Personalized orthodontics : from the sagittal position of lower incisors to the facial profile esthetics

Background: One of the goals of orthodontic treatment is to obtain maximum facial harmony. The sagittal position of the lower incisors plays a significant role in the planning of orthodontic treatment. The aim of the study was to evaluate the relationship between the sagittal position of lower incisors and facial profile esthetics with reference to the skeletal vertical dimension. Methods: There were 200 patients included in the study. Patients were allocated into three groups, depending on the vertical growth pattern: normal-angle, low-angle, and high-angle cases. Tweed–Merrifield cephalometric analysis was used to assess the sagittal and vertical position of the mandible, as well as to assess the sagittal position of the lower incisors. Results: Z-angle and Frankfort mandibular incisor plane angle (FMIA) decreased significantly (p < 0.001) with the increase of the skeletal vertical dimension. Incisor mandibular plane angle (IMPA) was significantly higher (p < 0.001) in low-angle patients compared to the high-angle ones. Z-angle appeared to be the most accurate parameter (area under curve, AUC = 0.957) describing patients with a convex profile. The cutoff value of Z-angle 68.0° was characterized by the sensitivity of 94.1% and the specificity of 84.3%. Conclusions: The sagittal position of the lower incisors significantly affects the facial profile convexity. The Z-angle is the parameter which most accurately describes the patients with a convex profile.

Opis:
Wires used for orthodontic arches play a very important role in the process of orthodontic treatment. In combination with the lock attached to the tooth, they move and align the teeth along the set trajectories. Wires of stainless steel are commonly used in orthodontics for several reasons: they are characterised by high resistance to corrosion, high strength and elasticity, formability and a possibility of obtaining defined properties through cold working and annealing during production process. The purpose of the research presented in the work is the analysis of differences of the selected structural properties in the context of corrosion resistance of the orthodontic wire material. The object of the research was edge arches of the 0.016”x0.022” size made of the stainless steel type AISI 304, provided by two different producers: G&H Orthodontics and Adenta. The research methodology involved analysis of chemical and phase composition of the tested alloy, microscopic tests with application of the light and electron microscopy methods, as well as electrochemical direct current measurements. The research presented in the work has shown significant differences in structural and physical-chemical properties of the orthodontic wires made of AISI 304 type stainless steel. Despite the fact, that the tested arches were manufactured of the theoretically the same materials, but by different producers, they significantly differ in chemical composition, metallurgical purity, phase composition and corrosion resistance. In addition, it is worth noticing that the tested materials, in terms of structure, do not meet the normative requirements obligatory for biomaterials.

Treatment of malocclusions using fixed orthodontic appliances makes it difficult for patients to perform hygiene procedures. Insufficient removal of bacterial biofilm can cause enamel demineralization, manifesting by visible white spot lesions or periodontal diseases, such as gingivitis periodontitis or gingival hyperplasia. The classic methods of preventing the above problems include, in addition to proper hygiene, ultrasonic scaling, periodontal debridement, and oral rinses based on chlorhexidine. New alternative methods of reducing plaque around brackets are being developed. There is a growing interest among researchers in the possibility of using photodynamic therapy in orthodontics. A literature search for articles corresponding to the topic of this review was performed using the PubMed and Scopus databases and the following keywords: ‘photodynamic therapy’, ‘orthodontics’, and ‘photosensitizer(s)’. Based on the literature review, two main directions of research can be distinguished: clinical research on the use of photodynamic therapy in the prevention of white spot lesions and periodontal diseases, and ex vivo research using a modified orthodontic adhesive by adding photosensitizers to them. Methylene blue is the most frequently used photosensitizer in clinical trials. The effectiveness of antimicrobial photodynamic therapy is mainly compared to the ultrasonic scaler as a single therapy or as an adjunct to the ultrasonic scaler. In their conclusions, the researchers most often emphasize the effectiveness of antimicrobial photodynamic therapy in reducing microbial levels in patients treated with fixed appliances and the possibility of using it as an alternative to routine procedures aimed at maintaining a healthy periodontium. The authors suggest further research on the use of photodynamic therapy to prove the validity of this method in orthodontics. It should also not be forgotten that proper hygiene is the basis for maintaining oral cavity health, and its neglect is a contraindication to orthodontic treatment.

Quantitative evaluation of the infrazygomatic crest thickness in Polish subjects : a cone-beam computed tomography study

Infrazygomatic crest (IZC) mini-implants are frequently used as an absolute anchorage when intrusive or distally directed forces are required during orthodontic treatment. The aim of the present study was to evaluate the thickness of the IZC area in Polish patients as well as to assess dependency between bone availability, sex, and age. The study material was 100 cone beam computed tomography scans (CBCT) of the maxilla of patients of the University Dental Clinic in Krakow (50 men and women each). IZC bone thickness was measured at nine different points. The biggest bone thickness was recorded in the interdental space between the first and second molar at the height of 12 mm (6.03 ± 2.64 mm). The thinnest bone depth was localized at the level of the mesial root of the first molar, 16 mm above the occlusal plane (2.42 ± 2.16). There was a significant and negative correlation between bone thickness and age in the case of measurements taken buccally to the first molar. Only two out of nine measurements showed a sex dependency (points I2 and I3). Considering vertical and sagittal dimensions, the most favorable conditions for IZC mini-implant placement were found interdentally, between the first and second molar, 12 mm above the occlusal plane.

