Bolus calculator feature (BC) is one of the most important options of modern insulin pumps. It enables both periprandial as well as correction insulin dosing. BC function leads to improvement of patient’s metabolic control. It makes the treatment safer andlimits the number of hypoglycemic episodes. In presented article we propose preliminary setting of BC in the Accu-Chek Combo system. It is obvious that those setting should be always verified according to a clinical situation.

Efficacy and safety of insulin pump treatment in adult T1DM patients : influence of age and social environment

Introduction and objective. Continuous subcutaneous insulin infusion (CSII) via personal insulin pump is a valuable therapeutic tool in T1DM patients. However, adherence to recommended CSII- related behaviours may be of concern to young adults with intensive, variable daily activities (students, young professionals). The aim of this observational study was to estimate treatment outcomes in young adult patients with T1DM, and compare them with older individuals. Materials and methods. Overall, 140 adults with T1DM on CSII were examined, divided into 2 subgroups: 77 patients younger than 26 years of age (mean 20.6 years) and 63 older subjects (mean 39.0). We compared the glycaemic control in both groups of T1DM subjects and analyzed treatment attitudes to identify potentially modifiable behaviours influencing the efficacy of the treatment. Results. The younger individuals were characterized by significantly worse treatment outcomes, compared to the older ones: the mean HbA1c levels were 7.6 ± 1.3% and 6.9±1.3% (p=0.00001), while the mean glucose levels based on glucometer downloads were 161±33.6 mg/dL and 136±21.8 mg/dL (p=0.00001), respectively. The frequency of self-monitoring of blood glucose (SMBG) was lower in younger individuals (5.3±2.1 vs. 7.0±2.8 daily, p=0.0005, respectively); they were also less frequently used advanced pump functions, e.g. the bolus calculator (48% vs. 67% users, p=0.0014, respectively). Conclusions. The efficacy of CSII treatment observed in young T1DM adults was worse than in older patients. The reason for this phenomenon remains unclear, it may be due simply to age-dependend behaviours, to social environment, or both.

This paper explains what insulin pump is and outlines basic requirements for such device. The material presented in this paper is an attempt to collect actual knowledge about closed loop insulin delivery systems. From scientific point of view we are going to propose the closed loop insulin-dispensing device with control built on the base of predictive neural network system. The blocks of such systems are listed, and main problems to overcome are defined.

Metabolic control in type 1 diabetes patients practicing combat sports : at least two-year follow-up study

Background: It is well recognized that physical activity should be an integral part of the management of diabetes. It remains controversial, however, whether combat sports, often preferred by young individuals type 1 diabetes mellitus (T1DM), may be performed without high risk of metabolic decompensation. The aim of this observational study was to summarize a two-year follow-up period of five young male patients with T1DM practicing combat sports under the care of a physical-activity oriented specialist diabetes outpatient clinic. Of the five patients, three mixed martial arts and two kick-boxing competitors were included in the study. To control glucose in each patient, an individual approach was used that took into consideration the type of training, the sequence of the exercises, and the relative proportion of different forms of exercise. Findings: During the follow-up, glycemic control was improved and maintained in all individuals. Neither an episode of hospitalization-requiring diabetic ketoacidosis nor severe hypoglycemia occurred in these patients during the follow-up. Conclusions: In conclusion, an individual approach for T1DM patients practicing combat sports may result in achieving and maintaining satisfactory glycemic control without increased risk of metabolic decompensation.

Clinical factors affecting the perception of hypoglycemia in type 1 diabetes patients treated with personal insulin pumps

Introduction and Objective. The ability to perceive the symptoms of hypoglycemia during the early decrease in plasma glucose concentration may be critical for the safety of T1DM patients treated with intensive insulin therapy, including those treated with continuous subcutaneous insulin infusion (CSII). In the presented observational study an attempt was made to asses clinical factors that might affect subjective awareness of hypoglycemia in CSII-treated T1DM patients, with special attention to factors specific for this mode of treatment. Materials and Methods. For the purpose of the study, data of 110 CSII-treated T1DM patients were collected (78 females and 32 males). The records were analyzed from glucose meters (200-300 measurements/download, depending on meter type) and insulin pumps (total insulin dose, basal/bolus ratio, number of boluses/day, bolus calculator and dual wave/square bolus usage, continuous glucose monitoring data) from the last 3 years. Results. It was found that the level of subjective hypoglycemia perception inversely correlated with the number of hypoglycemic episodes per 100 measurements, age, duration of diabetes, time on insulin pump, and positively correlated with mean glycemia (n = 98; r = 0.22; p = 0.0286). With respect to CSII-related factors, hypoglycemia perception inversely correlated with the percentage of basal insulin (n = 106; r = -0.20; p = 0.0354). In stepwise regression analysis, independent predictors for impaired hypoglycemia perception were: age β = -0.29 (p = 0.023), duration of diabetes β = -0.24 (p = 0.029) and number of the hypoglycemia episodes for 100 measurements β = -0.33 (p = 0.0005). Conclusions. Risk factors for impaired hypoglycemia perception in CSII-treated patients include those identified previously for the general population of T1DM individuals. In addition, the presented results suggest that a higher basal/bolus ratio may lead to impairment of the ability to perceive early symptoms of hypoglycemia.

