Opis:
One of the key parameters in the evaluation of disorders of consciousness (DOC) is visual behavior. In the past, visual potential testing or PET scanning was mainly used to assess these parameters. Recently, Eye Tracker (ET) technology for assessing visual functions has emerged; however, there are only a few publications devoted to the use of this technology in assessing people with disorders of consciousness (DOC). The purpose of this study was to evaluate the feasibility of using ET in the diagnosis of visual functioning in DOC patients. The study group consisted 25 patients (8 women and 17 men) awakened from prolonged post-traumatic coma in the Care and Treatment Facility of the "Light" Foundation in Toruń. The coma occurred as a result of severe brain damage: brain injury, stroke or sudden cardiac arrest. The mean age was 39.83 (SD 11.88) for the entire group, 38.85 (SD 9.99) for women, and 40.23 (SD 12.84) for men. All of these patients were in various states of disorders of consciousness (DOC), which was examined using the Coma Recovery Scale-Revised (CRS-R) (Giaciano et al., 2004). Appropriate inclusion and exclusion criteria were applied based on examinations by an internal medical physician and an ophthalmologist. Important exclusion criteria from the study were infection, elevated temperature, visual impairment and patient agitation. The visual functioning of the patients was measured with the use of the Eye Tracker Tobii X-120 device. A significant difference in visual functioning was demonstrated primarily in the areas of (1) reaction time to first fixation, where the Minimal Consciousness State minus (MCS-) group showed a significantly longer reaction time compared to the Minimal Consciousness State plus (MCS+) and the Emergency of Minimal Consciousness State (EMCS) groups; (2) fixation duration, where the MCS- group showed a longer time compared to patients in the MCS+ and EMCS groups; (3) the number of fixation points on the screen, where the MCS- group showed a significantly lower number of fixation points compared to the MCS+ and EMCS groups. Eye Tracker Tobii X-120 can serve as a valuable tool to aid in the neuropsychological diagnosis of individuals experiencing states of reduced consciousness. Indicators that most differentiate between the different levels of impaired consciousness include the time to the first fixation, the number of fixation points on the screen and the total number of fixations.

Pilomatrixoma (PMT), or calcifying epithelioma Mahlerbego, is relatively rare ectodermal origin cancer derived from cells of the hair matrix. Calcifying epithelioma usually presents itself as a single, hard, movable relative to the skin and substrate lump about the size of 1–5 cm. The overlying skin is often bluish tinted. Usually asymptomatic, but can be painful in case of ulcers or inflammation. Most pilomatrixoma does not cause clinical complications. The clinical course is usually mild, but are noted malignant transformation, especially in old age. In this case the cancer metastases are in the lungs, bone, brain, skin. There is also a malignant form called – pilomatix carcinoma. The differential diagnosis should include tumors of the parotid gland, cancer of the skin, foreign body reactions, hematoma, osteochondroma. Challenges in the diagnosis and treatment of peritoneal mesothelioma : a case study and review of the literature

Peritoneal mesothelioma is a rare neoplasm that is associated with multiple diagnostic and therapeutic challenges. Therapeutic guidelines are scarce and based on extrapolative data. Histopathological diagnosis is difficult as neither the morphological finding nor the immunohistochemical stains are specific. The mainstay treatment for resectable disease is cytoreductive surgery with intraperitoneal chemotherapy being a valuable addition. Treatment of non-resectable cases includes platinum-based chemotherapy, immune checkpoint inhibitors, and bevacizumab. We present a case of a 49-year-old woman suffering from inoperable peritoneal mesothelioma, which was initially diagnosed as ovarian cancer and treated accordingly.

Assessment of diagnostic methods in adrenal gland hemorrhage in neonates on the basis of own material from the years 2007-2011
Opis:
We analyzed a group of nine neonates diagnosed with adrenal gland hemorrhage in the years 2007-2011, to evaluate diagnostic methods. We assessed risk factors and factors predisposing to hemorrhage. Severe and moderate perinatal hypoxia was found in 5 cases, while sepsis in 4 cases. Three patients had bilateral adrenal hemorrhage. All patients underwent ultrasound examination and color Doppler US. Their levels of vanillyl-mandelic acid in 24-h urine collection were normal. A complete regression of changes without evidence of adrenal hemorrhage or vascular flow on color Doppler US was found in the period from the 20th to the 165th day of life. Conclusions: 1. Color Doppler ultrasound findings, observation of the evolution of changes and a lack of vascular flow in adrenal glands, are suggestive of bleeding. Color Doppler ultrasound seems to be the most important diagnostic method. 2. The level of urinary catecholamine metabolites (vanillyl-mandelic acid) does not fulfill the role of screening test for neuroblastoma. However, increased levels of these metabolites may suggest a diagnosis other than the adrenal hemorrhage.

Glomus tumor - a rare, benign tumor of the nail apparatus

Introduction: Glomus tumor is a painful, subcutaneous tumor. The lesion may occur in various locations. In the case of the nail apparatus, it affects both the matrix area and the nail bed. Case report: We present a 71-year-old patient who presented to our clinic with a painful nodule lasting for about 15 years, located on the fifth finger of his left hand. After surgical removal, a histological examination was performed, which confirmed the clinical suspicion of glomus tumor. Intraoperative contact onychoscopy revealed a gray-blue nodule with linear vessels at the periphery. Conclusions: In the differential diagnosis of glomeroma, the following should be considered: pyogenic granuloma, subugual viral wart, melanoma, hemangioma, leiomyoma, sweat gland tumor, gout, neuroblastoma and hemangioma. Dermoscopy can be useful in the differential diagnosis, but the final diagnosis is based on a histopathological examination. Surgical removal of the lesion is considered the treatment of choice.

Hobnail hemangioma

Opis:
We present a case of a 73-year-old woman who developed a small lesion on her tongue. The nodule was resected and hobnail hemangioma was diagnosed. Hobnail hemangioma is a rare vascular lesion with unusual morphology, including bland cells with hobnail appearance, biphasic grow pattern with superficial dilated vessels and slit-like vessels in the deeper portion of the lesion. The infiltrative pattern of grow may cause misdiagnosis. The differential diagnosis with hemangioendothelioma variants, low grade angiosarcomas and Kaposi sarcoma is of particular concern. The lack of recognition of this uncommon entity may result in excessive and unnecessary treatment.

Differential diagnosis of eating disorders with the use of classification trees (decision algorithm)

Background: The aim of the study was to establish whether it is possible to make a correct diagnosis of various types of eating disorders on the basis of several variables. Method: A group of 213 females, 20 to 26 years of age, took part in the study. 92 women (control group) and 121 women (research group). We used the Eating Disorder Inventory (EDI), the Socio-cultural Attitudes Towards Appearance Questionnaire-3 (SATAQ-3) and a questionnaire of behavior towards the body. A selection of dependent variable predictors of disorder was made and a classification tree was developed. Results: People suffering from eating disorders differ from healthy individuals in the intensity of the variable “pattern for constructing emotional relationships on the basis of lack of trust and certainty in interpersonal relations”. People suffering from various types of eating disorders differ – besides the BMI – on such psychological traits as fear of gaining weight and a restrictive strive to be slim, as well as dissatisfaction with the body. A differential diagnosis in line with a decision algorithm correctly identified healthy individuals in 100% of cases, bulimia in 78.72% of cases, binge eating disorder in 93.33% of cases, and anorexia in 86.36% of cases. Conclusions: Psychological predictors of eating disorders may be arranged in the form of a classification tree. It is possible to make an accurate differential diagnosis of eating disorders on the basis of results of measurements of six variables.
