Opis:
In the last few years, notable technical progress has taken place in ultrasound elastography. Qualitative methods have been replaced by quantitative ones, such as: transient elastography, acoustic radiation force impulse and shear wave elastography. Owing to the fact that the spleen is superficially located, it is possible to obtain reliable measuring accuracy of its hardness using sonoelastography. Lately, many researchers have been investigating how spleen elasticity changes in patients infected with hepatitis B virus or hepatitis C virus and in patients suffering from liver fibrosis, portal hypertension, esophageal varices or myelofibrosis. In this article, we review the role and current status of accessible qualitative ultrasound elastography methods, including recent advances in the evaluation of spleen stiffness and its clinical utility. As study results demonstrate, spleen stiffness correlates with liver fibrosis and is helpful in determining the level of fibrosis in the METAVIR scoring system. In patients infected with hepatitis B virus or hepatitis C virus, spleen stiffness increases even when liver elasticity remains unaltered. Furthermore, it is useful in diagnosing portal hypertension or predicting existence of esophageal varices. Moreover, in patients suffering from biliary atresia after Kasai portoenterostomy, spleen sonoelastography may be helpful in selecting patients for liver transplantation as well as for choosing the best strategy for portal vein reconstruction before liver transplantation. In myelofibrosis, spleen stiffness correlates with bone marrow fibrosis and may be used to assess the response to treatment. Spleen sonoelastography is also useful in the monitoring of transjugular intrahepatic portosystemic shunt function.
Od kilku lat zauważalny jest postęp w sonoelastografii. Metody ilościowe zostały zastąpione przez jakościowe, takie jak: elastografia dynamiczna impulsowa, elastografia impulsu mocy promieniowania akustycznego i elastografia fali poprzecznej. Powierzchowne położenie śledziony umożliwia uzyskanie za pomocą sonoelastografii wiarygodnych, dokładnych pomiarów twardości tego narządu. W ostatnim czasie przeprowadzono wiele badań dotyczących elastyczności śledziony u pacjentów z wirusowym zapaleniem wątroby typu B lub C, cierpiących na zwłóknienie wątroby, nadciśnienie wrotne, żylaki przełyku lub mielofibrozę. W niniejszym artykule dokonano przeglądu roli i aktualnego statusu dostępnych jakościowych metod sonoelastografii, uwzględniając ostatnie postępy w ocenie twardości śledziony i jej klinicznego zastosowania. Jak pokazują wyniki badań, twardość śledziony koreluje ze zwłóknieniem wątroby i jest pomocna w określaniu stopnia zwłóknienia wątroby w skali METAVIR. U pacjentów zakażonych wirusem wątroby typy B lub C twardość śledziony wzrasta, nawet jeśli twardość wątroby pozostaje niezmieniona. Co więcej, jest użyteczna w diagnostyce nadciśnienia wrotnego i przewidywaniu obecności żylaków przełyku. Ponadto, u pacjentów cierpiących na zarośnięcie dróg żółciowych leczonych portoenterostomią sposobem Kasai elastografia śledziony może być użyteczna podczas selekcji pacjentów do przeszczepu wątroby oraz do wyboru optymalnej strategii rekonstrukcji żyły wrotnej przed transplantacją wątroby. W mielofibrozie twardość śledziony koreluje z włóknieniem szpiku kostnego i może być użyteczna w ocenie odpowiedzi na leczenie. Sonoelastografia śledziony jest również użyteczna w monitorowaniu funkcjonowania przezszyjnego wewnątrzwątrobowego zespolenia układowo-wrotnego. Splenic cysts are rare disease that are diagnosed incidentally during imaging studies. In recent years, through the development of diagnostic methods the detection of their are increased, although documented and described in the literature of cases is still low. The disease can be asymptomatic - this concerns mainly small cysts, but greater changes cause unspecific symptoms resulting from oppression of enlarged spleen on adjacent organs. Due to the etiology of cysts, they are divided into primordial and false. Primordial cysts have an epithelial lining which distinguishes them from false and they are divided into parasitic and nonparasitic. Because of the possibility of complications cysts usually treated surgically, with the aim to preserve the splenic parenchyma. We present a case of a 28-year-old woman who has revealed the presence of epidermal cysts of the spleen

Introduction: We present the case of a primary spleen mucinous cystadenocarcinoma. Several cases of this primary tumor of the spleen have been described worldwide so far. These tumors are classified as mucinous cystic neoplasms (MCN) and occur mainly in the ovaries and pancreas. Case report: The case concerns a 45-year-old female patient with an accidentally diagnosed splenic tumor with approximately 20 cm in size. Histopathological examinations, following a splenectomy, confirmed the presence of mucinous cystadenocarcinoma.

