hygiene and the growth of yeast in patients without mucosal inflammation; and is there a need for mycological examination patients without mucosal inflammation? Background: Patients with candidiasis may report varied symptoms, but such infections are most often asymptomatic. In addition to its high incidence in denture users (60%-100%), there is a concern that Candida species from the oral cavity may colonize the upper gastrointestinal tract and lead to septicemia, which has a 40%-79% mortality rate and can require a prolonged hospital stay. It is thus important for all physicians to be aware of the risk factors, diagnosis, and treatment of oral candidiasis in older patients. Methods: A retrospective study was carried out on a group of patients who had undergone mycological examination and assessment of the intensity of yeast growth, and oral hygiene. Results: Ninety-one denture wearers who lacked signs of clinical inflammation were included in the study. The growth of Candida albicans was as follows: 14 patients had up to 20 colonies; 19 patients had over 20 colonies. Ten percent of patients with good oral hygiene proved to have more than 20 yeast colonies. 5% of patients with bad oral hygiene had more than 20 colonies. Conclusions: There was no relationship found between hygiene and the growth rate of fungal microorganisms. In patients without clinical symptoms of stomatitis, mycological examination should be considered.

Opis:: Depending on the clinical context, objectives, and treatment requirements, a variety of prosthetic devices capable of artificially reconstructing the dental arches in a manner as customized as possible can be utilized in the treatment of complete edentation. The purpose of this study is to compare the efficacy of general vitamin B12 treatment vs the efficacy of local treatment accomplished through full dentures. The study was performed on a group of patients of Dental Faculty in Iasi. Because cyanocobalamin deficiency is typically treated parenterally, the current study proved the superiority of local treatment with complete dentures obtained through the crosslinked polymerization method, a much easier and less expensive way to perform the same treatment in the same time period.

Opis:: Formulation of the problem. One of the most common complications in the process of orthopedic treatment with non-remavable dentures is the injury of the gums of the supporting teeth, which, under unfavorable conditions of the body, can lead to inflammatory changes in the periodontal of the supporting teeth. Objectives of this study was the quantitative and qualitative changes in indicators of gum liquid: its amount and the presence in it of mediators of inflammation – histamine and serotonin. Materials and methods. The study included 68 people aged 20 to 50 years with healthy periodont and generalized periodontitis volume of the initial-first degree, who first made non-removable dentures. The first group included 32 individuals without pathological changes in periodontal tissues. The second group consisted of 36 people with generalized periodontitis of initial – first degree. Results. Before orthopedic treatment, the level of gum liquid in group II was significantly higher than in the group I (0.41 ± 0,02 mm² according 1,5 ± 0,07 mm²). After tooth preparation, the number of gum liquid increased sharply in both observation groups compared to baseline. Patients in both groups using the proposed treatment-and-prophylactic regimen had a significantly lower level of gum liquid compared to subgroups, where no treatment-and-prophylactic procedures were applied. Conclusions. In all patients, who made non-removable dentures, the amount of gum fluid and histamine and serotonin content increased. The proposed therapeutic and prophylactic complex showed pronounced anti-inflammatory effect during orthopedic treatment.

Tytuł:: Study of the Mineral Structure of the Oral Fluid in Patients With Chronic Diseases of the Salivary Glands Using Different Fixed Dentures

The paper is devoted to a question of improvement of the prosthetics quality with fixed dentures in patients with inflammatory affections of the salivary glands. The comparative analysis of prosthetics results with metal and ceramic bridge dentures has shown that the level of the saliva osmolality in patients with CDSG was by 14.5 % higher. Their indices remain by 34.9 % decreased in application of the metal constructions, by 26.0 % - in metal - ceramic prosthetics and by 10.4 % - in prosthetics with whole cast ceramic constructions.

Opis:: The functionality of removable dentures is ensured by proper diagnostics and clinical modeling. No less important are the structural conditions and the biotribological and biomechanical context, which determine the wear resistance in the contact of opposing teeth and the contact of the denture plate with the stomatognathic system. The aim of the study is to evaluate acrylic teeth used in prosthetic reconstructions based on microstructural, micromechanical and tribological tests. Samples for testing were taken from teeth for removable dentures made by various manufacturers. Microstructural analyses were performed using an optical microscope and a scanning microscope. Microhardness and elasticity coefficient measurements were performed on the NHT device. Tribological tests were performed on a Roxana Machine Works tester using a friction node: ball – 3 discs made of the tested material. The conducted research allowed for the evaluation of the structural quality of acrylic teeth and the determination of the tribological interaction resulting from the contact of synthetic and natural teeth in the presence of artificial saliva.
Funkcjonalność protez ruchomych zapewnia prawidłowa diagnostyka i modelowanie kliniczne. Nie mniej ważne są uwarunkowania strukturalne oraz kontekst biotribologiczny i biomechaniczny, które decydują o odporności na zużycie w kontakcie zębów przeciwstawnych oraz o kontakcie płyty protezy z układem stomatognatycznym. Celem opracowania jest ocena zębów akrylowych stosowanych w rekonstrukcjach protetycznych na podstawie badań mikrostrukturalnych, mikromechanicznych i tribologicznych. Próbki do badań pobierano z zębów do protez ruchomych wykonanych przez różnych producentów. Analizy mikrostrukturalne wykonano na mikroskopie optycznym i mikroskopie skaningowym. Pomiary mikrotwardości i współczynnika elastyczności przeprowadzono na urządzeniu NHT. Badania tribologiczne wykonano na maszynie Roksana Machine Works z zastosowaniem węzła tarcia: kula – 3 krążki z materiału badanego. Przeprowadzone badania pozwoliły na ocenę jakości strukturalnej zębów akrylowych oraz na wyznaczenie interakcji tribologicznej wynikającej z kontaktu zębów syntetycznych i naturalnych w obecności sztucznej śliny.

