Opis:: Inflammation plays an important role in the development of many obesity-related diseases. This study aimed to investigate the effect of ezetimibe on inflammation and myocardial remodeling in obese rats. A rat model of obesity was established, and myocardial damage was examined by transmission electron microscopy and Masson staining. Twenty obese rats were divided into two groups (n=10): obese group and ezetimibe group. Ten SD rats were used as controls. Western blot was performed to monitor the expression of P-p38MAPK and interleukin (IL)-6. Immunohistochemical staining was used to monitor the expression of intercellular adhesion molecule-1 and vascular cell adhesion molecule-1. In the obese rats group, we observed increased inflammatory factors and myocardial hypertrophy. In contrast, the ezetimibe group exhibited decreased expression of inflammatory factors and an improvement in myocardial remodeling compared to the obese group. Mechanistically, we found that ezetimibe decreased P-p38MAPK, IL-6, intercellular adhesion molecule-1, and vascular cell adhesion molecule-1 levels in the hearts of the obese rats. Taken together, these results indicate that ezetimibe may improve myocardial remodeling in obese rats by inhibiting inflammation.

Tytuł:: Technological aspects of the liquisolid tablets formulation.
Aspekty technologiczne sporządzania tabletek z frakcją płynną.

Opis:: Sporządzanie tabletek z frakcją płynną jest jedną z technologicznych metod zwiększania rozpuszczalności substancji trudno rozpuszczalnych w wodzie. Substancją leczniczą, której roztwór stosowano przy wytwarzaniu formulacji był ezetymib - zakwalifikowany do II klasy BCS, praktycznie nie rozpuszczalny w wodzie, dobrze przenikający przez bariery biologiczne. Badania polegały na ocenie możliwości sporządzenia tabletek z frakcją płynną ezetymibu. Projektowanie składu formulacji opierało się na zastosowaniu różnych substancji o charakterze powlekającym, modyfikacji wartości współczynnika R i dodatku innych substancji pomocniczych. Składniki masy tabletkowej, jak i wytworzone tabletki charakteryzowano na podstawie wyników przeprowadzonych badań farmakopealnych. Ocenę dostępności farmaceutycznej tabletek z frakcją płynną prowadzono w 0,45% roztworze laurylosiarczanu sodu w buforze octanowym o pH= 4,5 oraz w 0,1mol/L roztworze HCl o pH= 1,2. Wyniki otrzymane dla tabletek z frakcją płynną ezetymibu porównywano z wynikami uzyskanymi dla tabletek konwencjonalnych DCT oraz z preparatem referencyjnym Ezetrol®. Z przeprowadzonych badań wynika, że sporządzenie tabletek z frakcją płynną wpływa na zwiększenie dostępności farmaceutycznej ezetymibu z tabletek. Formulacjami o najlepszych właściwościach okazały się te, w których jako substancję powlekającą zastosowano krzemian magnezowo-glinowy.
The liquisolid tablets formulation is one of the technological methods of enhancement the solubility of poorly-water soluble drugs. A drug substance used in all formulations was ezetimibe - classified into II class of BCS, practically insoluble in water, highly permeable. The study consisted of evaluation the possibility of liquisolid tablets with ezetimibe preparation. Designing the composition was based on the use of different coating materials, a modify R index value and addition of various excipients. Both, the excipients and the tablets, were characterized based on the pharmacopeal research results. Evaluation of farmaceutical availability was carried out in 0,45% SDS in pH 4,5 acetate buffer and in 0,1N HCl (pH= 1,2). The results obtained for the liquisolid tablets were compared to the results obtained for DCT formulations and Ezetrol®. The study shows that the liquisolid tablets formulations increase the farmaceutical availability of ezetimibe. The best coating material was magnesium aluminometasilicate.

