Background: Anticoagulants are used mainly in the prevention of venous thromboembolism. The frequency of haemorrhagic complications is correlated with patients’ age and level of International Normalized Ratio (INR). Spontaneous intramedullary spinal cord haemorrhage due to anticoagulation therapy is an extremely rare occurrence. A 62-year old male was admitted to hospital with acute abdominal pain and paraplegia. Because of persistent atrial fibrillation and previous history of ischaemic stroke, on admission the patient was treated with anticoagulant, INR – 15. MRI revealed an abnormal fusiform hyperintesity area in the spinal cord. ‘Haemo’ sequence confirmed the presence of hemosiderin deposits at the level of Th5 – Th7. Anticoagulation therapy might be a cause of spontaneous intramedullary spinal cord haemorrhage. Maintaining high levels of clinical suspicion and utilizing MRI with additional ‘haemo’ option may help in making the right diagnosis.

Opis:
Intramedullary nailing has received increased attention for the treatment of distal femoral fractures. There are many types of nailing systems commonly used. Among them there is locked intramedullary nailing, which combines closed nailing with a special locking piece (bolt), which expands distal part of a nail and anchoring it when a central screw is tightened. Compared to other interlocking nail systems it is very effective and not so invasive way of fractured long bone treatment. However, it is not perfect and has one major disadvantage related to material (metal) and necessity of removing all parts of the system after the bone is recovered. The biggest issue is connected with the locking piece removal. That is why there was a need to find a material for this piece which is biodegradable and strong enough to maintain its mechanical function at the same time. Poly-L-lactide (PLA) is nowadays one of the most used bioabsorbable materials. Unfortunately, this polymer may not meet some application requirements due to inadequate mechanical properties and its degradation characteristics. One of the easiest ways to overcome these obstacles is to introduce an inorganic phase into the polymer matrix to create a composite. In this article a production method of a biodegradable composite (PLA/TCP, 90/10) locking piece of an interlocking intramedullary nail called "OLIVE" and the influence of degradation process in simulated physiological conditions (H2O, PBS) on its mechanical properties is shown. Main stress was placed on the inner thread strength to discover how it changes with time. A special device for mechanical testing machine imitating working conditions was fabricated. 148 Based on SEM observations, pH and electric conductivity monitoring after 14 days no degradation was found. However, mechanical testing shown, that after incubation for such period of time the material was more ductile. Instead of cracking, what was observed for not incubated samples, they changed their shape during the testing process. Nevertheless, it didn't radically influence on the inner thread strength and didn't change mechanical function of the olive. No inner thread destruction was discovered contrary to a metallic screws which in all cases were broken. Preliminary results indicate that proposed composite and an implant made of this material might be used as a locking piece in a locked intramedullary nailing system.

Intramedullary tumours (IMTs) are the least common neoplasms of the spinal canal. The majority of them are ependy­momas and astrocytomas, the third commonest is haemangioblastoma, while other tumours of the spinal cord are relatively rare. This review presents on update on the imaging of spinal cord tumours. Magnetic resonance imaging (MRI) is the imaging method of choice in diagnosing IMTs, with other modalities playing a supplementary role. The authors discuss the MRI protocol in IMTs including advanced techniques and present the imaging features of particular tumours. The differentiation of IMTs from other spinal cord diseases is also presented.

Multiple CNS cavernous haemangiomas presenting with spinal cord haematoma secondary to an intra-medullary cavernous haemangioma : a case report

Background: Spinal vascular malformations are rare. Intra-medullary cavernous haemangiomas are very rare and only few cases have been described. Occasionally they may cause pain, myelopathy with sensory or motor deficit due to mass effect or hemorrhage. Case Report: We report a patient who presented to the emergency department with acute retention of urine and lower limb weakness. He was diagnosed as a spinal intramedullary haematoma secondary to a cavernous haemangioma. In addition, multiple intracranial cavernous haemangiomas were found. Conclusions: We are of the opinion that our case of spinal intramedullary cavernous haemangioma with multiple intracranial involvements deserves attention due to its rarity and it is also an important reminder to examine the whole neuroaxis in patients with spinal intramedullary cavernous haemangiomas, regardless they are symptomatic or asymptomatic.

