We consider the problem of fluid flow in a brain tumor. We develop a mathematical model for the one-dimensional fluid flow in a spherical tumor where the spatial variations of the interstitial velocity, interstitial pressure and the drug concentration within the tumor are only with respect to the radial distance from the center of the tumor. The interstitial ve- locity in the radial direction and the interstitial pressure are determined analytically, while the radial variations of two investigated drug concentrations were determined numerically. We calculated these quantities in the tumor, in a corresponding normal tissue and for the concentrations also in the cavity that can exist after the tumor is removed. We determine, in particular, the way the interstitial pressure and velocity vary, which agrees with the expe- riments, as well as the way one drug concentration changes in the presence or absence of a second drug concentration within the tumor. We find that the amount of drug delivery in the tumor can be enhanced in the presence of another drug in the tumor, while the ratio of the amount of one drug in the tumor to its amount in the normal tissue can be reduced in the presence of the second drug in the tumor and the tissue.

Disseminated lesions of the central nervous system in course of pediatric brain tumors

Background: Neoplasms of the central nervous system (CNS) are, apart from leukemia, the most frequent malignant disorders in the childhood. Among the brain tumors, those of poorly differentiated cells - give metastatic lesions to the CNS. The aim of the paper was to evaluate the features of CT and MR images detecting dissemination of the primary brain tumors. Material/Methods: From 1993 to 2005 in the Department of Radiology of the Polish Mother’s Memorial Hospital - Research Institute, the disseminations to CNS were observed in 35 children who were previously operated for primary brain tumors. CT and MR examinations of the brain were performed in all patients (22 males and 13 females; age: 5 mo - 18 y) and MR imaging of the spinal cord was done in 18 children. Results: Multiple metastases to the cerebral structures were detected more often (in 23 patients - 66%) as compared to single lesions. The most frequent disseminations were observed in patients with diagnosis of medulloblastoma - 13 children, PNET - 4 and pineoblastoma - 3 patients. Twelve children had single metastatic tumors (out of the primary neoplasm location): in the course of medulloblastoma - 6, and PNET - 2 patients. Eighteen MR examinations of the spinal canal showed disseminations of the brain tumors in 9 children; concomitant metastatic nodules in the brain were detected in 4 patients. Conclusions: CT and MR imaging of the CNS enables evaluating the dissemination of primary brain tumors in children. Any asymptomatic progression of the primary neoplastic disease may be detected by means of control diagnostic imaging, which reveals the tumor spread. Especially in patients with medulloblastoma and pineoblastoma, the spine MR imaging with gadolinium is mandatory.

Sarcoidosis is a systemic disease of unknown etiology. It is characterized by a formation of lumps known as noncaseating granulomas, which are not affected by necrosis. Typical collections of inflammatory cells may be found in any organ tissues but mainly in the lungs, skin or lymph nodes. Head and neck organs involvement is rare, although sarcoidosis can be found in lymph nodes, salivary glands, larynx and sinuses. A search was performed based on medical records of patients admitted to the Department of Otolaryngology within four previous years. We analyzed two cases of sarcoidosis of salivary glands (parotid and submandibular), two cases of larynx involvement and one case of lymphadenopathy due to sarcoidosis. Patients (4 women and 1 man), age range 26–76 years old, were directed to the Otolaryngology Department with neck lymphadenopathy or a suspicion of a neoplastic tumor of salivary gland or larynx. Only one patient had previous diagnosis of pulmonary sarcoidosis. Preoperative diagnostic procedures such as ultrasonography, biopsy, chest radiography did not suggest sarcoidosis on the initial assessment in other patients. Surgical removal of the pathological findings and histopathological examination confirmed extrapulmonary sarcoidosis. The computed tomography of lungs confirmed coexisting pulmonary sarcoidosis in only one patient.

Anaplastic astrocytoma (AA, WHO grade III) and glioblastoma multiforme (GBM, WHO grade IV) are malignant tumors of the brain. The average survival time of patients with GMB is approximately one year and two years in the case of anaplastic astrocytoma with standard therapy based on surgical tumor resection followed by chemotherapy or radiotherapy. High invasiveness of gliomas, the ability of rapid division and so-called diffusive infiltration of tumor cells into normal brain tissue, which prevents complete surgical removal, are hallmarks of theses tumors. Therefore, new specific therapies for eliminating cancer cells are needed to treat this tumors. Recently, it has been demonstrated that alternating electric field, also known as tumor treating fields (TTFields) has a unique mechanism of destroying glioma cells. TTFields applies electromagnetic energy frequency-dependent and intensitydependent and disrupts cancer cell replication as they undergo mitosis. Futhermore, TTFields turn out to act comparably to conventional chemotherapeutics, lacking numerous side adverse associated with chemotherapy. The authors provide an up-todate review of the mechanism of action as well as preclinical and clinical data on TTFields.

