INTRODUCTION: Sepsis stands as the primary cause behind intensive care unit (ICU) admissions. The most critical parameters in sepsis management have been shown to be early recognition. Management delays have been associated with increased mortality and morbidity The aim of this study is to study the lactate/albumin (L/A) ratio as prognostic tool for risk stratification in septic patients admitted to ICU. MATERIALS AND METHODS: This prospective observational study was conducted with100 patients. Admitted in ICU with sepsis and septic shock were studied. Serum lactate/albumin ratio was calculated at the time of admission. Apache 2 and SOFA score was calculated at admission. All patients received initial treatment according standard protocol. All patients were followed up till discharge. An adverse outcome in terms of in hospital mortality, length of ICU stays and inotropic support was used in this study. RESULTS: Lactate/albumin ratio >1.5(AUC 0.89) correctly predicted in-hospital mortality among 27% patients with sensitivity and specificity of 90% and 78.6% respectively (p value =0.001). Lactate/albumin ratio <1.50 (AUC 0.73) correctly predicted length of ICU stays <72 hours among 17% patients with sensitivity and specificity of 85% and 58.8% respectively (p value =0.001). Lactate/albumin ratio >1.50 (AUC 0.91) correctly predicted requiring inotropic support among 36% patients with sensitivity and specificity of 83.7% and 89.5% respectively (p value =0.001). CONCLUSIONS: We concluded that lactate/albumin ratio was a stronger parameter than lactate, albumin, APACHE score and SOFA alone in predicting mortality, length of ICU stay and requiring noradrenaline inotropic support among sepsis patients in the ICU.

Opis:
The aim of the research was to find the changes of exercises in the organism of the men specialized in middle distance running. Physical working capacity and simultaneous registration of the functions of vegetative systems of organism were investigated. As well biochemical tests (determination of the content of lactate of red cells, haemoglobin, glucose in blood) were done. 10 men (two Masters of Sport, two First - Class Sportsmen and six Second - Class Sportsmen ) aged 17 - 24 specialized in the middle distance ru nning took part in the experiment. The sportsmen's level of health was within the physiological standard. The examination of the men's adaptive body responses to the offered burdens made by us indicates that their functional body capacity increases in the first, second, third and fifth microcycles and decreases in the fourth one. It can be connected with the decrease of haemoglobin content in the blood, especially in the Masters' of Sport blood and in the blood of First - Class Sportsmen , it leads to the decr ease of their working capacity, respectively.

In sports training man tries to broaden the boundaries of his own capacities and the physical efficiency is an informative index, which defines a set of properties of the organism and, in the first place, the efficiency of blood flow and breathing mechanisms. In the article the impact of training load on the bodies of athletes, who specialize in middle distance running is analyzed and the dynamics of special efficiency, physiological mechanisms, responsible for the quality of athletes’ endurance, is determined. The dynamics of female athletes’ special efficiency manifestation has cyclical character and depends on the hormonal status change influence on the athletes’ body during the biological cycle. Special efficiency of male athletes has tendency of gradual results growth, which is explained by the functional possibilities change, resulted by the effective influence of training loads on their body. The highest efficiency has been found in the second and fourth phases, the lowest one - in the third and especially in the first and fifth phases of MC. This is due to the increased functionality of the cardiovascular system and the effectiveness of power-ensuring in the second and fourth phases and its decrease in the first and fifth phases of MC, leading to stress of adaptation processes of women’s bodies during these phases. As for men, their dynamics of special efficiency tended to gradual increase of the results, that, we think, can be explained by the growth of functional capabilities of the cardiovascular system and the effectiveness of power-ensuring on account of effective influence of training loads upon their organism.

