Opis:
Currently in Poland there is neither regulation aiming at framing the complexity of substitution process arising at pharmacy level, nor further guidelines depending on drug category. FDA describes a narrow therapeutic index drug (NTI) in the 21 Code of Federal Regulations 320.33(c) and highlights that safe and effective use of such drug products require careful titration and patient monitoring. In major therapeutic areas disadvantages of the generic substitution are irrelevant or minor in comparison to the economic benefit. In epilepsy, a number of experts and professional bodies have recommended caution with generic substitution of drugs used in this condition. Aim of the study is to legally and economically identify and assess drug sub-stitution occurrence in Polish background among selected narrow therapeutic index drugs used in epilepsy. Paper is divided into two sections: first is retrospective based and tackles the data received from the National Health Fund; whereas second section takes under consideration questionnaire based results gained during medical history being collect-ed with the patients. Basing on most commonly substituted substance in epilepsy: carbamazepine, the maximum single difference in cost in substitution at the pharmacy was identified around the level of 1.7 PLN, and maximum extra pay at the same level. Ob-served/reported changes, ADRs, discomfort were provided based on the question-naire. The study highlights the necessity of planning process and evaluation of cur-rent market regulations.

The main aim of this master's thesis is to review the legal problematic aspects of pharmacies functioning as entities which are entrusted by the legislator with realization of special tasks consisting of protection of public health. Profile of this business requires explanation of basic institutions of pharmacy law concerning the retail of medicinal products taking place within these entities. Activity of providing pharmaceutical services by qualified professional staff in the company like pharmacy (and limited-service pharmacy), which operates in economic freedom, is a reflection of country's goal to provide a wide access to broad-sense health care to the general public.Practical aspect of analyzed issue, explaining the complexity of law regulations concerning running the pharmacy in compliance with pharmaceutical law and executive acts, created the necessity of accurate depiction of particular law institutions which exhaust the subject of functioning of pharmacies in Poland.
Celem tej pracy jest omówienie problematyki prawnej w zakresie funkcjonowania aptek jako placówek, którym ustawodawca powierza realizację szczególnych zadań składających się na ochronę zdrowia publicznego. Charakterystyka działalności gospodarczej polegającej na prowadzeniu apteki implikuje również konieczność wyjaśnienia podstawowych instytucji prawa farmaceutycznego dotyczących obrotu detalicznego produktami leczniczymi, jaki dokonuje się w tego rodzaju placówkach.Działalność polegająca na świadczeniu usług farmaceutycznych przez posiadający odpowiednie kwalifikacje personel apteki powoduje, iż funkcjonujące w ramach swobody działalności gospodarczej przedsiębiorstwo, jakim jest apteka (także punkt apteczny) stanowi desygnat realizowanego przez państwo celu polegającego na zapewnieniu ogółowi społeczeństwa dostępu do szeroko rozumianych świadczeń zdrowotnych.Aspekt praktyczny analizowanego zagadnienia, a więc wyjaśnienie złożoności regulacji prawnej dotyczącej prowadzenia apteki na gruncie ustawy – Prawo farmaceutyczne oraz aktów wykonawczych, zrodził potrzebę dokładnego omówienia i opisania poszczególnych instytucji prawnych wyczerpujących tematykę funkcjonowania aptek w Polsce.

Pharmacies are acknowledged to have a key role in the provision of health services, including emergency contraceptive services and under the right circumstances could meet the growing expectations for reproductive health education and consultations. Pharmaceutical emergency contraceptive service provided by pharmacists include ensuring the availability of required medicines, correct dispensing, providing comprehensive information, all of which requires not only consultations that are health-oriented and educational, but also providing the patient with an adequately private situation to talk with the pharmacist. Objectives: The aim of the paper is to evaluate pharmaceutical emergency contraceptive services provided in polish pharmacies. Material and methods: The observational study included 60 community pharmacies of different profiles located in Gdansk (Poland). Our method was to use “standardized (simulated) patients”. Results: The study revealed the low availability of emergency contraception in Gdańsk pharmacies. Pharmacy employees provided drug-related information in 51.6% of study’s visits. None of the pharmacies provided patients with educational information on reproductive health. Regarding the respecting of patients’ rights privacy and intimacy, overall, the pharmacies scored 4.41 on a five-point scale. Conclusions: Our study sought to identify both the developed and underdeveloped aspects of a comprehensive emergency contraceptive service. Pharmacy employees in the study provided patients with a narrow range of information on hormonal medications and they failed to include sexual health education in their work. The patient’s right to expect that intimacy and privacy will be respected during their consultation was met to a satisfactory degree.

