Introduction. Tendinopathy of the patellar tendon is a complex problem characterized by non-specific symptoms and a clinical challenge for therapists. Due to the frequency of occurrence of the described injury among players of volleyball, it is important to cooperate with the trainer - physiotherapist - a motor training instructor, which can significantly accelerate the athlete's recovery time.Aim of study. The aim of this study is to evaluate the effectiveness of kinesitherapy and soft tissue therapy in the therapeutic management of patellar tendinopathy.Case description, methods and research tools. The study included a patient suffering from tendinopathy of the patellar tendon. An additional challenge was that the patient was an active athlete. The VISA-p (ang. Victorian Institute of Sports Assessment for the Patellar tendon) questionnaire and clinical tests were used to evaluate the effectiveness of the therapy, among others: modified Thomas' test, Thomayer's test. In addition, other assessment methods were used, such as the UHBE (Unilateral Hip Bridge Endurance) test, visual assessment of ski jumping and landing techniques, and palpation. The test was used before and after the therapy lasting 8 weeks.Results. Based on the re-conducted clinical tests and the patient's completion of the VISA-p questionnaire, it can be concluded that the rehabilitation was carried out correctly and the goals were achieved. The patient's result in the questionnaire increased from 64 to 86 points, while clinical tests improved. The patient increased the strength of the injured lower limb and improved the technique of patterns specific to the sport he practiced.Conclusions. Kinesitherapy in combination with soft tissue therapy is an effective form of therapy in the case of tendinopathy of the patellar tendon. Tendinopathy of the patellar ligament makes it difficult not only to participate in trainings, but also to function on a daily basis.
Wstęp. Tendinopatia więzadła rzepki jest złożonym problemem, charakteryzującym się niespecyficznymi objawami oraz stanowiącym wyzwanie kliniczne dla terapeutów. W związku z częstością występowania opisywanego urazu wśród zawodników piłki siatkowej istotna jest współpraca na linii trener- fizjoterapeuta- trener przygotowania motorycznego, co może znacząco przyspieszyć czas rekonwalescencji sportowca.Cel. Celem niniejszej pracy jest ocena skuteczności kinezyterapii i terapii tkanek miękkich w postępowaniu terapeutycznym w przypadku tendinopatii więzadła rzepki.Opis przypadku, metody i narzędzia badawcze. Badaniem objęto pacjenta cierpiącego na tendinopatię więzadła rzepki. Dodatkowym wyzwaniem było to, że pacjent był aktywnym sportowcem. Do oceny skuteczności terapii wykorzystano kwestionariusz VISA-p (ang. Victorian Institute of Sports Assessment for the Patellar tendon) oraz testy kliniczne m. in.: zmodyfikowany test Thomasa, test Thomayera. Ponadto zostały użyte inne metody oceny takie jak test UHBE (ang. Unilateral Hip Bridge Endurance), wizualna ocena techniki wyskoku oraz lądowania i badanie palpacyjne. Badanie zastosowano przed oraz po terapii trwającej 8 tygodni.Wyniki. Na podstawie ponownie przeprowadzonych testów klinicznych oraz wypełnieniu przez pacjenta kwestionariusza VISA-p można stwierdzić, że rehabilitacja przebiegła prawidłowo, a cele zostały osiągnięte. Wynik pacjenta w kwestionariuszu zwiększył się z 64 do 86 punktów, natomiast testy kliniczne uległy poprawie. Pacjent zwiększył siłę kontuzjowanej kończyny dolnej oraz poprawił technikę wzorców specyficznych dla uprawianej przez niego dyscypliny sportowej.Wnioski. Kinezyterapia w połączeniu z terapią tkanek miękkich są skuteczną formą terapii w przypadku tendinopatii więzadła rzepki. Tendinopatia więzadła rzepki utrudnia nie tylko partycypację w treningach, ale i funkcjonowanie na codzień.