The altered cerebral energy metabolism central to schizophrenia can be linked to lactate accumulation. Lactic acid is produced by gastrointestinal bacteria, among others, and readily crosses the blood–brain barrier, leading to the brain acidity. This study aimed to examine the association of the oral microbiota with the effects of acid stress induced by an increase of brain lactate in schizophrenia patients. The study included patients with a diagnosis of acute polyphasic psychotic disorder meeting criteria for schizophrenia at 3-month follow-up. Results: Individuals with a significantly higher total score on the Positive and Negative Syndrome Scale had statistically significantly lower lactate concentrations compared to those with a lower total score and higher brain lactate. We observed a positive correlation between Actinomyces and lactate levels in the anterior cingulate cap and a negative correlation between bacteria associated with lactate metabolism and some clinical assessment scales. Conclusions: Shifts in the oral microbiota in favour of lactate-utilising bacterial genera may represent a compensatory mechanism in response to increased lactate production in the brain. Assessment of neuronal function mediated by ALA-LAC-dependent NMDA regulatory mechanisms may, thus, support new therapies for schizophrenia, for which acidosis has become a differentiating feature of individuals with schizophrenia endophenotypes.

Background. The physical activity of 29 football players using either aerobic or anaerobic energy regimes was tested during official games and in the laboratory. Material and methods. Under conditions of gradually increasing power on a treadmill, maximal running speed (V, km/h), oxygen consumption (VO2, mL/min/kg), heart rate (HR, beats/min), and anaerobic metabolism threshold (AT mL/min/kg) were determined, and blood lactate (BLact, mmol/L) was measured. Results. The correlation between the total length of distance run during the game and treadmill running speed for the outcome measures were as follows: anaerobic metabolism threshold, r=0.54 (p=0.032), BLact, r=0.37 (p=0.034), VO2max, r=0.41 (p=0.027), and HR, r=0.36 (p=0.047). We found aerobic mechanisms were dominant in ensuring the game activity of football players. Specifically, aerobic mechanisms contribute roughly 32.4-43.8% to playing activity, while aerobic-anaerobic energy supply mechanisms provide for only 17%. Lactic and alactic mechanisms provide for 5.6% and 1.6%, respectively. Conclusions. The results provide valuable information on the relationship between football play activity and aerobic and anaerobic energy supply mechanisms. Enhancing the functionality of the anaerobic lactic-alactic energy system can lead to growth and improvements in physical performance in football players.

Phosphodiesterases (PDEs) are a family of enzymes that hydrolyze cyclic nucleotides, thereby modulating cell functions. Three highly selective PDE5 inhibitors (PDE5i), sildenafil, vardenafil and tadalafil, have been developed for treatment of erectile dysfunction (ED). Experimental evidence showed that chronic treatment with sildenafil PDE5i in a mouse model of diet-induced obesity and insulin resistance improved insulin action and decreased circulating fatty acid levels. It has recently been shown that healthy athletes use PDE5i as performance enhancers, hence in the present study we investigated whether the long-lasting PDE5i tadalafil influences energy metabolism in C2C12 skeletal muscle cells by evaluating lactate production, glucose consumption, and citrate synthase and 3-OH acyl CoA dehydrogenase activities. Our data demonstrate that tadalafil is able to modulate energy homeostasis in mouse skeletal muscle cells, depending on the treatment length and dose.

The aim of the study was to assess the effect of cold water immersion on changes in blood lactate concentration during post-exercise recovery in swimmers subjected to2-minute exercise test (front crawl swimming movements) using a VASA Swim Ergometer, with the maximum arm speed movements, as during the freestyle technique. The study covered 11 professional swimmers of the MKP Szczecin club, tested twice with a two-week interval. Each participant performed an exercise test twice, once with a passive recovery period, and the second time with cold water immersion after exercise, as a method potentially supporting the post-exercise recovery process. Each time before the test, immediately after and at 3, 6 and 9 minutes after exercise, the concentration of lactate in the capillary blood was measured. Statistical analysis of the obtained results showed that cold water immersion applied immediately after exercise resulted in a faster reduction of lactate concentration. The conducted research confirms that cold water immersion used in post-exercise recovery may be an effective method of restoring optimal physical fitness as part of the training process.
