Opis:
In liberal societies it seems to be important to provide orientation by philosophizing at school. We are used to doing this by discussing classic ethics with our students. Here, skills like rational argumentation can be trained. It is the universal rationality that can be applied to different ethical issues and, thus, provide orientation. When it comes to this learning objective phenomenology and postmodernism are mostly not expected to provide assistance. Phenomenology might be seen as just dealing with perception whereas postmodernism is under suspicion for contributing to indecision, arbitrariness and relativism. In this article I will try to outline the potentials of phenomenology and postmodernism in the field of orientation. In the tradition of Husserl’s ‘epoché’ we can let students discover the perspective of a first person and what it means to be a ‘self’. Heidegger and Merleau-Ponty have not only described a certain closeness to the world which can be described as ‘dwelling’ of a lived body. They have also delineated elements of a new ‘postmetaphysical’ and at the same time ‘prehermeneutical’ metaphysics. All this can help to open the depth of self, life, and world. Postmodern thinkers claim a plurality of truths. By this means, these theories can encourage self-empowerment. At the same time, authors like Lévinas (responsibility for the other), Lyotard (the sublime), and Rorty (solidarity) describe new ways of openness towards the world which are not founded by any primal truth and thus provide orientation.

Opis:
Households play an important role in disease dynamics. Many infections happening there due to the close contact, while mitigation measures mainly target the transmission between households. Therefore, one can see households as boosting the transmission depending on household size. To study the effect of household size and size distribution, we differentiated the within and between household reproduction rate. There are basically no preventive measures, and thus the close contacts can boost the spread. We explicitly incorporated that typically only a fraction of all household members are infected. Thus, viewing the infection of a household of a given size as a splitting process generating a new, small fully infected sub–household and a remaining still susceptible sub–household we derive a compartmental ODE model for the dynamics of the sub–households. In this setting, the basic reproduction number as well as prevalence and the peak of an infection wave in a population with given households size distribution can be computed analytically. We compare numerical simulation results of this novel household–ODE model with results from an agent–based model using data for realistic household size distributions of different countries. We find good agreement of both models showing the catalytic effect of large households on the overall disease dynamics.

Radiographic changes in the operative treatment of acute acromioclavicular joint dislocation : tight rope technique vs. K-wire fixation

Background: Operative treatment of higher degree acromioclavicular joint luxation is common. A new option is made available by the tight rope technique. It claims to provide adequate outcome with the use of a minimally invasive technique. First clinical studies justified its medical use, but the equivalence to established surgical methods remains unclear. We therefore analyzed radiographic data from patients that were treated with the tight rope system (TR) and compared them to those treated with K-wires (KW) fixation. Material/Methods: Retrospective study with inclusion criteria: surgery for acromioclavicular joint luxation between 2004 and 2011, classified as Rockwood type III, no concomitant injury, first event injury. We compared pre- and post-operative X-rays with those taken at the end of treatment. Clinical data from follow-ups and radiographic data were evaluated. The main outcome variable was the remaining distance between the acromion and clavicle (ACD), as well as the coracoid process and clavicle (CCD). Results: 27 patients (TR: n=16; KW: n=11) with comparable demographics and injury severity were included. Surgery reduced ACD (TR: p=0.002; KW: p<0.001) and CCD (TR: p=0.001; KW: p=0.003). Heterotopic ossification or postoperative osteolysis was not significantly associated with either one of the procedures. Three patients (18.75%) in the TR group showed impaired wound healing, migrating K-wires were recorded in 2 patients (18.2%) and impingement syndrome occurred in 1 patient (9.1%) with K-wires. Posttraumatic arthritis was not seen. There was a loss of reduction in 2 cases within the TR-group (12.51%) and 1 in the KW-group (9.1%). At last follow up, ACD and CCD were wider in both groups compared to the healthy side. Conclusions: This study shows that the Tight rope system is an effective alternative in the treatment of higher degree acromioclavicular luxation and comparable to the established methods.

Purpose: Old (>64 years) and very old (>79 years) intensive care patients with sepsis have a high mortality. In the very old, the value of critical care has been questioned. We aimed to compare the mortality, rates of organ support, and the length of stay in old vs. very old patients with sepsis and septic shock in intensive care. Methods: This analysis included 9,385 patients, from the multi-center eICU Collaborative Research Database, with sepsis; 6184 were old (aged 65–79 years), and 3,201 were very old patients (aged 80 years and older). A multi-level logistic regression analysis was used to fit three sequential regression models for the binary primary outcome of ICU mortality. A sensitivity analysis in septic shock patients (n = 1054) was also conducted. Results: In the very old patients, the median length of stay was shorter (50 ± 67 vs. 56 ± 72 h; p < 0.001), and the rate of a prolonged ICU stay was lower (>168 h; 9 vs. 12%; p < 0.001) than the old patients. The mortality from sepsis was higher in very old patients (13 vs. 11%; p = 0.005), and after multi-variable adjustment being very old was associated with higher odds for ICU mortality (aOR 1.32, 95% CI 1.09–1.59; p = 0.004). In patients with septic shock, mortality was also higher in the very old patients (38 vs. 36%; aOR 1.50, 95% CI 1.10–2.06; p = 0.01). Conclusion: Very old ICU-patients suffer from a slightly higher ICU mortality compared with old ICU-patients. However, despite the statistically significant differences in mortality, the clinical relevance of such minor differences seems to be negligible.