Opis:: Cardiovascular (CV) diseases are still the leading cause of death in Poland. In treatment of hypercholesterolemia, HMG-CoA reductase inhibitors (statins) are the first-line drugs with proven beneficial effect on patient prognosis. These drugs include one of the two potent statins, atorvastatin. If the target level of LDL cholesterol is not achieved despite lifestyle changes and statin therapy, a drug with a different mechanism of action, e.g. ezetimibe, is added to the treatment. In this way a greater hypolipemic effect is achieved. Single pill combinations (SPC) containing two active substances in one tablet are a good choice. Indications for therapy with SPC preparations with statins and ezetimibe include therapy for primary hypercholesterolemia, mixed hyperlipidemia, heterozygous and homozygous familial hypercholesterolemia, and prevention of CV events in patients at increased risk for these incidents. SPC drug is used to control hypercholesterolemia with atorvastatin and ezetimibe as separate formulations, at the same doses as in the combination formulation. SPC formulations containing 10 mg of ezetimibe in combination with 10 or, 20, or 40 or 80 mg of atorvastatin are available on the Polish pharmaceutical market.

Optymalizacja składu oraz metody sporządzania tabletek z frakcją płynną ezetymibu

Opis:: Ezetymib, zaklasyfikowany do II klasy według Systemu Klasyfikacji Biofarmaceutycznej (BCS), charakteryzuje dobrą przenikalnością przez bariery biologiczne a jednocześnie jest praktycznie nierozpuszczalny w wodzie. W celu poprawy tego parametru zastosowano technologię sporządzania tabletek z frakcją płynną. Przeprowadzone badania miały na celu ocenę możliwości poprawy dostępności farmaceutycznej substancji leczniczej przy zastosowaniu technologii liquisolid. Głównym celem było ustalenie optymalnego składu tabletek, obliczonego na podstawie matematycznych zależności przestawionych przez Spireas’a i Bolton’a. Pod uwagę wzięto także rodzaj oraz dobór proporcji użytych substancji pomocniczych. Formulacje tabletek sporządzono przy pomocy tabletkarki uderzeniowej a następnie poddano ocenie pod kątem wymagań zawartych w Farmakopei IX. Ocenę dostępności farmaceutycznej przeprowadzono przy pomocy dwóch płynów akceptorowych, a otrzymane wyniki porównano z wartościami uzyskanymi dla preparatu referencyjnego Ezetrol ®. Najlepszymi właściwościami spośród sporządzonych formulacji tabletek liquisolid odznaczały się te, w których jako nośnik zastosowano bezwodny wodorofosforan wapnia, natomiast jako substancję powlekającą- krzemian magnezowo-glinowy.
Ezetimibe is classified to II class of the Biopharmaceutical Classification System (BCS) because of its high permeability and low solubility in water. In order to improve this parameter liquisolid technology was utilized. The aim of the study was to evaluate the possibility of improving drug release using liquisolid technology. The main objective was to determine the optimal composition of tablets, calculated on the base of the mathematical relationships presented by Spireas and Bolton. Type of excipient and they proportions were taken into consideration. Formulations were prepared using the eccentric tablet press and then evaluated against requirements contained in the Polish Pharmacopoeia IX. The evaluation of the pharmaceutical availability was performed with two acceptor fluids and the results were compared with values obtained for the reference product- Ezetrol ®.The best properties of the prepared liquisolid tablets were obtained for tablets, where anhydrous dibasic calcium phosphate was used as a carrier and magnesium aluminometasilicate as a coating agent.

Opis:: A good therapeutic option in hypercholesterolemia is the use of combination preparations containing an HMG-CoA reductase inhibitor, such as atorvastatin, and a drug that selectively inhibits the absorption of holesterol and plant sterol derivatives from the gut - ezetimibe - in a single tablet. Such treatment can be implemented in adults with primary hypercholesterolemia, familial hypercholesterolemia or mixed hyperlipidemia, which is already controlled with atorvastatin and ezetimibe as separate preparations, at the same doses as in the combination preparation. The preparation is administered 1x /day at any time, day. It is mostly a safe drug and well tolerated by patients. Possible side effects are mainly due to possible side effects of statins.
