Opis:
The role of Sildenafil was found in the potential weight gain of the foetus in pregnant women with intrauterine growth retardation (IUGR). Sildenafil acted by inhibiting phosphodiesterase type 5 (PDE5), which was an important enzyme responsible for the degradation of Cyclic guanosine monophosphate (cGMP), resulting in prolongation of action of cGMP and therefore prolonged smooth muscle relaxation and vasodilation of smooth muscle. The mechanism behind the beneficial effect on IUGR was its vasodilator action and, hence, an increase in uteroplacental circulation. But the main issue during anaesthesia was fear of severe hypotension, ocular side effects, and bleeding due to the effect of sildenafil. which was expected in pregnant women taking Sildenafil for intrauterine growth retardation undergoing caesarean section. Severe hypotension was due to augmentation of hypotensive effect of sildenafil and regional anaesthesia. A 29-year-old female, previously healthy, non-smoking, at 33 weeks’ period of gestation presented with complaints of headache and pedal edema in third trimester and was diagnosed with gestational hypertension. On the 28th day of admission, she had episodes of elevated blood pressure. The patient was considered for emergency caesarean section after taking high-risk consent. Techniques for cesarean sections include the single shot spinal technique, epidural catheter technique, or the combined spinal-epidural technique (CSE). While spinal anaesthesia offers a simpler approach with a faster and more reliable onset of surgical anaesthesia, it may be associated with abrupt hemodynamic changes. On the contrary, epidural anaesthesia allows for a gradual onset of sympathectomy, titrated dosing, and postoperative analgesia. In this case, the decision was made to opt for epidural anaesthesia over spinal approach was made to allow for careful drug titration and enhance hemodynamic stability. We conclude that graded epidural epidural anaesthesia played an important role in the prevention of hypotension that was expected in pregnant women taking Sildenafil for intrauterine growth retardation undergoing caesarean section.

The purpose of this study was to evaluate the effects of moderate-intensity resistance exercise on postexercise hypotension (PEH) in the hypertensive. The study was conducted with eighteen hypertensive elderly individuals (20–30 years). They were subjected to two experimental sessions: control session (SC) and 50% (S50%) of 1RM. For each session, subjects were evaluated pre-and postintervention. In the preintervention, the blood pressure (BP) and FVR were measured after 10 min of rest. Thereafter, they were taken to the gym to perform the exercise sessions or remained at rest in each of the equipment during the same time.In the S50% group was composed of a set of ten repetitions of ten exercises, with an interval of 90 s between exercises. Subsequently,the FVR and BP measurements were again performed at 15, 30, 45, 60 and 75 min of recovery (postintervention). The PEH was greater in S50% compared with SC, with the lower value of BP being found at 75 min of recovery for the two sessions (systolic BP:125.21 ±0.98mmHg versus 145.45 ±1.72 mmHg; diastolic BP: 83.60 ±1.67 mmHg versus 95.14 ±0.74 mmHg respectively). Moderateintensity resistance exercise was effective in promoting PEH, this phenomenon being accompanied by a reduction in FVR within the first minute of recovery in the hypertensive young.

People are becoming increasingly sedentary leading to a rise in clinical conditions such as hypertension or high blood pressure that has major impact on public health concern and social care. With higher blood pressure the heart has to pump harder and the arteries have to carry blood that is flowing under greater pressure in which can lead to an increases risk of a heart attack, heart failure, stroke or kidney damages and disease. Physical activity is considered to be an antihypertensive strategy to prevent or control of high blood pressure in the form of post exercise hypotension. Post exercise hypotension emphasis to lifestyle modification rather than a sole reliance on pharmacological therapy. It has been reported for every 1 MET increase in fitness level, mortality risk decreases by 11% in people with high blood pressure. Subsequently researched indicated an optimal health benefit can be achieved through moderate dose of exercise intensity at 3–6 METs for all adult aged 18-65 year that seems to be a well-designed strategy to prevent cardiovascular problem such as high blood pressure. However, there are few national programmes to serve as models for prevention and control of hypertension and few countries have embarked on national hypertension prevention and control programmes on the exact magnitude of moderate – intensity at 3–6 METs .The application of moderate intensity exercise based on metabolic equivalent may lead to a new approach to tackle hypertension.