Collagens are major structural proteins of tendons, ligaments and other components of musculoskeletal tissues. Rare mutations in many of the genes, which encode for the collagen α-chains, result in serious musculoskeletal disorders, highlighting the importance of this protein family in the normal structure and function of musculoskeletal tissues. Since these rare mutations cause severe disorders, it has been proposed that a lack of biological redundancy exists within the collagen fibril, and that collagen-encoding genes are therefore ideal candidates for association with less severe exercise-related traits. This review identifies a number of collagen gene variants which are associated with various exercise-related traits. Based on the evidence outlined in this review, we propose that a general genetic continuum exists for collagen genes and their associated traits. At one end of this general continuum model, a single mutation within one or more collagen genes will result in severe Mendelian disorders. At the other end of the continuum, functional variants within these collagen genes collectively contribute to the aetiology of anomalous multifactorial connective tissue traits, which arise as a result of the interaction of genetic and non-genetic factors which modulate physiological responses to environmental stimuli.

Rotator cuff calcific tendinopathy is a common condition caused by the presence of calcification into the rotator cuff or in the subacromial-subdeltoid bursa. The pathogenetic mechanism of this pathology is still debated. Calcific tendinitis frequently affects the rotator cuff and may cause shoulder pain and reduction of range of motion. It can be diagnosed with conventional radiography, ultrasound, or magnetic resonance imaging. The first therapeutic option includes conservative management based on rest, physical therapy, and oral non-steroid anti-inflammatory administration. Extracorporeal shock wave therapy is a noninvasive technique that can be useful for the fragmentation of calcific deposits. Imaging-guided percutaneous irrigation is currently considered the gold standard technique for the treatment of calcific tendinitis due to its minimal invasiveness and its success rate of about 80%.

Background Regenerative injection therapies such as prolotherapy have gained importance in the recent years in non-surgical management of supraspinatus tendinopathy. The aim of the present study was to compare the efficacy of isometric rotator cuff strengthening versus corrective exercise program in the treatment of supraspinatus tendinopathy. Material and Methods Fifty patients aged 18 - 60 years were recruited with MRI diagnosed isolated supraspinatus tendinopathy or less than 50% thickness tear and symptoms persisting for longer than 6 months. All patients were randomised into two groups: one prolotherapy with isometric rotator cuff strengthening exercise therapy (group ISE; n = 25) and the other group also treated with prolotherapy injection followed by corrective exercise program (group CEP; n = 25). Patients were examined at baseline, 3, 6, and 12 weeks. Improvement was assessed using a visual analog scale (VAS) for pain, and Constant Murley Score (CMS) for shoulder function. Results Significant difference (p less than 0.001) was found between the groups in the VAS and CMS at any follow-up period. Conclusion Prolotherapy with CEP is more beneficial than prolotherapy with isometric strengthening exercise in supraspinatus tendinopathy. Improvement in pain and functional movements is more in the prolotherapy with CEP group than prolotherapy with ISE group.

This study aimed to perform a comprehensive systematic review, which reports the role of the Bonar score in the histopathological assessment of tendinopathy and its clinical relevance. To identify all of the studies that reported relevant information on the Bonar scoring system and tendinopathy, an extensive search of the major and the most significant electronic databases (PubMed, Cochrane Central, ScienceDirect, SciELO, Web of Science) was performed. A systematic review of the literature was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. The extracted data included—year of study, geographical location, type of the study, radiological modifications, gender, number of patients, region of tendinopathy, mean age, control group, characteristics of the Bonar score and alterations in the scale, mean Bonar score, number of investigators, area of tendon investigation, clinical and radiological implications. An extensive search of the databases and other sources yielded a total of 807 articles. Eighteen papers were finally included in this systematic review, and of these, 13 original papers included the clinical and radiological implications of tendinopathy. Radiological evaluation was present in eight studies (both magnetic resonance imaging (MRI) and ultrasound (US)). The clinical implications were more frequent and present in 10 studies. Using the Bonar score, it is easy to quantify the pathological changes in tendinous tissue. However, its connection with clinical and radiological evaluation is much more complicated. Based on the current state of knowledge, we concluded that the neovascularization variable in the Bonar system should be reconsidered. Ideally, the microscopic assessment score should follow the established classification scale with the radiological and clinical agreement and should have a prognostic value.

