Usefulness of ultrasound examinations in the diagnostics of necrotizing enterocolitis

Background: Necrotizing enterocolitis (NEC) is one of the most serious disorders of gastrointestinal tract during neonatal period. Early diagnosis and adequate treatment are essential in the presence of clinical suspicion of NEC. Plain abdominal radiography is currently the modality of choice for initial evaluation of gastrointestinal tract in neonates. However, when the diagnosis is uncertain, abdominal ultrasound with bowel assessment might be an important complementary examination. The aim of the study was to evaluate usefulness of ultrasound in the diagnosis of NEC and its value for implementation of proper treatment. Material and Methods: The data of nine neonates diagnosed with NEC, hospitalized at the Provincial Hospital No. 2 in Rzeszow in the period from September 2009 to April 2013 was retrospectively analyzed. Apart from abdominal radiography, abdominal ultrasound with bowel assessment was performed in all nine cases. Imaging findings, epidemiological data, coexisting risk factors and disease course were assessed. Results: Most children in the group were preterm neonates. Findings in plain abdominal radiography were normal or nonspecific. A wider spectrum of findings was demonstrated in all ultrasound examinations and intestinal pneumatosis, a pathognomonic sign for NEC, was more frequently noted than in plain abdominal x-ray. Most children were treated by surgical intervention with resection of necrotic bowel loops and in more than half of the cases location of changes identified during surgery was concordant with ultrasonographic findings. Conclusions: Abdominal ultrasound examination might be helpful in the diagnosis of NEC, especially when plain abdominal radiography findings do not correlate with clinical symptoms. However, abdominal radiography is still considered the modality of choice. The range of morphological changes detectable on ultrasound examination is much wider than in plain abdominal radiography. Ultrasound examination allows for more accurate assessment of changes within intestines and adjacent tissues, which aids clinicians in making more accurate therapeutic decisions and implementing proper treatment.

Perianal fistulas : diagnostic imaging techniques and their applications

Opis:
The application of imaging techniques in the diagnostic work-up of perianal fistulas is fundamental for an adequate description of the pathological lesion with an exact course of the fistular canal, together with its external and internal orifice which, that together with determined concomitant pathologies enables a right choice of a proper therapeutical method that most often involves a surgical intervention. The effectiveness of the treatment is in each case connected mainly with a minimised risk of recurrences and faecal incontinence in postoperative patients. The rate of these complications correlates primarily with unavailable comprehensive and precise imaging diagnostic tools and with different rates of precision of every radiological method. Our work summarises the currently available data on that subject

Coexistence of Ollier's Disease and non-Hodgkin's lymphoma in a nine-year-old boy

Ollier's disease (enchondromatosis, chondromatosis multiplex) is a rarely occurring affection classified as osteochondrodysplasia and characterized by ataxic, multifocal hypertrophy of the chondrus. Sarcomatous transformation appears in 25% o f those affected. There are no cases of a coexistence of Ollier's disease and non-Hodgkin's lymphoma reported in accessible articles. A 9-year-old boy with Ollier's disease diagnosed in the second year o f life was admitted to the Clinic of Pediatrics, Hematology, and Pediatric Oncology because of cervical, submandibular, inguinal, and left axillary lymph node enlargement. Status at admission was estimated as medium-severe. Laboratory findings were a mediocre state o f anemia, tricipher OB, and increased LDH activity. Chest radiogram showed upper mediastinum and right hilus extension. Diagnosis of non-Hodgkin's B-cell lymphoma was made from a specimen taken during mediastinoscopy. Chemotherapy according to the LMB-89 program was applied. The presence o f multiple chondromas was confirmed in the scapulas, left humerus, ribs, right ilium, and left femur. Imaging diagnostics revealed the malignant transformation of a chondroma in the left humerus. Histological examination of a specimen of the tumor confirmed non-granulomatous, lymphomatous proliferation in the primary pathological lesion. Despite intensive chemo- and radiotherapy, the child died in the 5th month of treatment.

