Opis:
The main purpose of the article is to identify key criteria being used for physician appraisals and to find how communication skills of physicians are valued in those appraisals. ScienceDirect and EBSCOhost databases were used for this search. The results show that a physician appraisal is underestimated both theoretically and empirically. The particular gap exists with respect to the communication skills of physicians, which are rarely present in medical training syllabi and physician assessments. The article contributes to the theoretical discourse on physician appraisals and points out at the inconsistency between the high status of physicians as a key hospital resource on the one hand and, on the other hand, at inadequate and poorly researched assessment of their performance with a special emphasis on communication skills. The article may inspire health managers to develop and implement up-to-date assessment forms for physicians and good managerial practices in this respect in hospitals and other health care units.

Opis:
This paper is an attempt to examine the senses and meanings attributed by physicians to the medical profession. The collected material is part of a larger project devoted to the physician-patient relationship reconstructed on the basis of individual narrations provided by physicians. The conducted research is closely linked to the qualitative perspective: Interpretative Paradigm and Interpretive Paradigm. The study was performed in the period 2015-2017. The examined group consisted of 16 subjects (6 female and 10 male physicians) from several provinces of Poland who were highly esteemed (subjective opinions) by their patients. The empirical data were collected by means of narrative interviews and the methodology applied for the analysis of the content was that of phenomenography.

The author agrees with the thesis of the verdict according to which, “the legal protection that is due to a public official granted to a physician in Art. 44 of the Act of 5 December 1996 on the Professions of a Physician and a Dentist Physician and in Art. 5 of the Act of 8 September 2006 on National Medical Emergency Service does not cover situations other than providing short-term assistance (i.e. providing first aid and performing emergency medical procedures) or medical assistance where any delay would result in exposing to danger of loss of life, grievous bodily harm or health disorder, and in other urgent cases”.
Autor zgadza się z tezą orzeczenia, zgodnie z którą „ochrona prawna należna funkcjonariuszowi publicznemu, przyznana lekarzowi w art. 44 ustawy z 5 grudnia 1996 r. o zawodach lekarza i lekarza dentysty oraz w art. 5 ustawy z dnia 8 września 2006 r. o Państwowym Ratownictwie Medycznym, nie obejmuje sytuacji innych niż udzielanie pomocy doraźnej (a więc udzielanie pierwszej pomocy i podejmowanie medycznych czynności ratunkowych) lub pomocy lekarskiej wtedy, gdy zwłoka w jej udzieleniu mogłaby spowodować niebezpieczeństwo utraty życia, ciężkiego uszkodzenia ciała lub ciężkiego rozstroju zdrowia oraz w innych wypadkach niecierpiących zwłoki”.

In the recent decade quite a few countries and regions legalised physician assisted death. While palliative care is already or becoming the standard end of life care in many countries, the increased availability of physician assisted death coupled with the secularisation of hospice in more settings require – where this has not happened yet – a clear response of palliative care specialists to patients’ requests for physician assisted death. The paper analyses the World Health Organisation’s current description of palliative care with a special focus on its prohibition of hastening death. Some palliative care professionals do not agree with the ban on hastening death, and these professionals’ non-conventional interpretation of palliative care actually seems to meet the wishes of some patients.

Objectives: The aim of this study was to estimate the level of exposure and average intensity of aggression towards particular professional groups of healthcare workers. Materials and Methods: Study participants (n = 1498) were employed at open and closed healthcare units within Podlaskie Voivodeship: 493 nurses, 504 midwives and 501 physicians. The MDM Questionnaire was applied. Results: Aggression originating from patients was experienced by 92% of nurses, 86% of doctors, and 74% of midwives examined. Aggressive behavior of co-working physicians concerned 55% of midwives, 54% of nurses and 40% of physicians. The highest average levels of patient aggression, ranging between 2.20 and 3.31, were reported by nurses. Conclusions: Nurses are the group most exposed to most aggression forms and sources. Physicians are least exposed to aggression, except for aggression manifested by patients.

Purpose:To analyze the relationship between the psychological condition of representatives from various professional groups of healthcare workers, the degree of their exposure to patient aggression, as well as with the duration of their professional experience.Materials and methods:Study participants (n=1498) were employed at open and closed healthcare units within Podlaskie province: 493 nurses, 504 midwives and 501 physicians. The Work Features Evaluation Questionnaire and General Health Evaluation Questionnaire GHQ28 were applied, and the psychological condition of medical staff was examined based on a 30-question surveyResults:When analyzing all of the groups of medical personnel, the mean level of exposure to patient aggression fell within the moderate range. The highest level of aggression was experienced amongst nurses, while the lowest – amongst midwives. The analyzed groups did not differ significantly in terms of the level of their psychological condition. With the exception of physicians, no significant association was observed between levels of patient aggression and the psychological condition of the medical personnel. Amongst physicians, individuals who experienced lower levels of aggression were characterized by significantly higher psychological condition values when compared to personnel exposed to moderate or high levels of patient aggression. No significant linear correlations between psychological condition levels and the frequency of patient aggression or duration of professional experience were noted in any of the analyzed professional groups.Conclusions:A moderate level of exposure to patient aggression is not the main factor affecting the psychological condition of medical personnel.

Exploring satisfaction and migration intentions of physicians in three University Hospitals in Poland

Introduction: University hospitals constitute a unique group of health care organizations which traditionally link three functions: (1) providing highly specialized services, (2) teaching activities, and (3) conducting research. Objectives: To assess the level of carrier satisfaction among physicians working in three university hospitals in Poland (1); to assess whether the physicians have the intention to migrate and what the main reasons for migration are (2); and to identify the actions that might be taken at the hospital level to mitigate physicians’ intentions to migrate (3). Methods: Cross-sectional study with both quantitative and qualitative components. In the quantitative part, an online questionnaire was distributed among physicians working in three university hospitals. A total number of 396 questionnaires were analyzed. In the qualitative part, in-depth interviews with six hospital managers were conducted and analyzed using thematic analysis. Results: On a scale from one “very dissatisfied” to six “very satisfied”, the mean career satisfaction of physicians was 4.0 (SD = 0.74). The item with the lowest mean concerned salary level (2.8, SD = 1.41). In the sample, 34% of physicians declared intentions to migrate from Poland. The main reasons for the intention to migrate were: Better working conditions abroad, higher earnings, the ability to maintain better work-life balance, better training opportunities abroad, and problems due to a stressful current workplace. Hospital managers considered the actions that can be taken at the hospital level to mitigate physicians’ migration to be specific to those focused on the working environment. Conclusions: Career development opportunities and features related to the working environment are the main factors influencing physicians’ satisfaction and migration intentions that can be modified at the university hospital level.