The main purpose of this work is to provide an extensive, simulation-based comparison of robustness of PID and MPC algorithms in control of blood glucose levels in patients with type 1 diabetes and thus answer the question of their safety. Cohort testing, with 1000 simulated, randomized patients allowed to analyze specific control quality indicators, such as number of hypoglycemic events, and length of hypo- and hyperglycemia periods. Results show that both algorithms provide a reasonable safety level, taking into account natural changes of patients’ physiological parameters. At the same time, we point out drawbacks of each solution, as well as general problems arising in close-loop control of blood glucose level.

Oral microbiota - one habitat or diverse niches? A pilot study of sampling and identification of oral bacterial and fungal biota in patients with type I diabetes mellitus treated with insulin pump

Objective: The oral microbiota is a very complex and dynamic microbial ecosystem. Alterations of its balance can result in oral and systemic diseases. We aimed to characterize the microbiota in particular niches of the oral cavity in adult type 1 diabetes patients treated with continuous infusion of insulin with insulin pump (IP). In addition, we aimed to determine optimal sites of oral microbiota sampling in studies of large research groups of patients with DM I. Design: In this pilot study, we sampled the buccal and soft palate mucosa, tongue, palatal and buccal dental surfaces and gingival pockets of adult DM I patients treated with IP. Results: In total, 23 patients were recruited. The oral microbiota was dominated by Streptococus and Neisseria, with a low incidence of cariogenic S. mutans and Lactobacillus, as well as periodontal pathogens such as Prevotella. There were significant differences in overall CFU counts of all strains, Gram-positive, Staphylococci, Streptococci and S. oralis strains between mucosal and dental surface sites. The overall CFU counts of all strains and Gram-positive strains were higher in dental sites vs. mucosal sites (both p < 0.001). CFU counts of S. oralis were significantly higher in dental sites vs. gingival pocket sites (p = 0.013). Candida species were rare. The mucosal sites on the buccae presented lower diversity and bacterial counts. Conclusions: In the study group of adult DM I patients treated with IP, the microbiota in particular niches of the oral cavity was significantly different. Three distinct and optimally appropriate sampling sites for oral microflora were identified: buccal and palatal mucosa, dental surface and gingival pockets. The results of this study may be the basis for further studies of large groups of patients with DM I.

Lack of major impact of implementation of the Advanced Hybrid Closed Loop System in technologically-naive patients with Type 1 Diabetes mellitus on their food choices or weight – a one year follow-up

Introduction and objective: The purpose of this follow-up study on the implementation of advanced closed-loop hybrid insulin pumps in people with type 1 diabetes was to assess the impact of introducing this advanced technology on quantitative and qualitative parameters of diet. Material and methods: 18 patients (8 women and 10 men, mean age 40.9 years) patients using the CE-marked MiniMed 780G AHCL system who completed 1 year of follow-up were included into the study. The research tool was the KomPAN questionnaire with several own questions added, asked in three study periods, concerning the number of meals consumed, general and night snacking, carbohydrate counting, frequency of consumption of various groups of products that affect postprandial glycaemia. Results: Although the mean body weight of the examined group did not increase significantly (from 75.1 kg at the beginning to 75,9 kg at the end), five various individual scenarios of weight change were observed. The eating habits has not changed, but patients began to consume less products containing simple sugars, e.g. fruit preserves, milk chocolate or fish in sauces (p<0.05). No statistically significant correlation was found between the change in body weight at the end of the study and the average amount of carbohydrates entered into the pump from the entire 12 months (p = 0.460). Conclusions: The implementation of AHCL system in technology naïve patients, despite offering more freedom of food choices due to better glycaemic control, did not have a significant impact on patients’ dietary patterns, also did not result in weight gain. This is important since AHCL system offers more freedom of food choices due to better glycaemic control. However, the longer follow up and the study based on larger population is required to finally address the issue of the impact of AHCL on body mass.

Purpose: To determine the level of physical activity in young and early adults with type 1 diabetes in comparison with their healthy counterparts and to determine whether the use of insulin pump facilitates physical activity. Materials and methods: This study included 40 type 1 diabetes (T1D) subjects of both sexes treated with a personal insulin pump therapy, and 30 healthy controls. The diagnostic survey included questions about nutrition, knowledge about the disease and whether the patient can control diabetes through physical activity, diet and self-monitoring. The International Physical Activity Questionnaire – long form (IPAQ-L), was used to assess the level of physical activity of both diabetic and control individuals. Results: 87.5% T1D subjects believe that using an insulin pump facilitates their physical activity. The level of physical activity associated with cycling (p=0.038) and vigorous physical activity (p=0.008) was higher in T1D than in the control group. Statistically significant differences (p=0.043) were found for total physical activity. The total mean activity was higher in participants with T1D (8147.70 MET-min/week) compared to the control group (5857.55 MET-min/week). Conclusions: Young and early adults with type 1 diabetes may be more physically active than their healthy counterparts, mainly in their leisure time. The use of a personal insulin pump facilitates physical activity, but most diabetics experience episodes of hypoglycemia after physical activity.
