The assessment of exposures of preterm newborns with respiratory disorders to selected traumatic factors generated in the process of nursing care : an observational study

Background: The hospitalization of preterm newborns carries a risk of frequent exposure to traumatizing factors generated by the hospital environment and multi-specialty services. Aim of study: The assessment of premature newborns with respiratory failure and exposure to traumatic factors such as pain and touch generated during nursing procedures. Material and methods: The study was conducted on the 1st and 7th days of hospitalization during 2020 of a group of 10 premature newborns with respiratory disorders at the Department of Newborn Pathology of the Clinical Department of Neonatology at the University Hospital in Krakow (Poland). The research method was based on the 24-hour observation of newborns for exposures to factors generating pain and traumatic touch. The research tools included scales for assessing pain expression — Neonatal/Infant Pain Scale (NIPS), CRIES, and an original questionnaire. Results: On the first day of hospitalization, the study group was subjected to 541 nursing procedures exposing the newborns to touch and generating pain. On the 7th day of hospitalization, the number of the procedures was 388. The average intensity level of procedural pain on the 1st day of hospitalization was 4.1 points (NIPS) and 2.5 points (CRIES), while the 7th day of hospitalization was 4.0 points and 1.8 points, respectively. The highest average pain intensity was generated by venous blood sampling for diagnostic tests and peripheral vessel cannulation. Conclusions: Preterm newborns with respiratory failure are exposed to a significant number of procedures and actions considered traumatic. Therefore, it is reasonable to use more than one pain assessment scale for more objective results.

Ofiary doświadczające wykorzystywania seksualnego przeżywają nie tylko fizyczną krzywdę ale przede wszystkim doznają głębokich urazów psychicznych, które bardzo często w różnych formach towarzyszą im przez całe życie. Dość często przy ustalaniu następstwbagatelizuje się płeć sprawcy. Dlatego bardzo ważne jest poznanie problemu nadużyć seksualnych z perspektywy bliskich i odległych następstw w przypadku, gdy sprawczynią tych czynów była kobieta – matka, opiekunka. W artykule, wykorzystując metodę przeszukiwania źródeł wtórnych, przedstawiono problem następstw seksualnego wykorzystywania u ofiar, które zachowań tych doświadczyły ze strony kobiet. Celem opracowania jest omówienie specyfiki przeżywanej przez ofiary obojga płci traumy powiązanej z wykorzystaniem seksualnym a także bliskich i odległych konsekwencji tych wydarzeń w postaci wiktymizacji pierwotnej oraz rewiktymizacji zaznaczającej się w dorosłym życiu w postaci zaburzeń psychoseksualnych i deficytów społecznych. Wśród konsekwencji na szczególną uwagę zasługują wyodrębnione czynniki traumatyczne o charakterze dynamicznym, które wywołują pojawienie się wielu symptomów charakterystycznych dla dzieci, które doświadczyły przemocy seksualnej. Wspomniane czynniki to: traumatyczna seksualizacja dziecka, zdrada, stygmatyzacja oraz bezsilność. Specyfika tych czynników wynika z faktu, że pozostawiają one odległe „ślady” w psychice i mogą ulec dodatkowemu wzmocnieniu w sytuacji, gdy sprawcą przemocy seksualnej jest kobieta. Wynika to z częstych postaw „zaprzeczania” wobec przemocy seksualnej stosowanej przez kobiety.W opracowaniu wskazano także na jedną z możliwych konsekwencji procesu rewiktymizacji jaką jest powielanie przez ofiary w ich późniejszym życiu wzorów zachowań powiązanych z seksualnym wykorzystywaniem dzieci. Proces ten wynikający z naprzemiennej identyfikacji sprawczyni i ofiary uruchamia mechanizm błędnego koła, który utrudnia tworzenie prawidłowych interakcji społecznych.
Victims experiencing the sexual abuse are surviving not only physical injustice but above all deep traumas, which very often in different forms, are keeping them company through the entire life. Quite often at establishing different results a sex is underestimated for the perpetrator. Therefore knowing the problem of sexual abuses from a perspective of close as well as distant results is very important in the event that a woman was a perpetrator of these acts – mother, minder. In the present article based on analysis of literature, a problem of results of the sexual abuse was presented at victims which experienced these behaviours on the part of women. In order to draw up discussing the survived specificity by victims was both of sex of the trauma connected with the sexual application as well as close and distant consequences of these events in the form prime victimisation and revictimisation for figure being noticeable in the adult life of psychosexual disorders and social shortages. Amongst the consequence isolated traumatic factors are deserving the particular attention about dynamic character which are provoking the appearance of many symptoms characteristic of children which experienced the sexual violence. Recalled factors it: traumatic sexualisation of child, the betrayal, the stigmatization and the helplessness. The specificity of these factors results from the fact that they will leave distant “tracks” in the psyche and they can undergo the additional reinforcement if a woman is a perpetrator of the sexual violence. It results from frequent attitudes of “denying” towards the sexual violence applied by women. In the study they pointed also at one of possible consequences of the revictimisation process copying patterns of behaviour connected with the sexual exploitation of children in their more late life by victims is which. This process resulting from the alternating identification of the perpetrator and the victim is starting the mechanism of the vicious circle which disturb creating normal social interactions.