Education of community pharmacy workers is a very important part of everyday practice, especially in professional counselling (PC) which brings many health benefits for the patients. The aim of the study was to investigate if the community pharmacy specialization has an influence on the scope and quality of counsels given by pharmacy workers during PC scheme implementation in community pharmacies. The research was carried out between May and November 2017. After PC scheme implementation in community pharmacies, an anonymous questionnaire was administered to 245 pharmacy workers in Poznan and Warsaw (241 women, 4 men): 146 pharmacists (143 women, 3 men) and 99 pharmacy technicians (98 women, 1 man). In the conducted study, 14.7% of pharmacists had community pharmacy specialization (according to Polish law). Pharmacists with a community pharmacy specialization referred patients more often to physicians (p<0.0001), as well as after earlier physician consultation (p<0.0001). Furthermore, they often noticed the interaction between drugs in patients’ self-treatment and in the treatment ordered by a physician (p≤0.001). The indication of medication contraindications after a physician consultation was also significantly higher within pharmacists with specialization (p<0.0001). It can be noted that pharmacists with community pharmacy specialization were asked more often for self-treatment consultation and after physician visits (p<0.0001). Development of PC in Polish community pharmacy, especially among pharmacy owners well-educated in community pharmacy specialization, may help to increase safety of pharmacotherapy.

The opinion analyzes the bill of amendment to the Act on Reimbursement of Medicines, Foodstuffs for Special Nutritional Purposes and Medical Devices. The article positively assesses the introduction of definitions for some of the terms found in the Act. It also approves the regulations on coordination teams, which are intended to remove doubts about their legal authority. The author also highlights the introduction of exceptions to the principle of permanence of final decisions, which, although legally permissible, can be assessed negatively from the perspective of the predictability of the reimbursement system. The opinion also addresses the problem of interpretative doubts as to the legal nature of the county board’s resolution to designate a community pharmacy to perform night or holiday duty.

Professions of public trust under the act are created by pharmacy self-government (Pharmacy Chambers). Within these structures, professional liability bodies are appointed, i.e. spokespersons for professional responsibility and courts, whose task is to thoroughly recognize any disciplinary cases committed by members of the profession, and which are in conflict with the provisions regulating the performance of a given profession and failures that offend the principles of professional ethics and deontology. It should be emphasized that proceedings regarding professional responsibility for the same act are carried out regardless of criminal, civil or disciplinary liability initiated in the employer’s organizational unit.
Zawody zaufania publicznego z mocy ustawy tworzą samorządy korporacyjne (izby aptekarskie). W ramach tych struktur, powoływane są organy odpowiedzialności zawodowej tj. rzecznicy odpowiedzialności zawodowej oraz sądy, których zadaniem jest wnikliwe rozpoznawanie wszelkich przypadków dyscyplinarnych popełnianych przez członków zawodu, a stojących w sprzeczności z przepisami normującymi wykonywanie danego zawodu oraz uchybieniami obrażającymi zasady etyki i deontologii zawodowej. Należy podkreślić, że postępowania w przedmiocie odpowiedzialności zawodowej za ten sam czyn, toczą się niezależnie od odpowiedzialności karnej, cywilnej czy dyscyplinarnej wszczynanej w komórce organizacyjnej pracodawcy..

Opis:
The objective of the study was to assess the Evidence-Based Practice Profile of PhD students at the Faculty of Pharmacy at Medical University of Warsaw. The study participants were 36 PhD students (72% women), including graduates with the title of Master of Sciences of Pharmacy (N=30) or Medical Laboratory (N=6). The data to determine the EBP profile of the PhD students from the Evidence-Based Practice Profile Questionnaire (EBP2Q) were collected using the CAPI method. Activity reports and the score for problems and analytical tasks obtained by students were generated from the MoodleTM e-learning platform. The evaluation concerned the relationship between specific domains comprising the EBP profile and the students' activity during the online course, as well as the results of the final tests (correlation coefficient (rp)). The total result and the score from specific domains, as obtained from EBP2Q, were average with the domination of lower results. The average time spent on the online course was about 7h, while the most time was spent on studying "Secondary data analyses: meta-analysis and systematic review" by the PhD students (average 1.5 h). The most difficult tests were the meta-analysis interpretation tests and the tests for guidelines for clinical practice. A significant correlation was observed between the number of hits and the score for tasks (rp = 0.661, p < 0.001). As the expected educational results depended on the involvement of PhD students in the online course, activating educational forms should be introduced to the course, such as workshops or project tasks.
