Background The Dizziness Handicap Inventory (DHI) was established to assess the impact of dizziness and balance problems on the quality of life. The aim of the study was to validate the Polish version of DHI for patients with vestibular disorders. Material and Methods Two hundred and thirty patients diagnosed with vestibular impairment and/or positional vertigo were included in the study. The mean age of the study group was 56.2 years (SD = 13.6). The factor structure (the principal component analysis − PCA), internal consistency (Cronbach’s α), and discrimination ability (the receiver operating characteristic [ROC] curve) were examined. Results A satisfactory internal consistency was found (Cronbach’s α coefficient = 0.92), while no floor or ceiling effect was revealed. The Dizziness Handicap Inventory demonstrated a good ability to discriminate between patients with and without the handicap (sensitivity and specificity about 80%, the cutoff point = 56). In PCA a 3-factor solution was obtained, with the factors related to restrictions in daily life, positional symptoms and visual-vestibular symptoms, which was not in agreement with the subscales provided in the original version. Conclusions The Polish version of DHI demonstrates satisfactory measurement properties and can be used to assess the impact of dizziness on handicap and the quality of life. The functional, emotional, and physical subscales were not confirmed. In particular, the functional subscale revealed no satisfactory internal consistency which provides an indication for further studies. Med Pr. 2019;70(5):529–34

Introduction: In elderly patients the dizziness and balance disorders may be elicited by the central nervous system dysfunction on various levels, caused be aging process and the coexisting diseases. The aim of the study was to assess the efficiency of rehabilitation in elderly patients with dizziness and balance unsteadiness.Material and methods: Out of the 84 patients over 65 years, with central vestibular impairment diagnosed in videonystagmography VNG, 31 with dizziness and balance unsteadiness, were enrolled to the study. Otolaryngological examination, Tinetti test, Time and Go test (TUG) and Functional Reach (FR) tests were conducted in all patients. They were assessed twice before and after 2 weeks of vestibular rehabilitation (VR). Training sessions took place five times a week.Results: Statistically significant improvement in total DHI and 3 subscale were observed after therapy. As many as 70% of patients presented better outcome in the gait and stability tests - in Time and Go test (TUG) an average score of 15.3 seconds, in Tinetti test an average of 22 points (low chance to fall) and in the FR test 27 cm were observed.Conclusion: In elderly patients, vestibular rehabilitation is a method that significantly improves posture and gait stability. In ageing patients with dizziness and unsteadiness clinical, functional and objective tests could confirm diagnosis and monitor VR therapy.

Dizziness is a symptom of many diseases. Patients very frequently come with such ailment to the hospital emergency department. It is a state which may last a few seconds or minutes and increase or recede with time. Therefore, the admitted patients frequently cannot assess their own illness in a precise and objective way. The dizziness definition is also quite ambiguous. Dizziness is defined as the sensation of one own’s body movement or spinning and movement of the surrounding. It is very important to record the patient’s medical history since the diagnostic procedure may depend on the symptoms’ character. Dizziness may be a symptom of a serious disease, although it is not easy to find its cause. It appears not only in case of the labyrinth and nervous system disorders, but also in the systemic and functional diseases. Dizziness and balance disorder are the direct cause of admitting one in every thirty patients. The symptom indicated the directly life-threatening disease only within the 3-8.5% of patients (cerebral circulation insufficiency - 6%, cardiac dysrhythmia – 1.5%, brain tumour < 1%). Analyzing the data concerning the problem of dizziness occurrence within the general population, it has been noticed that this symptom has been reported two or three times more frequent by women than by men. Dizzinesses are classified pathogenetically and clinically into labyrinthine and non-labyrinthine, paroxysmal and permanent, acute and chronic. Dizziness is hard to diagnose because the symptoms reported by patients are only their own subjective sensations. The data presented in the article implicate the increasing number of patients with such disorders. Apart from dizziness, the patients complain also about the hearing disorders and nausea, which make their proper functioning impossible. It is inappropriate to start the treatment without knowing the cause. Establishing, on the basis of patient’s medical history and physical examination, whether the dizziness is of peripheral or central origin, is essential for the further diagnosis. After establishing the main diagnosis, the casual and symptomatic treatment is implemented, in some cases - there is a vestibular rehabilitation or even a surgical treatment.