Relationship between calcific tendinopathy and rotator cuff tear on shoulder magnetic resonance imaging : case-controlled comparison

Purpose: To determine the frequency of rotator cuff tear on the shoulder magnetic resonance imaging (MRI) of patients with rotator cuff calcific tendinopathy and the relationship between rotator cuff tear and calcific tendinopathy. Material and methods: In this retrospective case control study, 137 patients with calcific tendinopathy and 137 control group patients without calcific tendinopathy with shoulder pain, whose age, sex, and shoulder laterality values were matched, were compared in terms of rotator cuff tears on their shoulder MRI images. Results: The frequency of rotator cuff tear was found to be significantly higher in the control group (37.2%) compared to the calcific tendinopathy group (23.4%) (p < 0.01). Partial thickness was 81.3% in the calcific tendinopathy group and 70.6% in the control group, and no significant difference was observed between the two groups in terms of the size of the rotator cuff tear (p > 0.05). In the calcific tendinopathy group, there was no significant relationship between the localisation of calcification and the rotator cuff tear, and only in 4.4% of the participants were calcification and tear at the same location on the same tendon (p > 0.05, r = 0.04). Conclusions: The patients with calcific tendinopathy, who had been admitted with shoulder pain, did not demonstrate an increased risk of rotator cuff tear based on their MRI compared to patients with shoulder pain without calcific tendinopathy. No significant relationship was determined between calcific tendinopathy and rotator cuff tear.

Running has been more popular activity for second middle of XX century. Following the ancien running Instinct socjety becomes more agile. However, like any sport, it may couse many injuries. One of the most populat is Achilles tendinopathy. It characterized by pain, structural chages decrease strenght, power and mobility.It caused by excessive overstrain.The subject of this MA thesis are discuss about this injury and characterize the risk group. Another important apect is the correlation between the characteristics of the respondents and the course of the injury.Resaerch material were 63 long-distance runners. , Individual features, training loades, course and kind of treatment of the injury were collected.Research method was a questionnaire which contain 24 questions. The chi square test of independence was used to answer the research questions.Older people need more time to recovery from Achilles tendinopathy. It has been observed the correlantion between asking medicine specialist for hel and the duration of treatment of the injury. People with more serious problems more often asking for help.
Od połowy XX wieku bieganie przeżywa ogromny wzrost popularności. Społeczeństwo idąc za pradawnym instynktem biegowym staje się coraz bardziej sprawne. Jednak jak każdy inny sport, uprawiany na coraz wyższym poziomie, niesie za sobą ciężar urazów. Jednym z najczęściej spotykanych jest tendinopatia ścięgna Achillesa. Ta przypadłość charakteryzuje się bólem, osłabieniem funkcji oraz zmianami strukturalnymi spowodowanymi nadmiernymi przeciążeniami.Celem pracy była analiza tendinopatii ścięgna Achillesa w grupie biegaczy oraz scharakteryzowanie czynników ryzyka. Kolejnym ważnym aspektem było znalezienie korelacji między cechami osobniczymi ankietowanych a przebiegiem urazu.Materiał badawczy to grupa 63 biegaczy, przede wszystkim długodystansowych. Określono cechy osobnicze, charakterystykę ich treningów oraz przebieg i metody leczenia urazu.Metodą badawczą było badanie ankietowe składające się z 24 pytań, w większości zamkniętych jednokrotnego wyboru. W celu znalezienia zależności posłużono się testem niezależności chi kwadrat.Zaobserwowano, że starszy wiek może wydłużyć czas leczenia tendinopatii ścięgna Achillesa. Wykazano również zależność między korzystaniem z usług specjalistów a czasem leczenia urazu. Do fizjoterapeutów i lekarzy zgłaszają się osoby mające poważniejsze problemy.

The purpose of this study was to investigate the effectiveness of different bacterial collagenase solutions for the induction of tendinopathy in laboratory animals. The search strategy was performed in the following databases: PubMed, ResearchGate, Science Direct and Wiley Online Library. Inclusion criteria was the use of bacterial collagenase type I from Clostridium histolyticum for induction of yatrogenic tendinopathy in animal model. Forty one articles were found relevant to this topic. Different solutions of bacterial collagenase type I were used for induction of tendinopathy and it was found that it could be effective as injury factor even in the low concentrations. Overall, the collagenase tendon injection is a safe and productive method for yatrogenic induction of tendinopathy origin.
