Opis:
Influenza is a global health concern. The single most effective way of protecting people against influenza infection and disease is vaccination. However, currently available vaccines against influenza induce only strain-specific immunity, and do not elicit long-lasting serum antibody titers. Therefore, they are ineffective in the case of possible pandemics. There is an urgent need for a new generation vaccine which would induce broad and long-lasting immune protection against antigenically distinct flu viruses. The paper presents recent achievements and the challenges in the field of universal vaccine construction.

The association of conspiracy beliefs and the uptake of COVID-19 vaccination: a cross-sectional study

The COVID-19 pandemic revealed that health denialism might be an important determinant of adherence to preventive measures during epidemic challenges. Conspiracy beliefs seem to be one of the most visible manifestations of denialism in society. Despite intensive efforts to promote COVID-19 vaccinations, the number of citizens reluctant to get vaccinated was very large in many countries. The main aim of this study was the analysis of the association between the acceptance of the COVID-19 vaccination and conspiracy beliefs among adult Internet users in Poland. The analysis was based on data from a survey performed on a sample of 2008 respondents in October 2021. Uni- and multivariable logistic regression models were applied to evaluate the association between attitudes towards COVID-19 vaccination and generic conspiracist, vaccine-conspiracy, and COVID-19-related conspiracy beliefs. In the multivariable model, the effect of conspiracy beliefs was adjusted for the level of vaccine hesitancy, future anxiety, political sympathies, and socio-demographic variables. Univariate regression models showed that COVID-19 vaccination acceptance is significantly lower among respondents with higher levels of all three types of conspiracy beliefs. In the multivariable model, the effect of COVID-19-related and vaccine conspiracy beliefs, but not generic conspiracist beliefs, was maintained after adjusting for vaccine hesitancy. We conclude that conspiracy beliefs should be treated as a potential indicator of lower adherence to preventive measures during epidemic challenges. The respondents revealing a high level of conspirational thinking are a potential group for intensified actions which employ health educational and motivational interventions.

Objectives Vaccination skepticism and vaccination refusal both constitute global public health concerns. Systematic monitoring of public attitudes towards vaccination is crucial for maintaining a high vaccination coverage rate. The study aimed to identify and characterize homogenous social groups distinguished by attitudes toward preventive vaccinations. Material and Methods Between October and November of 2021, a nationwide representative cross-sectional survey was conducted on a representative sample of 1560 inhabitants of Poland aged 15–39 years. The study questionnaire included 60 questions on public attitudes towards vaccines, vaccination, and the COVID-19 pandemic. Factor analysis was applied to identify the main dimensions of vaccination attitudes. Results Factor analysis included 22 variables and yielded 3 factors or dimensions that accounted for 48.5% of the model’s variability. Young adults were assigned into 6 homogeneous groups based on these factors: 1) general trust in vaccination, 2) vaccine safety concerns, 3) trust in fake medical news regarding COVID-19 vaccination, and denying the COVID-19 pandemic. Groups I–IV differed by sociodemographic factors and vaccination coverage rates. Nearly 60% of the respondents expressed concern regarding various aspects of vaccination. Conclusions Most of Poland’s young adults lack clearly defined attitudes towards vaccination. Attitudes towards vaccination can be divided into 6 heterogeneous groups.

Although the COVID-19 vaccine has been recommended as the safer and more effective prevention for COVID-19 disease relative to other alternative medications, yet across the globe, many people are resistant to receiving it. Setting out to explain such a paradox, we conducted an online survey among a sample of Indonesians (N = 4758) when the World Health Organisation (WHO) granted authorisations for the clinical trial of various vaccines against COVID-19. The results revealed that participants’ support for theories that the COVID-19 vaccine is invented to harm their nation (i.e., COVID-19 vaccine conspiratorial beliefs) positively corresponded with the perceptions that international collaboration in the COVID-19 vaccine clinical trial is not aligned with their nation’s actual needs (i.e., the perceived assumptive international collaboration) and negative attitudes towards the vaccine. In turn, the perceived assumptive international collaboration was positively related to negative attitudes towards COVID-19 vaccine. We also showed that the positive relationship between COVID-19 vaccine conspiratorial beliefs and the perceived assumptive international collaboration in the vaccine clinical trial was more prominent among participants who were strongly resistant to take vaccines supplied by other countries due to national pride (i.e., vaccine national glorification).