Bilateral multifocal hamartoma of the chest wall in an infant

BACKGROUND: Hamartoma of the thoracic wall is a rare benign tumor that occurs in infancy and can be mistaken for a malignancy due to its clinical and imaging features. Hamartomas are extrapleural soft tissue lesions that cause rib expansion and destruction and appear on imaging as cystic areas with fluid levels and calcification. They can cause scoliosis, pressure on the neighboring lung parenchyma and mediastinal displacement. While conservative treatment is recommended in asymptomatic cases, growing lesions require surgical excision. CASE REPORT: In this report, we present the imaging findings in a 3-month-old infant that presented with a firm swelling in the chest wall and was histopathologically confirmed to have a bilateral multifocal hamartoma. CONCLUSIONS: Radiological imaging methods are important for accurate diagnosis of this very rare condition that can be confused with a malignancy.

Magnetic resonance imaging in oral and maxillofacial surgery : a structured guide for trainees

Magnetic resonance imaging (MRI) is an important diagnostic tool in oral and maxillofacial surgery due to its superior ability to visualise soft tissue and complex anatomical structures without ionising radiation. This narrative review, conducted using a structured literature search following PRISMA guidelines, aims to provide oral and maxillofacial surgery trainees with a structured guide for understanding and interpreting MRI of the head and neck region. Emphasis is placed on enhancing the ability to read common MRI sequences, particularly T1- and T2-weighted images, and on appreciating the value of MRI in diagnosing a range of conditions, including soft tissue lesions, temporomandibular joint disorders, vascular abnormalities, and tumours. To support learning, a selection of case examples is included to illustrate both normal structures and pathological changes. The review also draws attention to the current lack of formal MRI education in oral and maxillofacial surgery training and stresses the need for more organised, interdisciplinary teaching approaches.

A rare case of aggressive fibromatosis infiltrating dorsal muscles in a 6-year-old patient : CT, MRI and elastography evaluation

Background: The term fibromatosis or desmoid tumor refers to a group of benign fibrous growths without metastatic potential but with a significant risk of local recurrence. These lesions typically present infiltrative growth pattern with local invasion of adjacent tissues. This tendency is the reason for a relatively high rate of local recurrence, even after surgical removal. Fibromatosis is a very rare condition in general population but occurs more frequently in one of the familial cancer predispositions known as familial adenomatous polyposis (FAP) or Gardner syndrome. There are two main groups of fibromatosis: superficial (small, slow-growing lesions) and deep, also known as aggressive fibromatosis (large, rapid-growing lesions). Case Report: We report a case of a 6-year-old boy suffering from an aggressive form of fibromatosis. The patient developed a large pathological mass extending from the neck to the loins. After incisional biopsy and histpoathological examination of the sample, a diagnosis of aggressive fibromatosis was established. During the whole diagnostic process, different imaging techniques including CT, MRI and sonoelastography were used. As the surgical treatment was not possible, the patient was finally qualified for chemotherapy. Conclusions: Eventual diagnosis of aggressive fibromatosis is based on histopathological examination. However, it is an important condition that should be included in differential diagnosis of soft-tissue masses found in diagnostic imaging. Radiologists should be careful especially in defining the margins of infiltration in case of potential surgical treatment.

