Opis:
In this study a multiaxial isokinetic dynamometer was used to measure strength during various upper-body isokinetic exertions. Ten male participants performed 7 different upper-body isokinetic exertions. In addition, to evaluate the effect of speed on strength, each participant performed sitting pull exertions at the speed of 0.026, 0.130, and 0.260 m/s. Average isokinetic strength increased from 236.6 ± 39.1 to 291.8 ± 65.8 N with the initial increase in speed from 0.026 to 0.130 m/s. The average isokinetic strength decreased to 276.7 ± 87.2 N with a further increase in speed to 0.260 m/s. The curve between isokinetic strength and speed followed a bell-shaped curve (fitted with the Gaussian function, R2 = .9). The results of this study could be useful in deciding on the work pace of various manual material handling tasks requiring maximal and/or near maximal exertions.

This study aimed to examine the reliability of trunk extensor strength assessment with a functional electromechanical dynamometer (FEMD). Methods: Thirty-one men performed strength assessment at different velocities (V) (V1 = 0.15 m·s−1, V2 = 0.30 m·s-1, V3 = 0.45 m·-1) and range of movement (R) (R1 = 25% cm; R2 = 50% cm), and isometric contraction at 90º. Reliability was obtained through the intraclass correlation coefficient (ICC), typical error (TE), and coefficient of variation (CV). Results: The absolute reliability provided stable repeatability of the average eccentric strength in the V1R1 condition (CV = 9.52%) and the maximum eccentric strength in V1R1 (CV = 9.63%) and V2R2 (CV = 9.66%). The relative reliability of the trunk extensor’s average strength was good (ICC = 0.77–0.83) for concentric and good (ICC = 0.78–0.85) and moderate (ICC = 0.67–0.74) for eccentric contraction. Also, good (ICC = 0.77–0.81) and moderate (ICC = 0.55–0.74) reliability of the maximum strength were obtained for concentric and eccentric contraction. The most reliable manifestation to evaluate the concentric (CV = 11.33%) and eccentric (CV = 9.52%) strength was the average strength in the V1R1 condition and the maximum strength (CV = 10.29%) to isometric assessment. The average concentric strength in the V2R2 condition (r = 0.69) and the maximum eccentric strength in the V1R1 condition (r = 0.65) were the best related to the maximum isometric strength. Conclusion: FEMD is a highly reliable device to evaluate trunk extensors strength.

The evaluation of asymmetry in isokinetic and electromyographic activity (sEMG) of the knee flexor and extensor muscles in football players after ACL rupture reconstruction and in the athletes following mild lower-limb injuries

This study was aimed at evaluating knee stabilizer (quadriceps and hamstring) muscle strength and the medio-lateral symmetry of hamstring fatigue in football players after ACL reconstruction and with mild lower extremity injuries. The study comprised 65 professional football players who were divided into three groups: Group 1 (n = 24; 22.7 ± 3.6 years; 175 ± 4 cm; 77.3 ± 7.6 kg) after ACL reconstruction, Group 2 (n = 21; 20.5 ± 3.7 years; 177 ± 6 cm; 74.3 ± 9.1 kg) with mild lower-limb injuries (grade 1 muscle strains) and Group 3 (n = 20; 23.1 ± 2.8 years; 178 ± 6 cm; 75.8 ± 8.8 kg) without injuries in the past 3 years. The concentric isokinetic test (10 knee flexions and extensions at 60, 180 and 300°/s with a 30 s interval for rest) was performed on both limbs. Fatigue symmetry between the medial and lateral hamstrings was measured with sEMG during 60 s of isometric contractions. In comparison to the other groups, the injured leg demonstrated significantly lower values of peak torque for the quadriceps (G1–G2 = 48%, 38%, 14%; G1–G3 = 49%, 25%, 14%) and hamstring muscles (G1–G2 = 36%, 35%, 18%; G1–G3 = 64%, 28%, 17%) as well as lower values of hamstring muscle work (G1–G2 = 262 J, 157 J; G1–G3 = 219 J, 179 J) and power (G1–G2 = 34 W; 11 W; G1–G3 = 29 W, 12 W). No significant differences were noted in strength between Groups 2 and 3. The significantly higher fatigue of the BF compared to the SEM muscle was seen in Group 1 for the involved (mean difference = 0.12) and uninvolved limbs (mean difference = −0.10), but in Group 2, a non-significant trend towards asymmetry was also noted. No asymmetry in hamstring muscle fatigue was determined in Group 3. The results of our study allow us to indicate that active football players who previously met the RTS criteria, had deficits in lower-limb muscle performance 2–3 years after reconstruction, which could lead to ACL re-injury. This observation is potentially of importance because these deficits may not be subjectively reported by such athletes and also may not be visible in regular orthopedic and physiotherapeutic assessment.

