Introduction: Voice express the psyche and personality of a person. Psychogenic dysphonia is called Phononeurosis. Neurosis, depression or family, occupational and social conflicts are the cause of voice disturbances. The most frequent type of dysphonia is hyperfunctional dysphonia, rarer – hypofunctional type. Aim: The aim of this study is an analysis of voice quality and diagnosis of clinical type of psychogenic dysphonia. Material and methods: The analyzed group consisted of 50 patients with voice disorders treated in 2017 and the control group – 30 people with physiological voice. In the diagnosed group 60% of patients were treated for neurosis, 12% due to depression, the others reported conflict situations. In the diagnosis of clinical type of psychogenic dysphonia GRBAS scale was used, maximum phonation time (MPT) and type of breathing were assessed. The visualisation of the larynx was performed using High Speed Digital Imaging (HSDI) technique. The parametric acoustic evaluation of voice was conducted. Results: The most often clinical type of psychogenic dysphonia was hyperfunctional dysphonia, rarer hypofunctional type and vestibular voice. Dysphonia occurred the most often in women during the highest professional activity period. In the diagnosis of clinical type HSDI technique was especially useful allowing to visualization of the real vocal fold vibration and objective differentiation of hyper- and hypofunctional dysphonia. The acoustic analysis of the voice objectively confirmed the presence non-harmonic components – noise generated in the glottis in hypofunctional dysphonia. Disturbances in the way and breathing type caused irregularities in respiratory-phonic and articulation coordination.

Introduction: Hyperfunctional dysfunction is one of the most common functional dysphonia, cha-racterized by voice insufficiency with excessive tightening of the muscles inside and outside the larynx during phonation. To make the treatment process more effective, new ways of rehabilitation are constantly being sought and developed. The aim of this work is to evaluate the effectiveness of laryngotaping - an innovative method of taping around the larynx and neck muscles. Material and method: 10 patients with diagnosed hyperfunctional dysphonia participated in the study. Using the kinesiotaping principles, for 7 days, the suprahyoid and infrahyoid muscles, ster-nocleidomastoid muscles as well as the thyroid cartilage were taped. Before and after the therapy, the patients completed the VHI voice self-evaluation questionnaire. The evaluation of the larynx according to the L. Mathienson scale was also assessed palpation. Results: Analyzing the results of the VHI questionnaire and evaluation of palpation evaluation of the larynx before and after the therapy, statistically significant differences were observed. The results on average decreased by half, which is the desired effect of therapy. Discussion: The results confirm the positive impact of kinesiotaping around the larynx. However, more research is needed on a larger group of patients to fully evaluate the therapeutic effect. Conclusions: 1. Laryngotaping is an effective way to normalize muscle tone, and thus to improve the quality of the voice. 2. The presented studies require continuation, however, positive reception of the introduced therapy by patients encourages further research on a larger group of patients.

Introduction: Voice disorders in children especially in the school-age period comprise a major and challenging medical, social and educational issue. There is a lack of epidemiological data regarding children’s voice in Poland which limits the development of preventive and medical pediatric procedures in our country. Objective: The aim of study was to determine the prevalence of voice disorders in children starting school education in Mazovia. Material and method: The study was a retrospective analysis of survey data from parents of children attending the first grade of primary school. The survey consists of five questions describing the voice. A statistical analysis of 7891 questionnaires was carried out. Results: Analysis of the survey showed a prevalence of dysphonia in 12.8% children aged 7 starting primary school. In this study, voice disorders were observed more often in boys (14.6%) than girls (10.8%). The authors presented the correlation between voice disorders and the expression of emotions. Conclusions: Studies have shown that the percentage of children with voice disorders is significant in our country. The survey regarding voice in school children in Mazovia shows preliminary results of voice disorders in the pediatric population in Poland.