Diagnostic imaging of pregnant women : the role of magnetic resonance imaging

BACKGROUND: Presentation of magnetic resonance imaging (MRI) findings in pregnant women in the Department of Diagnostic Imaging, Institute of Mother and Child, Warsaw, Poland. MATERIAL AND METHODS: Forty-three symptomatic pregnant women underwent MRI between 9 and 33 weeks of gestation (mean of 23 weeks). Moreover, we included 2 pregnant women who underwent fetal MRI and had incidental abnormalities. RESULTS: In 9 cases, we excluded the suspected brain abnormalities. In 4 cases, we found unremarkable changes in the brain without clinical significance. One patient was diagnosed with multiple sclerosis, one with cortical dysplasia, one with pineal hemorrhage and one with a brain tumor. On abdominal MRI, 2 patients had normal findings, one patient had colon cancer with a hepatic metastasis, one patient had a hepatic angioma, one patient had an extraadrenal pheochromocytoma, one patient had an abscess in the iliopsoas muscle, 9 patients had myomas, two patients had ovarian simple cysts, two endometrial cysts, three dermoid cysts, one patient had sacrococcygeal teratoma, one patient had a cystadenofibroma (partial borderline tumor), one patient had an androgenic ovarian tumor and two patients had hyperreactio luteinalis. One patient was diagnosed with transient osteoporosis of the hip and one with a stress fracture of the sacral bone. CONCLUSIONS: Magnetic resonance imaging is the best imaging modality for pregnant women. Although ultrasonography is the method of choice, doubtful cases as well as structures that cannot be examined with ultrasonography can be non-invasively evaluated with MRI.

Ultrasound elastography is a revolutionary medical imaging technique, enabling a quantitative and qualitative evaluation of tissue stiffness. This paper presents, based on published evidence, a wide range of possibilities for this method in clinical trials and scientific research. The use of dynamic elastography avoids the undesired influence of force applied to the tissue by the elastograph probe on the information content of the obtained image. In clinical practice, elastography is used to identify and examine the pathological condition of soft tissues (including cancer lesions and tendonitis) and to diagnose neuromuscular diseases. It is also used in scientific investigations as a non-invasive method to study the structure of skeletal muscle, including muscle thickness, fiber length and pennation angle using standard ultrasonography mode; it is also possible to obtain information about physical properties such as stiffness. Ultrasound elastography could also be a useful tool for physiotherapists monitoring the rehabilitation process. Based on the results of these studies, advances in elastographic imaging technology, and progress in biomedical diagnostic methods, elastography is expected to become a common method used in clinical diagnostics and scientific research.

Clear-cell sarcoma of the kidney in a child : case report

Background: Kidney tumours other than nephroblastoma (Wilms' tumours) occur relatively rarely in the pediatric population. They include but are not limited to: nephroma mesoblasticum, rhabdoid tumour of the kidney and clear-cell sarcoma of the kidney. Case Report: In this article we presented a case of a 20-month-old boy with clear-cell sarcoma of the kidney. Concluisons: Imaging studies - including abdominal and chest CT examinations - allow to determine the extent of the disease and help in the initial assessment of the tumour character, as well as in planning the treatment, thus having a positive prognostic impact on the pediatric patients.

The effectiveness of contrast-enhanced spectral mammography and magnetic resonance imaging in dense breasts

Purpose: Breast cancer is the most common cause of death from neoplastic disease in women. Among all breast anatomy types, glandular type is the most problematic concerning evaluation. While digital mammography still remains the basic diagnostic tool, one must be aware of its limitations in dense breasts. Although magnetic resonance imaging (MRI) has greatly improved sensitivity, its specificity is low. Moreover, there are contraindications for MRI for some patients, so a substitute has been searched for. This study was performed to check if contrast-enhanced spectral mammography (CESM) can be a viable option for patients with dense breasts. Material and methods: The study involved 121 patients with abnormalities detected on base-line diagnostic imaging (ultrasound or mammography). The patients had subsequent examinations, both CESM and MRI performed within a maximum 2-month time interval. The sensitivity and specificity of both methods in the whole group as well as in specific breast structure types were measured and compared. Results: Contrast enhancement was visible in all 121 cases on MRI, while on CESM lack of enhancement was noted in 13 cases. All of those 13 lesions turned out to be benign. There were 40 (33%) benign and 81 (69%) malignant tumours. The analysed group included 53 (44%) glandular type breast patients, 39 (32%) mixed type, and 29 (23%) fatty type. Although MRI proved to be slightly more effective in dense breasts, both methods showed similar results in the whole study group. Conclusion: CESM can be used with confidence in patients with glandular breast type when MRI is not available or there are reported contraindications to MRI.