The aim of the study was the evaluation of knee joint function in adolescent patients following operative treatment – fixation via arthroscopic or open surgery (arthrotomy), due to tibial eminence fractures. 28 patients, aged from 7 to 16 years, treated operatively between 1994-2009 in four orthopeadic centres underwent evaluation. Evaluation was performed 12-180 months following surgery. Patients were divided into two groups depending on the operative treatment received. Group A consisted of 14 patients who underwent arthroscopic reduction and stabilization. Group B consisted of 14 patients who were treated by open reduction (artrothomy) and stabilization. The results of clinical and radiological examinations and isokinetic tests used in the evaluation declared that operative treatment due to tibial eminence fracture, regardless of surgical method used, does not significantly disrupt knee joint function resulting in a slight weakening of knee joint extensor muscle strength.

Wstęp. Urazy stawu ramiennego stanowią jedną z najczęściej zgłaszanych dolegliwości ze strony narządu ruchu u osób trenujących dyscypliny sportowe, w tym pływanie lub wykonujących zawód wymagający unoszenia ramienia powyżej 90°. Celem pracy była próba oceny ryzyka powstania urazu stożka rotatorów u osób rozpoczynających regularne pływanie. Materiał i metody. Grupę badaną stanowiło 28 osób, które rozpoczęły regularne treningi pływackie. W grupie kontrolnej znajdowały się 24 osoby nietrenujące. W obu grupach przeprowadzono ocenę parametrów siłowych i czasowych w badaniu izokinetycznym dla ruchu rotacji wewnętrznej i zewnętrznej w warunkach skurczu koncentrycznego, przy prędkości 120°/s. Badania zostały przeprowadzone w odstępie 4 miesięcy. Wyniki. Grupy przed okresem treningowym były porównywalne w zakresie parametrów siłowych i szybkościowych. Grupa badana poprawiła się znacznie w zakresie parametrów siłowych i czasowych (p<0,05). Grupa kontrolna poprawiła się istotnie statystycznie jedynie w zakresie parametrów siłowych. Wnioski. 1. Czteromiesięczny trening pływacki wydaje się być wystarczający, aby poprawić parametry siłowe i czasowe mięśni stożka rotatorów, co może zmniejszać urazowość barku. 2. Nawet krótkotrwały trening pływacki może powodować powiększenie się asymetrii w parametrach siłowo-czasowych pomiędzy kończynami na korzyść kończyny dominującej. Należy zatem zwrócić uwagę na trening uzupełniający, który pozwoli na zachowanie symetrii pomiędzy stroną dominującą i niedominującą.
Background. Shoulder injuries are frequent problems in overhead workers or athletes, also swimmers. The aim of the study was rotator cuff injury assessment in young adults, beginning swimming training. Material and methods. A 4-month prospective cohort study was carried out in 52 physiotherapy students from Warsaw Medical University. 28 participants, aged 19-21, took part in swimming training including at least 1 session per week. The control group comprised 24 not training adults. Assessment procedure consisted of a questionnaire including questions pertaining to demographics, anthropometric features, training and injury history. Isokinetic shoulder internal (IR) and external rotation (ER) motion were measured. Strength and speed parameters were similar in both groups at the first assessment. Results. After the 4-month training period, both parameters significantly changed (p<0.05) in the training group. In the control group only strength parameters were better. Conclusions. 1. The 4-month training period was sufficient to improve rotator cuff speed and strength parameters, what possibly can help decrease shoulder injury rate. 2. Even a short term training can increase the difference in the strength and speed parameters in favor of the dominant limb. In swimmers, we should complement swimming training to obtain balance between the dominant and the non-dominant limb.