Opis:: W pracy przedstawiono analizę porównawczą zachowań mechanicznych protezy szkieletowej w oparciu o badania symulacyjne modelu matematycznego i pomiary laboratoryjne wykonane na rzeczywistym modelu protezy. Analizę doświadczalną protezy oparto głównie na ocenie wartości przesunięć liniowych siodła protezy pod wpływem symulowanych sił zgryzu przykładanych dynamicznie do wybranych węzłów modelu protezy. Weryfikację numeryczną przeprowadzono, posiłkując się metodą elementów skończonych MES.
In the research, a comparative analysis was presented. It concerned behaviour of mechanical skeleton's prosthesis based on simulating mathematical model and laboratory measurements carried out on a realistic prosthesis' model. Experimental analysis of the prosthesis is based mainly on the evaluation of values of linear movements of prosthesis' saddle triggered by simulated forces that were put dynamically to selected prosthesis' model's nods. Numerical verification was carried on the basis of finite elements method (MES).

Opis:: Purpose This study aimed to determine the effect of metal base removable partial dentures (MRPDs) and acrylic base removable partial dentures (ARPDs) on oral health-related quality of life. Design/methodology/approach The study was conducted on 40 patients. Twenty received ARPDs (nine in the maxilla and eleven in the mandible), and twenty received MRPDs (nine in the maxilla and eleven in the mandible). The patients were 45–65 years old. The impact of RPDs on the patient's quality of life was analysed with the Oral Health Impact Profile (OHIP-ALB14) questionnaire. The differences in OHIP-14 between the two dentures at three different time points were estimated. All statistical analyses were performed using SPSS software, version 22 (IBM), and a p-value<0.05 was considered statistically significant. Findings The OHIP-ALB14 score for both denture types was high at T1 and then remarkably decreased at T2 (p<0.05) and T3 (p<0.05), with no significant differences between MRPDs and ARPDs users. For ARPD users, the satisfaction level significantly increased (<0.001) after one year for the dimensions of Physical Pain, Functional limitation, and Psychological Discomfort. Whereas, for MRPD users, satisfaction level significantly increased (<0.001) in the dimensions of Functional limitation, Psychological Discomfort, and Psychological Disability after a year of denture use. Research limitations/implications The findings from this study can be taken as the first step towards future research with a longer post-operative follow-up that can analyse the differences between dentures if any. Practical implications The comparative analysis between ARPD and MRPD and their impact on the oral soft tissues will help prosthodontists reach accurate treatment decisions. Originality/value This study has not been conducted in the region of Kosovo before. Moreover, the engineers manufacturing dental prostheses will know about the effect of their product on oral health.

Częste uszkodzenia płyt protez mogą jednak świadczyć o zbyt małej wytrzymałości mechanicznej tworzyw akrylowych, z których są wykonywane. W pracy omówiono próbę modyfikacji materiału protetycznego Vertex R.S hydrofobizowanym bentonitem/montmorylonitem, w celu poprawy wytrzymałości protezy. Hydrofobizowany bentonit rozpraszano w metakrylanie metylu (MMA) przez sonifikację, a następnie przeprowadzano polimeryzację metodą roztworowo-strąceniową. Przebadano właściwości mechaniczne i optyczne polimerowych kompozytów stomatologicznych zawierających zmodyfikowany montmorylonit.
A wide range of polymer materials may be used in the rehabilitation of the stomathognatic system. However, frequent damage of denture plates indicates rather low mechanical strength of acrylic materials. In this article we describe an attempt to modify a prosthetic material Vertex R.S. with hydrophobized bentonite/montmorilonite in order to improve the mechanical strength of the denture. Hydrophobized bentonite was dispersed in methyl methacrylate (MMA) by sonification, and then polymerized by solution - precipitation method. Mechanical and optical properties of the polymer composites containing the modified montmorillonite were evaluated.