Spontaneous intracranial hypotension : two steroid-responsive cases

Purpose: Spontaneous intracranial hypotension (SIH) is characterised by orthostatic headache, low cerebrospinal fluid pressure and diffuse pachymeningeal enhancement after intravenous gadolinium contrast administration. Magnetic resonance imaging (MRI) often plays a crucial role for correct diagnosis. Case description: We described two similar cases of SIH, whose clinical and imaging features are typical for this pathology. At MRI brain scan, both patients showed diffuse and intense pachymeningeal enhancement and moderate venous distension and epidural vein engorgement. The two patients were treated with bed rest and oral steroid therapy, with complete and long-lasting symptomatic relief. Conclusions: Orthostatic nature of headache is the most indicative clinical feature suggesting SIH; contrast-enhanced MRI provides definite imaging diagnostic findings. Conservative treatment coupled to steroid therapy is often sufficient to obtain complete disappearance of symptoms.

Zaburzenia posturalne obserwowane w pracy fizjoterapeutów głównie kojarzone są z dysfunkcjami narządu ruchu. Niejednokrotnie prowadzona terapia nie przynosi oczekiwanych efektów dotyczących wyeliminowania wszystkich objawów zgłaszanych przez pacjentów. Okoliczność ta zmusza terapeutów do poszukiwania innej przyczyny dolegliwości chorego. Praca z pacjentem z dysfunkcją przepony objawiającą się jej hipotonią oraz towarzyszącymi objawami niejednokrotnie może wydawać się niezwykle trudna. Związane jest to z problematyczną diagnostyką nakładających się dolegliwości. Cel pracy: Celem niniejszej pracy jest ukazanie możliwości postępowania fizjoterapeutycznego polegającego na skojarzeniu wielu metod stosowanych w rehabilitacji, mających za zadanie usprawnienie terapii pacjenta z hipotonią przepony. Opis przypadku: 28-letni mężczyzna skarżący się na dolegliwości układu pokarmowego i oddechowego z dodatkowymi objawami takimi jak: brak regularnego snu, przyspieszony oddech oraz ból w okolicy karku i odcinka piersiowego kręgosłupa promieniujący w stronę żeber, został poddany diagnostyce fizjoterapeutycznej. W opisywanym przypadku zastosowano elementy reedukacji wzorca oddechowego, terapii manualnej, terapii punktów spustowych, masażu powięziowego, poizometrycznej relaksacji mięśni, kinesiologytapingu oraz treningu funkcjonalnego w celu normalizacji napięcia oraz uwolnienia pracy przepony i żeber, zmniejszenia zgłaszanych dolegliwości bólowych kręgosłupa, reedukacji posturalnej, stabilizacji obręczy brakowej i odtworzenia prawidłowych wzorców ruchowych łopatki i kończyny górnej oraz utrwalenia osiągniętych efektów i zapobiegania nawracaniu dolegliwości. Wnioski: Wykorzystanie wielu metod fizjoterapeutycznych przyniosło wymierny efekt oraz spowodowało znaczne ograniczenie zgłaszanych dolegliwości. Odnotowano znaczącą poprawę stanu zdrowia i wydolności fizycznej pacjenta.
Postural disorders observed in the work of physiotherapists are associate with dysfunctions of the locomotory organ. Most of the therapy does not bring the expected effects of eliminating all symptoms reported by patients. This circumstance makes therapists to look for another cause of the patient dysfunction. Conducting a patient with hypotension diaphragm and accompanying symptoms may often appear to be extremely difficult. This is related to the problematic diagnosis of overlapping complaints. Aim of the study: The aim of this paper is to present the possibility of physiotherapeutic treatment combining many methods used in rehabilitation aimed at improving the effectiveness of the patient's therapy. Case report: 28-year-old man complaining of digestive and respiratory ailments with additional symptoms such as lack of regular sleep, rapid breathing and pain in the neck and thoracic spine radiating towards the ribs, underwent physiotherapy diagnosis. In this case, improvement of respiratory pattern, manual therapy, trigger points therapy, fascial massage, postisometric muscle relaxation, kinesiologytaping and functional training were used to ease the tension and release the diaphragm and ribs. It also aimed to reduce spinal pain, postural reeducation, reconstruction of the correct movement patterns of the scapula and upper limb and fixing the achieved effects to prevent the recovery of the ailments. Conclusion: The use of many physiotherapeutic methods has resulted in measurable effect and caused a significant reduction of reported complaints. Significant improvement in the patient's health and physical efficiency was noted.