Introduction. Appendix, located within the Spiegel hernia is a rare condition. Few cases have been reported to date. Although it is generally asymptomatic, patients can apply with strangulation findings. Along with the physical examination findings, imaging methods play an important role in diagnosis and definitive diagnosis is made intraoperatively. Per-operative surgical method is determined according to the condition of the structures in the hernia sac. If an appendix is detected in the hernia sac, appendectomy is often preferred regardless of symptoms. Postoperative pathology is mostly benign but malign appendix pathologies should be kept in mind. Aim. Here, we aimed to present our case undergoing emergency surgery due to incarcerated hernia as it is the first case of appendix neuroendocrine tumor in the Spiegel hernia sac according to our literature review. Description of the case. A 77-year-old female patient who was admitted to the hospital with complaints of nausea and vomiting was evaluated as an emergency. In the clinical evaluation of the patient, we detected ileus due to hernia. We operated on the patient and found the appendix and cecum in the spiegel hernia. We did appendectomy and hernia repair. Histopathological examination of the appendix revealed a well-differentiated neuroendocrine tumor. Conclusion. Detection of the appendix in a Spiegel hernia is a rare condition. This is the first case of appendiceal malignancy in a Spiegelian hernia.

Introduction: Giant cell tumor of the tendon sheath is the most common benign proliferative lesion involving the upper limb, characterized by relatively high recurrence rate after surgery. Aim: The objective of the study was a retrospective analysis of outcomes of the operative treatment of these tumors, in a longterm (a mean of 4,2 year) follow-up. Material and methods: Preoperative examination was performed in 58 patients, 36 females (62%) and 22 males (38%), in a mean age of 41 years, and treatment outcomes were assessed in 47 persons (81% of the operated patients), at a mean of 4.2-year follow-up. The final assessment was performed in a form of phone interview. Results: The most common site of the tumors was the fingers – 42 cases (72%). In 31 patients (53%) the lesion had a well-defined capsule, and in 11 (19%) a satellite nodule was found around the main tumor. A total of 9 relapses (21%) occurred, all within the first 2 years following surgery. Two patients had a next episode of recurrence after the second operation. In 8 out of the 9 patients with a recurrence, the primary lesion did not have a well-defined capsule. In 38 patients who had no relapse, 31 were completely symptom-free, whereas 7 complained of mild pain of the scar and/or numbness of a part of the involved finger. Conclusions: The main factor that impacted the high rate of recurrence was incomplete tumor excision, which resulted from inadequately accurate surgery and the tumor morphology (having no well-defend capsule). The role of operating with the use of magnifying devices and keeping a greater surgical margin at resection of the non-capsulated lesions was emphasized, as it could translate into reducing the recurrence rate.

Opis:
Myoblastoma is a rare tumor deriving from Schwann cells. It is located mainly in the tongue, skin and subcutaneous tissue, as well as in the bronchi and digestive tract, although it is rarely found in the larynx. It is a benign tumor but recurrence or even malignant transformation is possible. In case of laryngeal location symptoms include hoarseness, cough, discomfort in the larynx or dyspnea. Surgical treatment is necessary through endoscopic or external approach. Authors present two cases of Abrikossoff’s tumor of the larynx. In the first case, tumor was located in the posterior commissure, reaching up to 15 mm in size. Hoarseness was the only symptom. In the second case, tumor was located on the vocal fold near the posterior commissure. Surgery was performed in both cases. In the first case, we used external approach through a laryngofissure, while in the second case surgery was performed endoscopically. Patients have been under follow-up in our Department for several years without relapse.

The aim of this study is to depict and to analyze the play "Tumor Mózgowicz" written by Stanisław Ignacy Witkiewicz in the aspect of the battle of the sexes and to present the importance of this subject in the whole literary output. For this purpose, the analysis of selected dramatic person shall be performed, with particular emphasis on the figures of Titan and Demonic Woman. Their relationships are shown in terms of battle of the sexes and are confronted with misogynic theories of Otto Weininger. Then the analysis is extended to the issues of Ernst Kretschmer’s typology and the issues of Sigmund Freud’s psychoanalysis. To emphasize the ambivalent nature of women, the Mother Goddess theme is presented. Finally, the issue of battle of the sexes is related to the general in the works of Stanisław Ignacy Witkiewicz, catastrophism. Although subjects raised in this study (battle of the sexes, Demonic Woman, eroticism) had already been discussed in various publications about Stanisław Ignacy Witkiewicz, here they have been extracted from all other conditions and put forward on the first plan.
Celem pracy jest zbadanie motywu walki płci w dramacie "Tumor Mózgowicz" Stanisława Ignacego Witkiewicza. W tym celu przeprowadzona zostaje w pierwszej kolejności analiza wybranych person dramatycznych, ze szczególnym uwzględnieniem Tytana i Kobiety Demonicznej. Ich relacje zostają ukazane w aspekcie walki płci i skonfrontowane są z mizoginicznymi teoriami Otto Weiningera. Następnie analiza postaci zostaje poszerzona o zagadnienia z typologii Ernsta Kretschmera i zagadnienia z psychoanalizy Zygmunta Freuda. Aby podkreślić ambiwalentny charakter kobiecych postaci przywołany zostaje motyw Bogini Matki. Na zakończenie, zagadnienie walki płci zostaje odniesione do powszechnego w twórczości Stanisława Ignacego Witkiewicza, katastrofizmu. Choć poruszane w niniejszej pracy tematy (walka płci, Kobieta Demoniczna, erotyzm) były już wcześniej omówione w różnych publikacjach dotyczących twórczości Stanisława Ignacego Witkiewicza, tutaj zostały wydobyte z wszelkich innych uwarunkowań i wysunięte na plan pierwszy.