Introduction. Dizziness is a common complaint among patients seen by primary care physicians, neurologist and otolaryngologist. It presents difficult clinical and diagnostic problems. Objectives. The purpose of this study was to determine the prevalence and incidence of dizziness in the general population of the town and district Bytów, northern Poland, and to describe its clinical characteristics. Materials and method. Epidemiological questionnaires were completed by 4,799 inhabitants of the town and district of Bytów. All participants were randomly selected from the patients’ list of the Family Health Centre ‘Salus’ in Bytów. The main variables studied were gender, age, school education, work situation, clinical characteristics of dizziness symptoms and circumstances provoking dizziness. Results. Prevalence of dizziness was diagnosed among 16.4% of the inhabitants of the town and district of Bytów. Symptoms of dizziness appeared more often among the elderly and among women. Among young people, the condition shows as vertigo or presyncope, among people aged 50 or older, the condition showed as disequilibrium. The most common precipitant of dizziness was postural change. Dizziness was more likely to occur among those suffering from high blood pressure, diabetes, increased blood lipids, hypothyroidism, cardiac arrhythmia and depression. Dizziness was more likely to occur among women using OCS or HRT, and individuals taking anti-epileptic drugs. Conclusion. The authors present the first Polish neurootologic survey of epidemiology of dizziness in the general population. This condition is common in the general population, and this study indicates its prevalence in 16.4% of the inhabitants of the town and district of Bytów. It is recommendable that epidemiological studies should be carried out.

Background: Dizziness, numbness, and paresthesia of upper limbs are common symptoms in patients who undergo physiotherapy. Most of the symptoms are caused by neurological and skeletomuscular diseases. Subclavian steal syndrome is a rare case of such symptoms. Aim of the study: This study aimed to analyze how to proceed with symptomatic patients suspected of subclavian steal syndrome. Material and methods: Medical documentation was used. Case report: A 69-year-old patient, long term cigarette smoker, with the anamnesis of spine surgery due to discopathy, atherosclerosis of the lower extremities, and hypertension was referred to our hospital due to exacerbation of coronary artery disease. During his stay in the cardiac department, after smoking a cigarette, he felt pain and numbness in his left arm. He began intense movement of this hand, and then lost consciousness. A difference in pulse filling and blood pressure between the upper extremities was noted. In a duplex Doppler study, reversal flow in the left vertebral artery due to stenosis of the left subclavian artery was found. Angio-CT of the head vessels confirmed a significant stenosis of the proximal left subclavian artery. The patient was referred for further treatment to a Vascular Surgery Clinic. Conclusions: The subclavian steal syndrome is a rare cause of dizziness and paresthesia of the upper extremities. Physiotherapy procedures on the affected limb can exacerbate neurological symptoms. It is easy to identify the disease based on differences in pulse amplitude and blood pressure between upper limbs. Diagnosis should be established before proceeding with physiotherapy, due to the fact that some procedures can worsen the patient’s condition.

Introduction: Dizziness and imbalance occur in 20–30% of adults, which is a significant health problem. Aim: The aim of the study was to assess the value of the VHIT test against the background of other diagnostic tests, including videonystagmographic (VNG) examination, Dix-Hallpike maneuver, VHIT test, posturographic examination, which are possible to perform in a patient with dizziness. Material and methods: The study was performed in 200 patients aged 18–78 years, who were divided into two groups: I study – 100 patients with mixed dizziness, aged 18–78 years, II (reference group) – 100 healthy patients (students of the Faculty of Mili-tary and Medical University of Lodz and patients of the Clinic, without anamnesis of body imbalances (preventive examination), aged 20–60 years. After performing an otorynolaryngological subjective and physical examination, each patient was subjected to a caloric test according to Fitzgerald-Hallpike in the VNG test using a water calorimeter. Then the Dix-Hallpike maneuver was performed, while the activity of semicircular canals in the VHIT test was determined on the basis of the GAIN value. In randomly selected patients, posturographic examinations were additionally carried out. Results: Based on the positive Dix-Hallpike maneuver, damage in the posterior semicircular canal in group I was found in 9 women (9.0%) and 4 men (4.0%), while a negative maneuver was noted in all people in group II. In group I in the VHIT study, damage to the semicircular canals most often concerned the following parts: anterior in 29 women (44.60%) and in 15 men (42.85%); posterior, in 19 (29.23%) and 8 (22.85%), respectively; lateral, in 3 (4.61%) and 3 (8.57%), respectively. Conclusions: The VHIT study in detail assesses the labyrinth canal deficit in all cases of peripheral and mixed dizziness, inc-luding otolithic origin, but does not allow for evident differentiation of BPPV with mixed dizziness of other origin. The VHIT test is a recommended diagnostic tool in BPPV in combination with other positional tests that allow to determine otolithiasis, such as: Hallpike test for the posterior semicircular canal. The VHIT test should be analyzed against the background of other diagnostic tests that can be performed in a patient with dizziness and balance disorders.