The heterogeneous population of patients affected by trauma is extremely difficult to systematize. This is due to the diversity of mechanisms of injury, the nature and severity of the injury and the population, which relate to injuries, diverse in terms of gender, age, presence of comorbidities that make up the final severity of the injury and a certain degree of danger to life. The aim of the study was to develop a universal method to assess the severity of injury and loss of life resulting from their consequences, using the parameters available in the Polish administrative databases, a similar diagnostic efficacy as other used scales to assess the severity of damage. Material and methods. The study analyzed a group of 92 463 patients hospitalized due to injuries sustained as a result of injuries in all hospitals of the Lublin region in 2003-2005. Developed catalogs risk factors, reference to the population of the Lublin province. Results. Developed five models predictive of injury severity scale counterparts, which include different combinations of risk factors associated with the type of injury, age of the patient and the mechanism of injury that have been evaluated for their diagnostic efficacy in differentiating the expected outcomes. Prediction model have the best diagnostic efficacy AUROC 0.9615, SE 0.0025 and 95% CI 0,9565-0,9665, hereinafter referred to as Life Hazard Ratio (LHR), which has a similarly high diagnostic efficacy as the other, examined in the work of the rock assess the severity of damage AUROC 0.9585, SE 0.0368, 95% CI0 0.8849-1. Conclusions. 1. The proposed method of use of the International Classification ICD-10 allows the use of regional administrative databases to conduct detailed analyzes of population and monitor trends in the epidemiology of injuries. 2. Developed Life Hazard Ratio (LHR)is a universal method for the objective evaluation of the severity of injuries and the associated risk of life-threatening, can also conduct population studies. 3. LHR has a comparable rate of diagnostic efficacy as other commonly used scales assessing the severity of the injury.

The paper discusses the post-traumatic disorders as the consequence of disaster, their course and determinants. As an illustration, the summary of the results of the Polish studies on flood survivors, conducted in 1999-2007, was presented. The studies were focused on the identification of risk factors for post-traumatic stress disorder (PTSD), assessed in several groups of victims in the early (about three months) and late phase (from one to years) after the flood. The findings indicate that PTSD is a multi-causal phenomenon with many risk factors, preceding the flood and operating during the trauma and in the post-traumatic period, which result in arousing and maintaining in time PTSD symptoms. The main conclusion of Polish studies on flood survivors underlines the eminent role of personality traits, including temperament, as well as within-family factors influencing PTSD.

