The human nail unit is a highly specialised, keratinised structure belonging to the group of skin appendages. It performs many important functions necessary to maintain homeostasis, such as protection of the distal phalanx against damages or stabilisation of the fingertips. Human nail unit is formed by the nail plate and the proximal nail fold, lateral nail folds, nail matrix, nail bed and hyponychium. Human nail unit diseases are often diagnosed with delay, mainly due to anatomical conditions, but also due to understating the problem not only by the patients themselves, but also by doctors. Changes within the human nail unit are a frequent component of many dermatoses or separate disease entities. The main disturbances associated with pathologies within the nail matrix include the Beau lines (transverse pits indicating temporary disruption of the nail plate growth), onychomadesis (nail shedding), onychorrhexis, nail pitting, koilonychia (spoon nails) or trachonychia (sandpapered nails). The group of diseases associated with disorders of the nail bed includes onycholysis (loss of communication between the nail plate and the nail bed), subungual hyperkeratosis, apparent leukonychia (associated with the swelling of the nail bed) and subungual petechiae. The article also presents selected nail diseases associated with the deposition of dye (including melanonychia), some congenital disorders of the nail unit (such as, for example, malalignment of the great toenail or nail-patella syndrome) and selected manifestations of systemic diseases within the human nail unit. Getting familiar with the correct onychological nomenclature and presented issues will certainly facilitate establishing a dialogue between specialists in skin diseases and general practitioners.

Every day human bones undergo different injuries resulting from public transport accidents, diseases or trauma. Consequently, they cause temporary or constant patients disability or even death. Fractures happen not only to elderly people suffering from osteoporosis, but also more and more frequently to young people, who got some injuries as a result of life style, e.g. doing sports. The desire to enhance the physical well being and quality of patients' life, who got some injury causes constant development of medical devices for osteosynthesis. In the case of complicated fractures, in which plaster treatment is not sufficient, surgical treatment and applying implant are indispensable. The aim of the article is to present two case studies on intramedullary nails. The first one concerns a method of osteosynthesis for a patient, who was injured in an accident. The second one presents the failure of the implanted intramedullary nail. The research included evaluation of the type of fracture and the analysis of microstructure, chemical composition and hardness of the implant materials.

Introduction and aim. Glomus tumors represent benign neoplastic proliferations of the glomus body, an integral thermoregulatory component within the cutaneous microvasculature. Although they can manifest at various anatomical sites, they are predominantly observed in the subungual region. The tumors present as painful, firm, purplish, solitary nodules of the extremities, especially in the nail bed. They may be solitary or multiple; solitary lesions are encapsulated and most commonly subungual, while multiple tumors are unencapsulated and rarely subungual. Description of the case. Here, we present a case of multiple glomus tumors of the right hand in an 18-year-old female who presented with complaints of painful bluish discoloration of the right little finger for six years and progressive asymptomatic nodules on the right index finger for six months. A clinical examination revealed acute tenderness in both fingers. Love’s pin test and Hildreth’s test were positive. Excision of all lesions was done and sent for histopathology, which confirmed the diagnosis. The patient was symptom-free immediately following surgery. No nail deformities were noticed, and there was no recurrence of symptoms after one year of follow-up. Conclusion. The transungual approach is a safe and effective minimally invasive surgical technique for the treatment of symptomatic nail bed glomus tumors. It offers high success rates, promising cosmetic outcomes, and minimal complications. However, careful patient selection and meticulous surgical technique are essential to avoid potential nail deformities.

Bone fractures are one of the most commonly seen problems in veterinary traumatology. The authors of this study strive to find a new intramedullary nail, which is intended for treating femoral bone fractures for canine patients. The purpose of this study was to analyze biomechanical parameters of the intramedullary nails, which use a new bolt system concept. Methods: Dissected femoral bones of a large breed dog were cut in order to simulate interfragmentary gap, and then the bones were stabilized using intramedullary nail with locking bolts. Bone-nail systems were subjected to cyclic loading using force which corresponds to the load on the femoral bone in the first few days after surgery. Micro-CT scans were taken of the bone samples around implant in order to determine deformation and structural parameters of bone tissue. Results: The calculation of the bone-nail system stiffness was done through analysis of the forcedisplacement curves recorded during experimental studies. Using monocortical locking bolts resulted in smaller stiffness of the bone-nail system than using bicortical locking bolts. Conclusions: The results obtained in this study can indicate that the intramedullary nail could work well when used for treatment of bone fractures in dogs. The authors focused on using monocortical bolts which provides good stability and adequate biomechanical environment. Described fixation method is easily adjustable to a particular patient individual parameters.