Objective. To relate the authors’ experience to the diagnosis and follow-up of patients with benign paroxysmal vertigo of childhood (BPV) who were followed-up at the Children’s Hospital of Bydgoszcz between 1999 and 2004, and to review and discuss controversial issues regarding the disease. Methods. Among 124 children suffering from vertigo 14 were classified as having BPV. All the children were submitted to differential diagnosis protocol which consisted of meticulous history, otolaryngological, ophthalmological, psychological, neurological examination, biochemical tests and standard neurootological examination including caloric tests. The children were followed-up and the tests were repeated if no improvement was observed. Results. All the children suffered from episodic vertigo of variable intensity and frequency. All of them were neurologically intact. In 8 patients pathologic ENG results were found, only 1 patient with canal paresis could be considered as having peripheral lesion, 7 patients had central/mixed pathology. The follow-up was favorable in majority of patients. Six of them recovered completely, in 6 an improvement was noted and in 2 no improvement was observed. Three patients after remission of BPV attacks developed migraine. One child before development of BPV attacks suffered from paroxysmal torticollis of infancy. Conclusions. Childhood BPV is a disorder of vestibular system with the onset occurring mainly in preschoolers aged 1–7. Older children with the onset of BPV - like symptoms should be suspected for functional background of the disease. There are no typical ENG features for BPV. The only objective evidence of vestibular dysfunction is the presence of nystagmus during the attack. The disease is probably of vascular origin and there is strong evidence for close relationship between spasmodic torticollis, BPV and migraine.

Vertigo as One of the Symptoms of Lyme Disease

Objectives: The aim of the study was to evaluate the frequency of vertigo symptoms and potential labyrinth damage in patients with diagnosed Lyme disease (LD). LD can affect the vestibulocochlear nerve, leading to hearing loss and vertigo/dizziness. Material and Methods: The study included a group of 38 patients between the ages of 20 and 77, who were hospitalized due to vertigo/dizziness between 2018 and 2019. All of the patients underwent a detailed medical interview and an otolaryngological and neurological examination, including video electronystagmography (VENG), in addition to audiological and diagnostic tests. Additionally, ELISA and Western blot tests were performed to confirm the diagnosis of LD. Results: In 20 patients (53%), the Romberg trial was positive (p < 0.001). The degree of vestibular dysfunction as shown by the VENG test was associated with the rate of hearing loss as confirmed by the Auditory Brainstem Response (ABR) test (p = 0.011), and it mainly concerned high-frequency sounds (p = 0.014). Conclusion: Vertigo can be a symptom of LD. It is often associated with labyrinth and hearing-organ damage, which can imply that the inner ear or nerve VIII is dysfunctional in the course of this disease. Antibiotic therapy is effective in reducing otoneurological symptoms.

Introduction. In the literature there are some discrepancies concerning the occurrence of vertigo and disequilibrium, as well as the type of vestibular dysfunction in patients after mild traumatic brain injury (MTBI). Objective. The aim of the study was to assess the type of vestibular system dysfunction in MTBI patients after injury at baseline and 6-month follow-up. Materials and method. From a group of 188 patients with vertigo/dizziness and balance instability after MTBI, prospective analysis in 50 patients with abnormal videonystagmography (VNG) were conducted. Anamnesis, otoneurological examination, self-assessed Dizziness Handicap Inventory (DHI) and VNG test were carried in each patient twice – at baseline and after 6 months. Results. A significant improvement in the DHI total score (9.8 points, p<0.001) and in the subscales was found after 6 months. At baseline, VNG test showed the peripheral vestibular system dysfunction in 19 cases (38%), central vestibular dysfunction in 20 patients (40%) and mixed vestibular dysfunction in 11 cases (44%). After 6 months, the VNG records normalization was observed in 19 patients (38%), an improvement was significantly more frequently related to peripheral disorders, compared to central ones (p<0.001). Conclusions. In patients after mild traumatic brain injury, about one-fourth of them had vestibular system dysfunction with the same frequency of peripheral, central and mixed lesions in videonystagmography. Subjective post-traumatic symptoms and vestibular system dysfunction in patients after MTBI decrease after 6-month follow-up. However, more than half of patients still have abnormal VNG results, which indicate dysfunction of the central vestibular system.

Introduction: Videonystagmography (VNG) using a rotating chair is a recent tool for examining the vestibular system with infrared-sensitive cameras for ocular movement registration. This method aids in the differential diagnosis and treatment of dizziness. In practice, it can also be used by skilled physiotherapists as a complement to the diagnostics of balance disorders. Aim: The aim of the presented project is to evaluate whether VNG using a rotating chair in a physiotherapist’s practice is a useful tool to serve as an addition to clinical diagnostics in the assessment and planning of rehabilitation. Materials and methods: During a three-month period of study at the neurology department, 12 out of 42 patients with vertigo or dizziness were included for further analysis. The group consisted of eight women and four men aged 27–79 years. The study included only patients with vertigo of unspecified aetiology. VNG testing involving BURST protocol was performed by a physiotherapist to confirm the origin of the dizziness. Furthermore, the patients completed the Dizziness Handicap Inventory (DHI) questionnaire. Following the tests, a tailored set of physiotherapeutic exercises was prescribed and performed by the patient for two days. Subsequently, the patients underwent re-examination using a rotatory chair with VNG. Results: Among the 12 patients examined, the overall quality of life improved after therapy, as evaluated through the DHI questionnaire. This improvement was correlated with elevated fixation index scores post-therapy. Conclusions: The use of a rotating chair has enabled to apply the optimal physiotherapy treatment. However, further research is needed to develop patient-specific therapy using VNG testing.
