Opis:
Objectives: Hepatitis B (HBV) and C viruses (HCV) are among the most frequent blood borne pathogens. According to WHO, 5% of healthcare workers (in central Europe), are exposed to at least one sharps injury contaminated with HBV per year, 1,7% - contaminated with HCV. Aims: The aims of the study were to determine prevalence of HCV and HBV infections, vaccination efficacy against hepatitis B and usefulness of alanine aminotransferase (ALT) testing in prophylactic examinations in healthcare workers (HCWs). Material and Methods: In a group of 520 healthcare workers, a survey, laboratory and serologic tests such as ALT, HBsAg, anti-HBs, anti-HBcT and anti-HCV were carried out. Results: The study revealed a low rate of workers with presence of HBsAg and anti-HCV (1,2% and 0,8% respectively). Anti-HBcT was found in 99 subjects (19%) without a significant association with experiencing an occupational percutaneous injury. Being vaccinated against HBV was declared by 90% of the subjects. There was no relationship between ALT level rise and positive HBsAg, anti-HCV and anti-HBcT tests. Conclusion: A seroprevalence of HBV and HCV markers in HCWs found in the study is low and similar to the one found in general population. Current or past hepatitis B infections were independent of needle stick injuries. Vaccination against HBV coverage, although found to be high, should improve to 100%. Occupational prophylactic medical examinations found performing ALT test (obligatory in Poland for HCWs) not helpful. It seems that determination of anti-HBcT and anti-HCV status would be essential in pre-employment medical examinations.

Opis:
A study was carried out to determine the histopathology of the liver in cattle slaughtered in Port Harcourt Abattoir, River State, Nigeria, between October to December 2016, and to assess the prevalence of fascioliasis. In undertaking this, faeces were collected and processed using the formal ether concentration technique, while tissue samples for both infected and non-infected livers were collected for proper histopathological procedures. Herein, the livers of slaughtered cattle were examined through visualization, palpation and incision. The outcome of our work showed that out of the 712 randomly sampled cattle, 38 (5.34%) were infected. Of these, 571 were males with an infection rate of 32 (5.60%), while 141 were females with infection rate of 6 (4.25%). Based on breed, infection rates were 12 (4.35%), and 26 (6.02%) for breeds of Sokoto Gudali and Red Bororo, respectively. No infection was recorded in the White Fulani breed. There was no statistically significant association between infection and breed and between infection and sex of the animals sampled. In our histopathology studies of non-infected and infected livers, microscopically, the non-infected liver tissue cells, sinusoids, bile duct and portal tract were all normal. Our results also showed that there were no abnormal changes in the nucleus and the colour of the tissue. However, infected liver tissue appeared pale in colour and greatly swollen, indicating fibrosis. In addition, there was dilation in the central vein and debris and large patches scattered over the parietal surface. Moreover, the pipe stem appearance of the liver was evident. Such histopathological changes indicate tissue damage and this results in economic losses in rearing and in butchering in animals, and in harm to human health. Proper abattoir inspection and care by veterinary workers is therefore required to ensure that heavily damaged livers are not sold for public consumption.

Opis:
Orthorexia nervosa (ON) is a disorder characterized by obsessive behavior towards healthy eating. Some groups present a higher prevalence of ON and athletes seems to be a population at risk. The aim of this study was to determine the prevalence of ON in the rock and sport climbing community. population using two different questionnaires. Düsseldorf Orthorexia Scale (PL-DOS) was used to assess ON. The study involved 236 climbers (including 159 men and 77 women, 67.4 and 32.6%, respectively), with an average age of 34.7±9.2 years. The group was divided into groups according to criteria such as the preferred type and style of climbing, participation in climbing competitions or the volume of training undertaken. Orthorexia was diagnosed in 12.7% (n=30) of climbers, with a mean score of PL-DOS of 32.8±2.4 with a 95% confidence interval ranging from 31.9 to 33.7. Another 25% of the respondents were diagnosed with ON risk (mean±SD 26.8±1.5, 95% CI 36.4-27.2). It was not found that the adopted stratification variables significantly differentiated the groups distinguished on their basis. The only exceptions were age and weekly training frequency. However, the effect sizes were small in both cases.

Opis:
Background: Hypertension very often accompanies progression of chronic kidney disease (CKD) in children. A cross-sectional analysis of hypertension prevalence in dialyzed children in Poland was designed with a comparison with the data previously recorded 10 years earlier. Methods: Two cohorts of children were analyzed: 59 subjects dialyzed in 2013, and 134 children from the previous study performed in 2003 that were reevaluated according to the current methodology. The incidence of hypertension (defined by SDS of sBP or dBP >1.64), clinical data, medical history, dialysis modalities and selected biochemical parameters of dialysis adequacy were analyzed. Results: The prevalence of hypertension increased from 64% in 2003 to 78% in 2013. The efficacy of antihypertensive treatment remained unsatisfactory (61% proper BP control). Preservation of residual urine output and strict fluid balance may prevent development of hypertension in children on dialysis. Conclusions: Despite the higher awareness of hypertension and its complications in dialyzed children, the incidence of this entity has increased during the last decade, with the percentage of undertreated patients comparable to that observed 10 years ago. Thus, more attention should be paid to therapy efficacy in this population to prevent further damage to the cardiovascular system and to decrease morbidity.