The aim of the study was to assess the usefulness of isometric torque (IT) and peak torque (PT) of the hamstring to quadriceps muscles ratio (H/Q ratio) in monitoring the effectiveness of physiotherapy (PH) in males after ACLR. Hypothesis: The H/Q ratio is a diagnostic tool for monitoring of the effectiveness of the 6- month PH after ACLR. Methods: Twenty males 6 months after ACLR (ACLR group) and 20 male controls underwent IT and PT (60°/s and 180°/s) bilateral measurements of H and Q muscles. The IT and PT were normalized to body mass, and expressed as relative IT (RIT) and relative PT (RPT). The RIT and RPT H/Q ratios, and Limb Symmetry Index (LSI) were calculated. Results: In the ACLR group, the RIT for the H and Q, the RIT for the H/Q ratio and most of the RPT, as well as the H/Q ratio, ROM and LSI values of the operated knee, were not significantly different (NSD) than those of the non-operated side (NOS) or the control group. The between-group comparison of the H/Q ratio for RIT and RPT weren’t NSD. The isokinetic test at 180 °/s showed lower RPT, H/Q ratio and LSI values for the Q muscle than those of the NOS (p = 0.042, p = 0.001). Conclusions: The H/Q ratio, in combination with the RIT, RPT and LSI, is a useful diagnostic tool for monitoring the effectiveness of 6-month PH after ACLR. Restoring the correct H/Q ratio can reduce the risk factor for ACL graft rupture.

Purpose: The shoulder girdle is a complex system, comprised by a kinematic chain and stabilizers. Due to the delicate equilibrium and synchronism between mobility and stability, high external loads may compromise its physiology, increasing the risk of injuries. Thus, this study intends to fully characterize the effects of a rugby tackle on the shoulder’s anatomy and physiology. Methods: For the experimental procedures, a matrix of pressure sensors was used, based on the Teckscan® pressure in-soles, force plates, an isokinetic dynamometer and sEMG (surface electromyography). Results: The anterosuperior region of the shoulder girdle confirmed the highest pressure values during impact (100 kPa to 200 kPa). Also, the right and left feet performed a vertical peak force of 1286 N (1.4 BW) and 1998 N (2.21 BW), respectively. The muscular activity of the shoulder muscles decreased after performing multiple tackles. Conclusions: During a tackle, the clavicle, scapula, trapezius and acromioclavicular joint are the anatomical structures with higher risk of injury. Also, the strike force on the feet decreases for stability purposes. After performing multiple impacts the muscular activity of the trapezius and rotator cuff muscles decreases, which may lead, in the long-term, to instability of the shoulder and inefficiency of the scapulohumeral rhythm.

Purpose: The aim of this study was to explore the strength capacities of the injured and non-injured leg following a unilateral anterior cruciate ligament reconstruction. Methods: Eight elite soccer players (age = 25.5 ± 3.9 years; height = 1.83 ± 0.04 m; body mass = 78.9 ± 4.5 kg) volunteered to participate in this study. Nine months after the anterior cruciate ligament reconstruction and just before initiating the full training process, the maximal peak torque of the quadriceps and hamstring muscles were measured at 60 and 180 degrees/second using isokinetic dynamometry. Obtained peak torques were used for calculating hamstring-to-quadriceps ratio, limb symmetry index, and to estimate maximal torque capacity using a two-velocity method (i.e., linear torque-velocity relationship modeled considering peak torque obtained at 60 and 180 degrees/second). Results: No differences were found between hamstring-to--quadriceps ratios of the injured and non-injured leg (p = 0.165), nor between limb symmetry index of the quadriceps and hamstring muscles (p = 0.985), regardless of the angular velocity applied during tests. The two-point method revealed significant differences between quadriceps and hamstring muscle groups both in the injured and non-injured leg (p < 0.001; maximal estimated torque was higher for quadriceps compared to hamstring muscles), while the differences between same muscle groups of the different legs were not significant. Conclusions: Collectively, these findings indicate that 9 months were enough for reaching a certain strength level of the injured leg that permits a safe return to play.