Committed pro- and anti-vaccine groups : how they view each other’s competence, warmth, and level of identity

This paper investigates how active opponents and supporters of vaccination (meeting the criteria of group members) perceive each other in the context of the dimensions of competence and warmth, and how they perceive each other’s group identity. The study was conducted online, the final research sample included 192 committed pro-vaccine and 156 committed anti-vaccine group members. Both groups assigned an average level of competence to the other group, while they rated the outgroup’s warmth as below average. These results may be associated with the conflicting attitudes of these groups. It also turned out that both groups evaluated each other’s identities above the theoretical average. Results are discussed in context of the concept of in-group bias and the heterogeneity of motivations causing vaccine rejection.

Background: Social attitudes and context have a fundamental impact on the COVID-19 vaccine acceptance. This issue will be challenging the existing Polish public health infrastructure during the planned immunisation program. Material and methods: We attempt to exemplify Poland-specific social background via causal diagram and regional factor analysis based on dimensions (epidemiological potential, ability to act, magical thinking, koronascepticism, information needs etc.) by December 2020 – shortly before vaccination started. Results: We identified several potential target subpopulations and provide possible regional classification. We distinguish clusters of regions where communication strategies should be taken into consideration: 1) extending campaign reach, common social goods and conformism; 2) individual’s profits and misinformation blocking. Conclusions: We are signalling the need for profiling and regionalization in campaigns and propose possible starting points for protocols for various voivodeships when safe and effective vaccines are available in Poland.

Transparency as a double-edged sword : public health ethics perspective

Transparency has gained much attention in both public health and medical contexts. Alberto Giubilini, Rachel Gur-Arie and Euzebiusz Jamrozik make a valuable contribution to addressing the ethical question on the value and role of transparency by claiming that transparency is a necessary condition for the moral trustworthiness of experts. This comment is conceived as a follow-up on this issue. Usually, ethical discussions on transparency concern its consequentialist aspects such as the influence of disclosure on stakeholders’ trust. However, as the author will claim, there is something morally significant and non-consequentialist at stake. The aim of the comment is to highlight two important aspects concerning transparency. Firstly, its role in public health ethics, and, secondly, the moral basis for this role. Regarding the former, the author will claim that transparency influences moral legitimacy of coercive public health policies. Regarding the latter, the author will claim that by being transparent, policymakers respect stakeholders as moral agents capable of reason.

The popularity of the 5- and 6-component combination vaccines among infants parents in a selected Primary Health Care Clinic

Introduction and objective: Vaccinations as one of the highest achievements of medicine contributed to a significant reduction of infectious diseases. According to the current vaccination calendar in Poland, parent have possibility to choose free cost classic (DTP, polio, HBV) and not refundable 5- or 6-component combination vaccine. The aim of the study was to assess the popularity of the 5- and 6-component combination vaccines in years 2010-2019 among infants' parents starting a mandatory vaccination series in a selected Primary Health Care Clinic. Material and methods: The study was conducted at Med-All Medical Centre in Krakow which provided Protective Vaccination Program and included 1108 immunization cards of children born between 2010-2019. Most of the sample (98.5%) lived in Cracow. This retrospective ecological study was used analysis of medical records. Data analysis MS Excel (Microsoft) and PQStat Soft. The significance level was p< 0.05. Results: From 2010 to 2016, 5-component combination vaccine was the most popular choice for infants. During the study period the 5-component combination vaccine usage decrease with years (p=0.017). From 2017, the most popular was the 6-component vaccine and the usage increase with years with strong statistically significance (p=0.005). In 2019 there was the largest percentage of unvaccinated infants. Conclusions: Despite the costs, in 2010-2016, the most popular vaccine for infants was the 5-component high-combination vaccine. Since 2017, the 6-component vaccine has gained the greatest popularity.

COVID-19 vaccination raises numerous concerns among the public, and also among medical personnel including nurses. As nurses play a crucial role in the process of vaccination, it is important to recognize the attitudes of students of nursing, nurses in spe, toward COVID-19 vaccination, as well as to define the factors influencing students’ pro-vaccine choices. The study was conducted between March and April 2021 at all medical universities in Poland educating nurses in spe. The study included 793 first-degree students from 12 universities. The results revealed that the vast majority of students of nursing (77.2%) were vaccinated against COVID-19, as 61.2% received an mRNA vaccine and 16% a viral vector vaccine. Every other person in the non-vaccinated group declared their intention to get a vaccination. A trend was observed whereby people co-living with persons from the risk group, who are at risk of a severe form of COVID-19, showed greater willingness to get a vaccine. The study results identified the role of universities in increasing the vaccination rate among students, both in terms of education about vaccinations and in shaping pro-vaccine attitudes among students, as well as organizing vaccinations on university campuses to facilitate the process.