Glomus tumor - a rare, benign tumor of the nail apparatus

Introduction: Glomus tumor is a painful, subcutaneous tumor. The lesion may occur in various locations. In the case of the nail apparatus, it affects both the matrix area and the nail bed. Case report: We present a 71-year-old patient who presented to our clinic with a painful nodule lasting for about 15 years, located on the fifth finger of his left hand. After surgical removal, a histological examination was performed, which confirmed the clinical suspicion of glomus tumor. Intraoperative contact onychoscopy revealed a gray-blue nodule with linear vessels at the periphery. Conclusions: In the differential diagnosis of glomeroma, the following should be considered: pyogenic granuloma, subugual viral wart, melanoma, hemangioma, leiomyoma, sweat gland tumor, gout, neuroblastoma and hemangioma. Dermoscopy can be useful in the differential diagnosis, but the final diagnosis is based on a histopathological examination. Surgical removal of the lesion is considered the treatment of choice.

Nail art (creation and decoration of artificial nails) is a growing fashion trend. Proportionally to its popularity, the number of allergic reactions to the materials used has recently increased. We report the case of a nail art operator who developed occupational allergy to acrylates, manifested with the unusual simultaneous presence of asthma and dermatitis. Cutaneous lesions only affected areas not covered by individual protection devices or clothes, even if such areas were not in direct contact with acrylates, suggesting airborne allergic contact dermatitis. While respiratory problems were solved with the correct use of a mask at the workplace, facial dermatitis proved impossible to avoid or control and, for this reason, the patient had to change her work.

A rare case of Lindsays nails in a patient treated with paclitaxel for breast cancer

Introduction: The nail apparatus is a skin appendage which, among other functions, stabilises the fingertips and protects the distal parts of fingers or toes from injuries. It may become dystrophic following chemotherapy. Diverse nail symptoms require a complex differential diagnosis. Objective: Presentation of a case of Lindsay’s nails in a patient treated with paclitaxel for breast cancer. Case report: A 65-year-old woman reported to a dermatology outpatient clinic with changes to her fingernails and toenails. Six months earlier, she had undergone left-sided mastectomy for breast cancer and then commenced chemotherapy with paclitaxel in combination with trastuzumab. A few weeks after starting this chemotherapy regimen, the patient noticed pronounced haemorrhages in the region of the toes, followed by matt whitening of the proximal part of the fingernail apparatus, sharply demarcated from the brown-coloured distal part. In addition, the patient experienced neuropathic symptoms in the fingers. The clinical picture of the lesions and the patient’s disease history suggested a variant of leukonychia, i.e. Lindsay’s nails, induced by chemotherapy. Conclusions: The presented case of Lindsay’s nails (“half-and-half” nails) is unique, as it was never before reported in the literature in association with paclitaxel therapy.

Qualitative indicators are an important element in establishing truth in polygraph examinations. Considering this, a study of the change in the quality of the signal from the photoplethysmograph in subjects who have used nail polish is extremely relevant and requires a comprehensive qualitative study. The paper uses general scientific and empirical methods of observation and experimentation, and is a comprehensive study of the effects of nail polish on the quality of the photoplethysmograph signal. It is the first time that this type of research has been conducted using a comparative analysis of subjects who did and did not use nail polish.

Medicinal nail lacquers are the most effective topical treatment of nail diseases. These formulations generally are organic solutions of the active substance as well as film-forming polymer and plasticizer, which affects the characteristics of the film formed after application and solvent evaporation. The aim of this work was to test effects of plasticizer present in nail lacquer formulations on permeation kinetics of fluconazole through the bovine hoof membrane in a novel in vitro test. The formulations contained Eudragit RS100 dissolved in acetone, and dibutyl-phthalate, PEG 400 or propylene glycol as plasticizers present in two different concentrations. Permeation studies were carried out during 7-day period, and the obtained permeability profiles analyzed using similarity and difference factors, and by model dependent permeation kinetics. When analyzed within the same strength, the highest extent of fluconazole permeation was obtained from formulation with lower concentration of propylene glycol at 0.9% fluconazole concentration, while for formulations with 1.8% and 2.7% of fluconazole, the highest permeation was achieved from formulation with the higher content of PEG400. The permeation profiles showed greater difference within one formulation of different fluconazole content, than with the same plasticizer present in different concentrations, when using dibutyl-phthalate and PEG400. Permeation profiles were similar when using propylene glycol. When comparing formulations with the same concentration of plasticizers, there were differences in formulations with the higher fluconazole concentrations. Permeation kinetics depended on fluconazole concentration as well as the path length the active substance had to pass to reach the receptor solution.