Introduction: Coexistence of dysphagia with voice disorders is a topic rarely raised in the literature. Particular attention is paid to the aspect of dysfunction of laryngeal and pharyngeal muscles. Aim: The aim of the study was to analyse cases of patients with dysphonia in relation to coexistence of swallowing disorder. Material and method: The material of the study included 515 patients hospitalised due to dysphonia in 2018. Patients whose interview indicated swallowing difficulties were subjected to additional diagnosis for dysphagia (FEES, extended speech therapy test, SEMG). R esults: 11.8% of people requiring treatment for voice disorders reported coexistence of swallowing difficulties. Dysphagia was diagnosed in 9.3%. The percentage of respondents diagnosed with swallowing disorder differed depending on the type of underlying disease and was the highest in the group with neurological disorders. Analysis of the correlation between the severity of dysphagia (according to the assessed grade, DHI, EAT-10 results) and the severity of VHI showed a weak correlation between VHI and EAT-10 (p = 0.1), statistically significant correlations (p < 0,05) between the value of VHI and RSI in people with diagnosed neurological disease, between the value of VHI and DHI in people with hyperfunctional dysphonia and the value of VHI and BMI and EAT- 10 in people with chronic laryngitis. Moreover, statistically significant correlations were found between the severity of dysphagia and EAT-10 and DHI (p < 0.05). The speech therapy test indicated the co-existing problem of non-normative swallowing pattern. The electromyographic study showed the largest asymmetries in recording the average and maximum amplitude from masseters. C onclusions: Treatment of patients with voice disorders requires interdisciplinary care. A history of dysphagia in these patients should complement the medical history of voice disorders. The characteristics of swallowing disorders vary depending on the cause of the voice disorder and their co-occurrence affects on average 9.3% of patients. Coexistence of muscle tension dysphagia with voice disorder requires separate diagnostic protocol. Logopaedic procedure ought to be a key element in the interdisciplinary care of patients suffering from muscle tension dysphagia.

Objectives: Occupational voice disorders are accompanied by increased tension of the external laryngeal muscle which changes the position of the larynx and consequently disturbs the conditions of functioning of the vocal tract. The aim of the study is to assess the use of osteopathic procedures in the diagnosis and treatment of occupational dysphonia. Material and Methods: Study subjects included 40 teachers with chronic diseases of the voice organ (38 women and 2 men) aged from 39 to 59 (mean age: 48.25). Before and after the voice therapy the osteopathic examination according to Libermann’s protocol was performed as well as phoniatric examination including laryngovideostroboscopy (LVSS), assessment of the maximum phonation time (MPT) and the Voice Handicap Index (VHI) score. The voice therapy, scheduled and supervised by a laryngologist-phoniatrician and conducted by a speech-language pathologist, was supplemented with osteopathic myofascial rehabilitation of the larynx. The chisquare McNemar test and non-parametric Wilcoxon matched pairs test were applied in the statistical assessment. Results: The applied interdisciplinary treatment including osteopathic and vocal therapy resulted in a statistically significant decrease in tenderness of muscles raising the larynx (cricothyroid ligament, sternocleidomastoid muscles, and pharyngeal constrictor muscles) and in lowering the tonus (geniohyoid muscles, pharyngeal constrictor muscles and sternocleidomastoid muscles). A significant improvement was also observed in the case of dysfunction of the cricothyroid joint examined during glissando and yawning, as well as in asymmetry of the thyrohyoid apparatus. Moreover, the therapy resulted in significantly better normalization of the head position and better control of the centre of gravity of the body. Statistically significant post-therapy improvement was observed in the phoniatric examination, including VHI scores, MPT results and parameters of videostroboscopic examination. Conclusions: The use of osteopathic therapy helps significantly improve the functions of the vocal tract in patients with occupational dysphonia.

Opis:
The work assessed clinical utility of open quotients measured during laryngovideostroboscopy and electroglottography. Values of the parameters were analyzed according to clinical diagnosis. An algorithm based on direct and indirect mucosal wave measurement is presented, which allows for differentiation of voice disorders. The method developed for the objectification of glottal functions in various voice pathologies is characterized by high sensitivity and specificity.