Evaluation of palatal bone thickness at the implantation areas of two popular bone-anchored distalizers : a cone beam computed tomography retrospective study

Since class II malocclusion and lack of space within the dental arch due to early loss of deciduous molars is a common orthodontic problem in the Polish population, bone-anchored distalizers are becoming more and more popular. The aim of the present study was to evaluate palatal soft and hard tissue thickness using cone beam computed tomography (CBCT) at the area of micro-implant placement of two appliances for maxillary first molar distalization: Beneslider and TopJet distalizer. The study data were 100 consecutively selected CBCT images (53 of men and 47 of women). Measurements of bone and mucosa thickness were performed at six locations in the palate and tested according to their correlation with sex and age. The biggest bone and mucosa thickness were recorded in the insertion site of the TopJet miniscrew. Bone thickness in all points of paramedian insertion was significantly greater in males and the mean difference was approximately 1–1.8 mm. Age correlates significantly (p < 0.05) and positively (r > 0) with the thickness of the mucosa at all points: the older the patient, the thicker the mucosa at each measurement point. Anatomical diversity of the hard palate in the population involves the need to perform bone and mucosa thickness measurements before palatal micro-implant placement.

The aim of the study was to evaluate the shrinkage stress during cross-linking of composite orthodontic adhesive systems. The elastic-optical method was used for shrinkage stress analysis. Based on the obtained results, basic statistics were determined, including averages and standard deviations. For the comparative analysis of the mean values, the Tukey HSD test was used at the significance level α = 0.05. The average value of shrinkage stress during cross-linking ranged from 7.2 to 11.5 MPa.
Celem pracy była ocena naprężeń skurczowych podczas sieciowania kompozytowych ortodontycznych systemów adhezyjnych. Do analizy naprężeń skurczowych zastosowano metodę elastooptyczną. Na podstawie uzyskanych wyników wyznaczono statystyki podstawowe, w tym średnie oraz odchylenia standardowe. Do analizy porównawczej wartości średnich użyto testu Tukeya HSD na poziomie istotności α = 0,05. Średnia wartość naprężeń skurczowych podczas sieciowania ortodontycznych systemów adhezyjnych wynosiła od 7,2 do 11,5 MPa.

The following article presents a case study of a child treated with orthotropy and functionally stimulated during speech therapy. In the first part, the author outlines the assumptions of orthotropy in the context of orthodontics and identifies the aspects of speech therapy that should complement the interventions carried out by the orthodontist. The second part describes the study’s methodology and progression, details the patient’s diagnosis and treatment, and presents the author’s conclusions.
Poniższy artykuł to studium przypadku dziecka leczonego metodą ortotropii i jednocześnie stymulowanego funkcjonalnie w trakcie terapii logopedycznej. W części pierwszej autorka przedstawiła założenia ortotropii w odniesieniu do ortodoncji oraz zaznaczyła, jakie elementy terapii logopedycznej powinny wspierać działania prowadzone przez lekarza ortodontę. W części drugiej badaczka omówiła metodologię i przebieg badania, spisała przeprowadzanie diagnozy i terapii pacjenta oraz przedstawiła swoje wnioski.

Orthodontic retention is the final important stage of orthodontic treatment, the aim of which is to consolidate the functional and aesthetic position of teeth. Among adults, fixed retainers made of different types of wires are the most common. The aim of this study was to analyse the mechanical properties of a new generation of fixed orthodontic retainers—printed by 3D printers. Materials and Methods: The study was conducted using samples made of Nextdent MFH C&B N1 resin in the form of cuboid bars with nominal dimensions of width b = 3 mm, thickness d = 0.8 mm; 1 mm; 1.2 mm, length l = 30 mm for each type. The influence of the thickness of the retainers on their strength under loaded conditions was evaluated. Flexural strength, elastic properties, deflection, and creep were compared. The samples were aged in an artificial saliva bath at 37 ± 1 ◦C during the strength tests. Results: It was shown that differences in the thickness of the samples affected their elastic and strength properties. The highest average flexural modulus, the highest deflection, creep, and strength was characteristic of the samples with the highest thickness (1.2 mm). Samples with an average thickness of 1 mm had the lowest modulus of elasticity. Conclusions: The mechanical properties of 3D printed retainers show that they can be an alternative to metal retainers and the procedure of making new retainers, especially when patients have aesthetic requirements or allergies to metals.

The strength of the connection between orthodontic brackets and sandblasted dental ceramics was evaluated. The strength of the bracket-ceramics connection was determined in four time intervals. Based on the obtained results, basic statistics were calculated. For the comparative analysis of average values, the following were used: ANOVA analysis of variance and Tukey’s HSD post-hoc test at the significance level of α = 0.05. The statistical analysis performed did not show any influence of the method of surface treatment (with sanding, without sanding) between orthodontic brackets and ceramics. At the same time, it has been shown that with samples storing time in aqueous environment, the quality of the connection decreases significantly, regardless of the surface treatment method used.
Zbadano wytrzymałość połączenia zamków ortodontycznych z ceramiką dentystyczną poddaną piaskowaniu. Wytrzymałość połączenia zamek-ceramika wyznaczono w czterech przedziałach czasowych. Do analizy porównawczej wartości średnich zastosowano m.in. analizę wariancji ANOVA oraz test post - hoc Tukeya HSD na poziomie istotności α = 0,05. Przeprowadzona analiza statystyczna nie wykazała wpływu metody obróbki powierzchni porcelany dentystycznej (z piaskowaniem, bez piaskowania) na jakość połączenia zamków ortodontycznych z ceramiką. Jednocześnie wykazano, że wraz z upływem czasu przechowywania próbek w środowisku wodnym, istotnie zmniejsza się jakość połączenia, niezależnie od stosowanej metody obróbki powierzchni.