Opis:
Objectives Web information systems may serve as a diagnostic tool for the Internet users and they also support the epidemiological work of doctors and health care providers. As part of this study, a system has been created for detecting and calculating cardiovascular risk. The aim of this study has been the comparison of cardiovascular risk factors and calculated fatal cardiovascular risk in 2 periods of time: 2004–2009 and 2010–2015 in Poland, as determined via the Internet. Material and Methods The “Ryzyko program” (“Risk program”) is available on the website of the Medical University of Gdańsk. To assess the cardiovascular death risk in a 10-year period, the algorithm of the SCORE (Systematic Coronary Risk Evaluation) project was used and 30 402 results of the algorithm have been analyzed. Results Over 30 402 webpage visitors entered the required data and received the outcome. More than 78% of the Internet users who had entered the data, received a recommendation for medical check-up. Significant differences between the data collected in 2004– 2009 and 2010–2015 were noticed. Hypercholesterolemia prevalence (67.3% vs. 70.8%; p < 0.001), mean total cholesterol concentration in blood (5.60±1.65 mml/l vs. 5.66±1.35 mml/l; p < 0.001), prevalence of hypertension (36.6% vs. 35.3%; p = 0.039), mean systolic blood pressure (131.5±20.3 mm Hg vs. 132.6±18.0 mm Hg; p < 0.001), prevalence of declared smoking (30.7% vs. 26.5%; p < 0.001), declared diabetes mellitus (DM) (6.4% vs. 9.7%; p < 0.001), and declared coronary artery disease (CAD) (7.2% vs. 14.1%; p < 0.001), respectively. Conclusions The prevalence of cardiovascular risk factors has changed during the observed period of time. Online automatic gathering of new data by “Ryzyko program” provides up-to-date observations. Opis:
Chronic bronchitis (CB) symptoms are commonly reported in individuals without chronic obstructive pulmonary disease (COPD), but CB is rarely diagnosed in this population. We aimed to determine the prevalence and burden of CB, as well as its risk factors, in a population of patients without COPD. Data from the "Health Action" program (a lung cancer prevention and health care improvement program conducted in Proszowice County, Poland) were used. All county inhabitants aged 40 years or older without COPD were invited to participate. As part of the program, a questionnaire was administered to assess CB symptoms and risk factors. Spirometry at baseline and after the bronchodilator test was also performed. CB symptoms were present in 9.1% of the 3558 participants. The prevalence of CB in the study population was 7.12% (95% CI, 6.70-7.56). Patients with CB had more dyspnea and more often received medical treatment for lung disease or were hospitalized for respiratory disorders than patients without CB. CB was associated with worse lung function and a worse score in the modified Medical Research Council Dyspnea Scale even after adjustment for possible confounders. In a multivariate analysis, male sex, age over 70 years, current smoking, passive exposure to tobacco smoke, gas or wood heating, occupational exposure to chemical agents, lower forced expiratory volume in 1 second, and asthma correlated with an increased risk of CB. CB symptoms are common in individuals without COPD aged 40 years or older and are associated with more dyspnea irrespective of lung function and comorbidities.

Opis:
Objectives: The analysis presented in the article is the first study of suicide on railway tracks in Poland. Methods: An epidemiological assessment was conducted, study type: case series, population-based with use of the data from the National Police Headquarters. The rates of railway suicides in Poland in the years 2007-2018 were compared to the overall number of suicides in Poland and in the EU. Results: In 2007-2018, the average share of deaths by railway suicide in the overall number of suicides committed in Poland amounted to 2.06%. Between the years: 1999 -2018 there is a growing number of railway suicide attempts and railway suicide attempts without deaths. Since 2012, an increase of suicide attempts on railway tracks, along with a constant number of suicide deaths, has been observed. In 2017 - 2018, throwing oneself under a train to end one’s life was the third most commonly chosen method in Poland. Although it accounted for only a small percentage of all methods of suicide, it was chosen relatively more often than in other European countries. Conclusions: Poland belongs to a small group of European countries where the number of deaths by suicide has displayed an upward tendency in the course of 20 years. There is a growing number of unsuccessful railway suicide attempts. The index of deaths on railway tracks in Poland, was considerably higher than the average index for the EU.