Introduction: Laryngeal paralysis deteriorates all laryngeal functions. Therefore the therapeutic process must include restoration of respiratory, defensive and vocal function. Selection of a proper rehabilitation path plays a key role. Appropriate research protocol that includes objective methods of voice evaluation is an important element of monitoring the return of vocal efficiency. Voice efficiency is important for the patient particularly due to psychological and social reasons. Aim: The aim of the study was the assessment of short-term functional voice therapy (FVT) in patients with unilateral paralysis of the larynx with the use of objective parameters describing the glottis and voice quality. Material and Method: In the last 10 years 355 patients with laryngeal paralysis were hospitalized in the Audiology and Phoniatrics Clinic due to dysphonia. All patients underwent a 5-day FVT. In 2015 we unified the diagnostic protocol measuring parameters obtained from videostrobokymography (VSK), electroglottography (EGG), perceptual and acoustic voice analysis before and after 5-day hospitalization. Results: After FVT patients showed improved voice quality and glottal compensation. The majority of patients achieved a statistically significant improvement in VSK, EGG, MDVP, and perceptual analysis. The group of patients with unsatisfactory voice improvement after therapy required a prolonged rehabilitation or was qualified for laryngeal microsurgery. Potential causes of insufficient effects of FVT were analysed. Conclusions: The complexity of voice rehabilitation is crucial for the success of therapy. An interdisciplinary therapeutic team plays a significant role during voice rehabilitation in patients with vocal fold paralysis.

Degenerative lesions with hyperostosis on the anterior surface of cervical spine are common in clinical practice. In addition to pain being an effect of spinal dysfunction, they sometimes cause difficulties in swallowing or speaking as well as breathing disorders. A 52-year-old farmer with 4-year history of gradually intensifying dysphagia was admitted to hospital due to inability to intake a solid food, significant weight loss, and because of the appearance of a new symptom – dysphonia. Previous conservative treatment for swallow difficulties was ineffective. CT revealed a bone excrescence on the anterior surface of two cervical vertebrae which caused an oesophageal obstruction and compression of the vocal folds. Structural abnormalities of cervical spine should be considered in differential diagnosis of symptoms from the oesophagus and upper respiratory tract, especially when a first-line conservative treatment is not effective. In these cases, surgical removal of the osteophyte is an effective way of treatment.

Wprowadzenie: Porażenie krtani pogarsza wszystkie funkcje tego narządu. Z tego powodu proces terapeutyczny powinien obejmować przywrócenie funkcji oddechowej, obronnej i głosowej. Dobór właściwej ścieżki rehabilitacyjnej odgrywa kluczową rolę. Odpowiedni protokół badań, który obejmuje obiektywne metody oceny głosu, jest ważnym elementem monitorowania powrotu sprawności głosu. Wydajność głosu jest ważna dla pacjenta szczególnie ze względów psychologicznych i społecznych. Cel: Celem pracy była ocena krótkoterminowej funkcjonalnej terapii głosu (functional voice therapy; FVT) u pacjentów z jednostronnym porażeniem krtani, z wykorzystaniem obiektywnych parametrów opisujących aparat głosowy krtani i jakość głosu. Materiał i metody: W ciągu ostatnich 10 lat w Klinice Audiologii i Foniatrii Instytutu Fizjologii i Patologii Słuchu z powodu dysfonii hospitalizowano 355 pacjentów z porażeniem krtani. Wszyscy chorzy zostali poddani 5-dniowej FVT. Od 2015 roku ujednolicono parametry pomiaru protokołów diagnostycznych uzyskiwane z wideostrobokimografii (videostrobokymography; VSK), elektroglottografii (electroglottography; EGG) oraz percepcyjnej i akustycznej analizy głosu przed i po 5 dniach hospitalizacji. Wyniki: U pacjentów po FVT uzyskano poprawę jakości głosu i kompensacji aparatu głosowego krtani. Większość pacjentów osiągnęła statystycznie istotną poprawę w VSK, EGG, MDVP oraz analizie percepcyjnej. Grupa pacjentów z niezadowalającą poprawą głosu po terapii wymagała przedłużonej rehabilitacji lub została zakwalifikowana do mikrochirurgii krtani. Analizowano potencjalne czynniki, które mogły powodować niedostateczne efekty FVT. Wnioski: Złożoność rehabilitacji głosu jest kluczowa dla powodzenia terapii. Interdyscyplinarny zespół terapeutyczny odgrywa istotną rolę w rehabilitacji głosu u pacjentów z porażeniem fałdów głosowych.