The main goal of the present study was to examine muscle strength and power of dominant and non-dominant leg, knee extensors and flexors, and their correlations with jumping performances in soccer players. A secondary aim was to evaluate muscle sense. Methods: 31 male professional strikers (age 20.9 ± 2.3 years, body mass 75.1 ± 6.6 kg, body height 179.5 ± 4.7 cm) participated in the study. The power output of lower extremities and the height of rise of the body mass centre during vertical jumps were measured using a force plate. The maximum muscle torque of the flexors and extensors of the knee were measured under isometric conditions using a special isometric torquemeter. Force sense was measured in isometric conditions in two tests: (a) fifty percent of the maximal voluntary contraction was set as a value of target force and the participants were instructed to reproduce the target force, (b) the participants attempted to develop a torque reproducing a sine course within the range of 10 to 50% of MVC performed. Results: A direct relationship was observed between the peak muscle torque in knee extensors developed during isokinetic contraction at all velocities and power and height of three types of vertical jumps ( p < 0.05). No correlation was observed between jumping performance and muscle torque under isometric condition. No differences were found in strength and jumping abilities as well as in force sense between dominant and non-dominant legs. Conclusions: This study offered a comprehensive and complete evaluation of leg muscle strength, sense and power, with the use of using force plate and isokinetic dynamometry.

Opis:
Scientific research [Bleakley’a, 2012, s. 348; Dewhurst i in., 2010, s. 451-458; Garcia i in., 2016, s. 347-358] show different effects of skeletal muscle cooling, others for therapeutic activities and others for strength training. Pilot studies were carried out on a small sample of AWFiS students in Gdańsk. The aim was to determine the probability of changes in the isokinetic strength of lower limb muscles when cooling with cold water. Tanita TBF-300 and Biodex SYSTEM 4 PRO measuring equipment were used in the research. It was found that after a twenty-minute cooling of the lower limbs in water at 11±0,5°C, there is no significant change in the muscular strength of the rectifiers and flexors of the knee during isokinetic work. It turned out that the muscle group at work with angular velocity 180°/s in the majority of measured parameters was on the edge of weaker results than before cooling. Correct verification of observations made requires a larger group of respondents. Research will be continued.
Biorąc pod uwagę doniesienia naukowe na temat konieczności rozdzielenia działań terapeutycznych od działań zwiększania siły mięśni szkieletowych pod wpływem zabiegów schładzania [Bleakley’a, 2012, s. 348; Dewhurst i in., 2010, s. 451-458; Garcia i in., 2016, s. 347-358] oraz kosztowną i skomplikowaną procedurę badawczą przeprowadzono pilotażowe badania na małej próbie dobranych celowo 6-studentów AWFiS w Gdańsku. Celem było określenie prawdopodobieństwa możliwości występowania zmian w poziomie przejawiania siły izokinetycznej mięśni kończyn dolnych pod wpływem chłodzenia zimną wodą. Do pomiarów podstawowych komponentów składu ciała posłużył BODY COMPOSITION ANALYZER TBF-300 firmy TANITA, a do pomiarów szczytowych momentów siły, czasu ich osiągnięcia, pracy całkowitej oraz średniej uzyskanej mocy BIODEX SYSTEM 4 PRO. Stwierdzono, że po dwudziestominutowym schłodzeniu kończyn dolnych w wodzie o temperaturze 11±0,5°C nie dochodzi do istotnych zmian wartości przejawiania siły mięśniowej prostowników i zginaczy stawu kolanowego podczas pracy o charakterze izokinetycznym. Dodatkowo, ze względu pilotażowy charakter badań i to, że grupa mięśni uda po schłodzeniu podczas ruchu z prędkością kątową 180°/s w większości mierzonych parametrów była na granicy odnotowania słabszych wyników niż przed ich schłodzeniem, daje nam podstawę do kontynuowania badań i zweryfikowania tego pomiaru na większej grupie badanych.