Opis:
Every day human bones undergo different injuries resulting from public transport accidents, diseases or trauma. Consequently, they cause temporary or constant patients disability or even death. Fractures happen not only to elderly people suffering from osteoporosis, but also more and more frequently to young people, who got some injuries as a result of life style, e.g. doing sports. The desire to enhance the physical well being and quality of patients' life, who got some injury causes constant development of medical devices for osteosynthesis. In the case of complicated fractures, in which plaster treatment is not sufficient, surgical treatment and applying implant are indispensable. The aim of the article is to present two case studies on intramedullary nails. The first one concerns a method of osteosynthesis for a patient, who was injured in an accident. The second one presents the failure of the implanted intramedullary nail. The research included evaluation of the type of fracture and the analysis of microstructure, chemical composition and hardness of the implant materials.

Bone fractures are one of the most commonly seen problems in veterinary traumatology. The authors of this study strive to find a new intramedullary nail, which is intended for treating femoral bone fractures for canine patients. The purpose of this study was to analyze biomechanical parameters of the intramedullary nails, which use a new bolt system concept. Methods: Dissected femoral bones of a large breed dog were cut in order to simulate interfragmentary gap, and then the bones were stabilized using intramedullary nail with locking bolts. Bone-nail systems were subjected to cyclic loading using force which corresponds to the load on the femoral bone in the first few days after surgery. Micro-CT scans were taken of the bone samples around implant in order to determine deformation and structural parameters of bone tissue. Results: The calculation of the bone-nail system stiffness was done through analysis of the forcedisplacement curves recorded during experimental studies. Using monocortical locking bolts resulted in smaller stiffness of the bone-nail system than using bicortical locking bolts. Conclusions: The results obtained in this study can indicate that the intramedullary nail could work well when used for treatment of bone fractures in dogs. The authors focused on using monocortical bolts which provides good stability and adequate biomechanical environment. Described fixation method is easily adjustable to a particular patient individual parameters.

Praca miała na celu ocenę właściwości mechanicznych różnego typu kompozytów o osnowie z polilaktydu pod kątem ich zastosowania na wielofunkcyjne gwoździe śródszpikowe. Zastosowanie resorbowalnej osnowy pozwoli na uniknięcie powtórnej operacji w celu usunięcia implantu, natomiast stosowane dodatki mają na celu zapewnienie odpowiedniej charakterystyki mechanicznej (wzmocnienie polimeru włóknami węglowymi, z alginianu sodu i poliakrylonitrylu) i bioaktywności (ułatwienie osteointegracji przez obecność cząstek ceramicznych z trójfosforanu wapnia). Największe wzmocnienie osnowy PLDL uzyskano poprzez zastosowanie włókien węglowych. Zastosowanie włókien alginianowych zapewniło ponad 100% wzrost wytrzymałości na rozciąganie w stosunku do czystego polimeru, jest to jednak wartość nie wystarczająca na zastosowanie tego kompozytu na gwoździe śródszpikowe. Dodatek cząstek TCP w ilości 15% wag. spowodował niewielkie obniżenie wytrzymałości kompozytu. Optymalnym rozwiązaniem wydaje się być połączenie w jednym kompozycie obu typów włókien, węglowych i alginianowych, dla zapewnienia wysokiej wytrzymałości, przy równoczesnym zastosowaniu kilku procent TCP dla nadania cech bioaktywności, co będzie kontynuowane w kolejnych badaniach.
This paper was aimed to evaluate mechanical properties of various composites with polylactide matrix regarding their application for multifunctional intramedullary nails. Due to the presence of the resorbable matrix, there is no need to perform another operation for implant removal. And used additives are to provide the adequate mechanical characteristic (polymer reinforcement with carbon fibres, sodium alginate fibres and polyacrylonitryle fibres) and bioactivity (facilitated osseointegration due to the presence of ceramic particles with tricalcium phosphate). The most significant reinforcement of PLDL matrix was obtained by using carbon fibres. Alginate fibres provided over 100% increase in tensile strength in relation to pure polymer. But this value was not satisfactory for the application of such composites in intramedullary nails. Adding 15 wt% of TCP particles resulted in slight decrease in the composite strength. The combination of both types of fibres, carbon and alginate ones, in one composite seems to be the optimal solution supporting high strength, at simultaneous addition of a few percent of TCP to provide bioactive features of the material. This issue will be continued in further studies.