Asymptomatic appearance of splenic infarction in Wegeners granulomatosis

Splenic involvements in Wegener's granulomatosis (WG) are rarely diagnosed ante-mortem, while an autopsy is able to reveal a high rate of spleen lesions (78-100%). To date, there have been a few reported cases of splenic abnormalities in WG, including: splenomegaly, capsular adhesion, dysfunction and infarction. We reported a case of biopsy-verified WG with radiological evidence of diffuse spleen infarction despite the lack of any clinical symptoms. We concluded that due to a potential risk of severe hemorrhagic complications when anticoagulant therapy is necessary, radiological assessment of spleen should be performed regularly in this group of patients, particularly because spleen involvement can be asymptomatic.

Endovascular treatment of splenic artery aneurysms

Background: Aneurysms of visceral arteries, including splenic artery, are relatively rare vascular pathology. Currently they are diagnosed more and more commonly due to widespread application of imaging procedures in diagnostics of other abdominal conditions. Splenic artery aneurysm develops often asymptomatically, and its rupture is associated with a high mortality rate. Hence, elective treatment of such aneurysms is suggested as a prophylaxis of the rupture. Material/Methods: This report contains presentation of treatment outcome of nine patients with splenic aneurysms (10 aneurysms) treated in the years 2001-2006. In eight cases the treatment involved endovascular placement of metal embolisation coil into the aneurysm sac. In one female patient with two splenic artery aneurysms, following dissection of the proximal aneurysm and end-to-end anastomosis of the artery, the latter was repaired endovascularly in a delayed procedure. Results: In none of the described cases any serious complications were observed. Complete occlusion of the aneurysm lumen following endovascular procedure was observed in 7 of 9 cases (77.8%). Moreover, we managed to spare the spleen in all the described cases. Conclusions: Endovascular techniques provide a good alternative for open surgery in treatment of the splenic artery aneurysm, although the latter is still the treatment of choice in cases of rupture of visceral artery aneurysm.

Introduction. Primary extrahepatic hydatid cysts are rare, and primary splenic hydatid cysts even rarer. Splenic hydatidosis constitutes 2% to 3.5% of all hydatid cysts. Aim. To present a case report of splenic hydatidosis with abdominal pain. Description of the case. We report here a case of isolated splenic hydatid cysts in a 23 year old female, who presented with dull dragging pain in the left hypochondrium. Diagnosis was made on computed tomography imaging of the abdomen and microscopic examination of the laminated hydatid cyst wall and supplemented with positive enzyme linked immunosorbent assay for hydatid antibodies. Conclusion. The incidence of splenic involvement by hydatid cysts is very low. Man is an accidental intermediate host, as entry of the larval forms into humans represents an end stage in its life cycle. Until recently the gold standard treatment for splenic hydatidosis was splenectomy, as medical therapy seems to be ineffective. However, the last two decades have shown a tendency towards splenic conservative surgery in suitable cases, to reduce opportunistic post splenectomy infection.

Tuberculosis(TB) is one of the most common infections affecting the population in the developing countries. With the rising human immunodeficiency(HIV) infection its incidence is on a rise even in the developed countries. Pulmonary TB is the commonest form of infection, However multiple extrapulmonary sites have also been reported. Spleen is thought to be a rare organ involved in this infection. Various presentations of the splenic TB have been reported in literature.The definitive diagnosis of this is essentially formulated on the post splenectomy specimen. A consensus statement based on the available case reports is lacking. The authors are providing an insight into this form of extrapulmonary TB after reviewing the available literature.

In the beginning of the 20th century, enzymes with proteolytic activity were classified as peptidases, Erepsin, and proteases. Among these, pepsin, trypsin, and autolytic enzymes were of the protease class. Spleen-derived proteases were poorly characterized until Sven Gustaf Hedin performed several digestion experiments with bovine spleen. He incubated minced bovine spleen under acidic or neutral conditions and characterized two active proteases; the results were published in 1903. The first protease was named α-protease and was active under neutral conditions. The second was named β-protease and was active under acidic conditions. We replicated Hedin's experiments according to his methods and found, by using activity-based probes to visualize proteases, that the historical α-protease is the present-day serine protease cathepsin G (CatG), which is known to be important in several immune processes, including antigen processing, chemotaxis, and activation of surface receptors. The β-protease, however, comprised different proteases including CatX, B, S, and D. We suggest that Hedin described CatG activity in bovine spleen over 100 years ago.

An otherwise healthy 28-year old male presented to his general practitioner with dyspnoea in the morning and abdominal discomfort through months. Four months earlier, he had experienced a blunt trauma to the left side of his abdomen. Abdominal ultrasonography revealed a splenic hematoma and the patient was admitted to hospital. Vital signs were normal, and blood samples revealed a marginal anaemia and elevated C-reactive protein, but were otherwise normal. Computed tomography showed an 18 centimetre wide splenic hematoma. The patient was referred to another hospital for conservative treatment in the outpatient clinic.