ObjectivesThe problem is to determine whether, after a disaster, unemployment is a possible cause of post-traumatic stress disorder (PTSD) – the classic model, or whether PTSD is a possible cause of unemployment – the alternative model.Material and MethodsThe study was conducted on a probabilistic sample of 26 213 adults who provided responses using the Davidson Trauma Scale and lived in regions near or far from the epicenter (Cobquecura, the coast of Chile) of the earthquake and tsunami of February 27, 2010.ResultsIndependent of the proximity to the epicenter, there is an association between PTSD, unemployment, female sex and the poverty line. For regions close to the epicenter, the alternative model has better adjustment indicators than the classic model.ConclusionsGiven the adjustment of the alternative model, the occurrence of PTSD cases is more likely to explain the unemployment condition.

The overall positioning of the functioning of an individual in a professional work has been a subject of interest in numerous fields of science. Nowadays, a great amount of consideration has been given to the subject of high-risk professions, which entail severe disorders in professional activity. The aim of this article is to present the impact of stress on the professionals who work in the uniformed services of the police, fire department, correctional department, therefore such individuals who are carrying out their chores under conditions of being particularly at risk of experiencing various types of traumatic incidents. The article, based on the both theoretical and empirical researches of the experts of this issue, presents the main criteria, symptoms and factors conditioning development of the post-traumatic stress disorder. It also presents data indicating the scale of this phenomenon and the effects of the trauma associated with acting a stressful professional role. The interest for this issue stems from the fact of an increasing amount of the traumatic experiences in the world. It is estimated that ca. 70% of the population has experienced in their life this kind of occurrence, while the post-traumatic stress disorder is considered as one of the most severe of its consequences, leading to negative psychosomatic symptoms, which disorganise a hitherto functioning in all of the life spheres. The aspects addressed in this article, represent a cognitive value, since they expose the widely understood traumatic situation of the individuals working in the high-risk professions. Furthermore, another value added of this paper is its diagnostic character due to the presentation of the knowledge not only about the issue of trauma, but also of the criteria of the post-traumatic stress disorder.

Introduction. The symptoms of dissociation, depersonalization and derealization are often associated with exposure of patients to mental and physical injuries, usually occurring in childhood. Most of these observations were carried out in populations of patients with various disorders (posttraumatic, conversion-dissociation, personality disorders – especially borderline), who reported their exposure to adverse life circumstances through questionnaire interviews. Aim. Assessment of the risk associated with various traumatic events in childhood and adolescence concerning the symptoms of pain and tactile dissociation, depersonalization and derealization. Material and method. The coexistence of the earlier life circumstances and the currently existing symptoms was examined on the basis of KO “0” Symptom Checklist and Life Inventory, completed prior to treatment in a day hospital for neurotic disorders. Results. In the group of 2582 women, patients of a day hospital for neurotic and personality disorders, the symptoms of pain and tactile dissociation, depersonalization and derealization were present in 24-36 % of patients, while the maximum severity of these symptoms reported approximately 4-8 % of patients. The studied patients reported the exposure during childhood and adolescence (before 18yo) to numerous traumatic events of varying severity and frequency, including hostility of one parent (approximately 5% of respondents), the sexual initiation before 13yo (1%), worse than peers material conditions (23%), harassment of the family of origin (2%), reluctance of their peers (9%). Conducted regression analysis showed illustrated by the coefficients OR (odds ratios) a statistically significant relationship between the majority of the analyzed symptoms and many of the listed events, such as being regarded as worse than siblings, mother’s anger in the situation of the patient’s disease in childhood, lack of support, indifference of parent, poverty and worseness of the family of origin, inferior position in the classroom and the school grades, total sexual unawareness, incest or its attempt. Conclusions. The symptoms of dissociation, depersonalization and derealization occurred in significantly more patients reporting burdening life events – difficult situations in childhood and adolescence. Therefore, in clinical practice in patients presenting such symptoms, regardless of diagnosis (e. g. a specific neurotic disorder), we can expect revealing information about such events.
